Patient Zhu contracted epidemic pestilence turbid pathogens (turbid pathogens within epidemics). The pathogens were inhaled through the mouth and nose, distributed throughout the Sanjiao (Triple Burner), and diffused to cloud the Shen (Spirit/Mental Consciousness). This is not an external pathogen of Wind-Cold, nor is it a retention syndrome in the interior. Therefore, using methods to disperse the exterior or promote digestion would violate the prohibition against depleting Body Fluids. It differs significantly from the Six Meridian differentiation of Cold Damage diseases. Currently, the patient has sore throat, Dan Zhen (erythematous papules), a tongue red like cinnabar, restlessness of spirit, and evening drowsiness. This indicates that upper-accepted turbid pathogens have reversed course and moved into the Shanzhong (Pericardium region). One should clear the Blood collaterals to prevent obstruction and closure. However, large doses of detoxifying herbs must be used to drive out the turbidity. It is hoped that after nine days, deep coma will be avoided, anticipating that once the pathogen is expelled, the Healthy Qi will recover.

Sudden cold attacks exacerbate latent heat, making it more intense. Pathogen stagnation causes Qi and Blood congestion and obstruction, preventing Sha Zhen (scarlet rash) from reaching the exterior. Phlegm and Qi obstruct each other, leading to mental confusion, panting, and shortness of breath. The condition gradually enters the Pericardium Collateral, raising the concern of internal closure and external collapse. Heat pouring downward forces spontaneous diarrhea with sticky, unsmooth discharge. The treatment method should open the obstruction and detoxify to relieve the congestion in the Shanzhong. Only when the spirit becomes clear can one ensure no further complications.
Lianqiao Xin (Forsythia Heart), Fei Huashi (Prepared Talcum Powder), Shichangpu (Acorus Rhizome), Chao Jinyinhua (Stir-fried Honeysuckle Flower), Shegan (Belamcanda Rhizome), Tongcao (Tetrapanax Pith), decocted with Niuhuang Wan (Calculus Bovis Pill).
The edges of the tongue are red; there is mental confusion and delirious speech, with symptoms lighter in the morning and heavier at night. Maculae and rashes appear intermittently. This indicates that Warm-Damp pathogens have entered the Blood collaterals. Since the Heart governs Blood, and pathogens interfere with the Shanzhong, gradual obstruction leads to the danger of coma and convulsions. Bitter, sinking, and cold herbs entering the Middle Jiao directly, and pungent-warm digestive herbs merely depleting Stomach fluids, are all major taboos in treating Warm pathogens. It is proposed to clear and diffuse the Blood level, using light agents to vent the maculae. Aromatic herbs should be added to expel turbidity and open the internal orifices. The prohibitions clearly stated by the modern physician Yu Jiayan should be observed.

Neck lymph nodes swell with heat, appearing only on the front of the face. It worsens significantly in September and October, and improves in May and June, persisting for seven or eight years. Considering that in summer, Yang Qi disperses and spreads, allowing Ying (Nutritive) and Wei (Defensive) Qi to flow without interruption; whereas in autumn, the climate becomes cold and thin, causing Qi and Blood to stagnate. This damp-heat, lacking release, results from the stagnation of Shaoyang Wood-Fire and the steaming dampness of Yangming. Thus, the Upper Jiao is particularly severe. The treatment method uses pungent-cool herbs as the mainstay, assisted by bitter-cold herbs, so that the stagnant heat in the Yang level can be dispersed, hopefully slowing the intensity of the episodes.
