Jinghua (Spermatorrhea with slippery discharge) and Bianzhuo (Turbid Urine) are different conditions. Bianzhuo refers to turbid urine, which is the Gaolin (Gao-pulse/Chalky strangury) among the Five Stranguries. It is caused by damp-heat in the Stomach seeping into the Bladder and has absolutely no connection with the Kidney Meridian. Jinghua, on the other hand, presents with a sticky, viscous sensation of sperm leakage; even without urination, it occurs frequently. This is due to insufficient Kidney Water, allowing Ministerial Fire (Xianghuo) to move easily, causing the essence to leave its proper position and thus gradually accumulate and flow out. Treatment should focus on nourishing the Kidney Yin, clearing Heart Fire, strengthening the Spleen, and consolidating to prevent collapse. Although Jingzhuo belongs to the fluids of the Kidney, the Heart is the master of Jingzhuo. Once the Sovereign Fire (Junhuo, i.e., Heart Fire) moves recklessly, true essence is lost; being able to calm the Mind (Shen) is the great elixir of one's body. Furthermore, examining the *Qianjin Fang* (Essential Prescriptions Worth a Thousand Gold), there are nine formulas for treating spermatorrhea and turbid urine, half of which use Jiu Zi (Allii Tuberosi Semen). Jiu Zi is acrid and hot; why did Sun Simiao value it so deeply? It is likely that when used to treat Bianzhuo, its acrid-hot nature is utilized to burn away dampness in the Earth (Spleen), causing steaming heat to ascend rather than descend. This is the method of "adding fire to the bottom of the cauldron," namely benefiting the source of Fire to dissipate Yin shadows. When used to treat spermatorrhea, its acrid-hot nature is used to strengthen True Yang, enabling it to contain Yin Essence without leakage. This is the method of benefiting Earth to protect against water overflow, guarding the exterior and consolidating. These untransmitted secrets can only be discussed with those who understand.
Spermatorrhea (Yijing) refers to spontaneous emission without sexual intercourse; dream emission (Mengyi) refers to emission during sexual dreams. Both exit through the essence pathway. Although essence is stored in the Kidneys, it is actually governed by the Heart. Even if driven by evil arts, such as dreaming of intercourse with others, or sensing ghostly pregnancies at night, these may not necessarily be unrelated to deficiency of both Heart and Kidney, allowing evil pathogens to enter due to deficiency. Why? Because the Heart stores Shen (Spirit), and the Kidneys store Jing (Essence); the Heart and Kidneys are the roots of Spirit and Life. Whenever men overthink, Water and Fire fail to interact; indulging in lust causes Yuanjing (Primordial Essence) to lose its guard. Therefore, when the Heart moves, Shen wanders; when Shen wanders, Jing scatters. Because of daytime thoughts, nighttime dreams occur. Nowadays, people often use astringent agents like Long Gu (Fossilia Ossis Mastodi) and Mu Li (Concha Ostreae) to stop emission, but it stops temporarily and recurs. They only know how to consolidate the Kidney but do not know how to treat the Heart. Little do they realize that if Shen does not return to its residence, there is no master, and how can it guard itself? Baiyin (White Turbidity) exits from the urinary orifice, resembling oil or paste, sometimes with five-colored luster, red or white, or settling with turbid sediment at the bottom, or resembling semen. Although the discharge route differs from spermatorrhea, the pathogenesis is the same: it is also caused by deficient Heart Blood, blazing internal Ministerial Fire, damp-heat in the Middle Jiao, and unclear lascivious qi. For example, water drawn from a well is very clear, but when boiled, it becomes a white soup; after settling, sediment appears at the bottom. Is this not a manifestation of damp-heat turbidity? Physicians should only differentiate the deficiency/excess severity and the degree of damp-heat to treat accordingly. If there are various deficiency-wasting symptoms, treat based on deficiency. If caused by the Heart, nourish the Heart to gather Shen; if caused by the Kidney, treat the Kidney to astringe Jing; if Zhengqi (Upright Qi) is deficient and dampness prevails, use warming and tonifying formulas for Heart and Kidney; if yin deficiency with heat predominates, use cooling and tonifying formulas for Heart and Kidney; for damp-heat patterns, follow Zhu Danxi's treatment methods; for chronic cases, use herbs that raise and lift. Additionally, there are cases of sperm leakage due to unfulfilled desires, or sperm leakage due to abundant Qi and long-term absence of sexual desire; these are not diseases.
