Patient Tao, aged sixty-eight. Spasticity and contracture of the left limbs, swollen tongue with dysarthria (speech impediment), inability to speak, thick white coating on the tongue, inability to swallow even a drop of water, choking when drinking. This is an excess syndrome of stroke complicated by phlegm. The previous physician mistakenly used yin-nourishing tonification, causing all the meridians and collaterals to be blocked; therefore, the initial treatment was to open the Lung Qi.
Sheng Shigao (Raw Gypsum Fibrosum, 4 liang) Fangji (Stephaniae Tetrandrae Radix, 5 qian) Xingren (Armeniacae Semen Amarum, 4 qian) Jiang Banxia (Pinelliae Rhizoma Preparatum cum Zingibere et Alumine, 5 qian) Fuling Kuai (Poria, 5 qian) Sangzhi (Mori Ramulus, 5 qian)
Chenpi (Citri Reticulatae Pericarpium, 3 qian) Bai Tongcao (Tetrapanacis Medulla, 1.5 qian)
After one dose, liquids could be swallowed; after seven doses, the tongue swelling subsided. After twenty doses, although symptoms gradually decreased, there was no significant effect; left limb spasticity remained unchanged. Although the tongue swelling subsided, speech remained unclear, and the pulse was intermittent (Jie). I stated: "This is blockage of phlegm in the collaterals, caused by mistaken tonification leading to stagnation. Acupuncture is indispensable." Thus, Brother Jia Qiwu was invited to perform acupuncture. One needle at Zhongquan (Ex-HN12) on the tongue discharged half a teacupful of dark purple-black blood. Subsequently, an earthworm-like object emerged, which his son extracted by hand, and a strip of gelatinous phlegm, resembling fine powder, was pulled out from the needle hole, measuring seven to eight cun in length. Another needle was inserted into Zhigou (SJ6) on the left hand, penetrating to the Yang-Wei collaterals on the dorsum of the left Guan area, using more than ten small needles. Thereafter, the herbal medicine showed daily efficacy. Only half the dosage of Raw Gypsum Fibrosum was reduced from the original formula. After taking over seventy doses, the patient walked out of the hall and rode away in a sedan chair independently.
Patient Ha, aged sixty-six. Suffered from wind-dampness strike, facial paralysis (deviation of mouth and eyes), inability to raise arms, leg edema, surging and rapid pulse, thirst, and closed Stomach Qi. Treated with acrid-cool methods to open the water passages.

Efficacy seen after two doses, cured after fourteen doses. Later, tonifying the Spleen and Stomach consolidated the recovery.
Stroke presenting with mental dullness and mutism. Previously, when able to speak, the patient reported dizziness, numbness of the left limbs, severe deviation of the mouth, and anorexia. The six pulses were wiry and rapid. This is Fei disease (Wind Fei/Ataxic Aphasia), treated with softening Liver methods.

After one dose, mental clarity improved slightly, nodding in response to conversation. Three qian of Sheng Ejiao (Asini Corii Colla) and 4 qian of Danpi (Moutan Cortex) were added to the previous formula. After three doses, halting speech was achieved; after seven doses, cured, able to eat; after twelve to thirteen doses, fully recovered.
Patient Li, aged seventy-two. Summerheat lingering with phlegm-fluid retention, Liver Qi stagnation, complicated by stroke, headache, swollen tongue with white coating, dysarthria, aversion to cold, surging, rapid, and wiry pulse. Initially treated with acrid-cool methods to clear the Upper Jiao.

After four doses, headache and aversion to cold ceased, tongue swelling gradually subsided, but the coating remained. At this stage, the method shifted to unblocking and dispersing the Sanjiao (Triple Burner), combined with resolving Liver stagnation.
Fuling (Poria, 5 qian, with peel) Xingren Ni (Armeniacae Semen Amarum Paste, 5 qian) Banxia (Pinelliae Rhizoma, 4 qian) Baikou Ren (Amomi Fructus Rotundus, 2 qian) Fei Huashi (Talcum, 6 qian) Xiangfu (Cyperi Rhizoma, 2 qian) Tongcao (Tetrapanacis Medulla, 1 qian) Guang Yujin (Curcumae Radix, 2 qian) Yiren (Coicis Semen, 5 qian)
