Mr. Guo, male, eight years old. On the 11th day of the 7th lunar month in the Guihai year, he presented with cough accompanied by vomiting, which is Stomach Cough (cough caused by pathogenic factors affecting the Stomach organ). There was excessive phlegm and saliva obstructing the airways, wheezing, chest fullness, and shortness of breath.
Prescription: Banxia (Pinelliae Rhizoma, 3 qian), Xiao Zhishi (Immature Bitter Orange, 1 qian), Chenpi (Aged Tangerine Peel, 1 qian), Xingren (Apricot Seed, 2 qian), Sugeng (Perilla Stem, 2 qian), Sheng Yiyiren (Coix Seed, 3 qian), Shengjiang (Fresh Ginger, 2 qian), Fuling (Poria, 3 qian).
Subsequently, modifications were made to the above prescription:
Added: Ku Tinglizi (Lepidium Seed, 1.5 qian), Banxia (Pinelliae Rhizoma, 2 qian), Suzi (Perilla Fruit, 2 qian).
Removed Sugeng, administered for two more doses.
On the 20th day: The child has Spleen deficiency with heavy dampness leading to Stomach Cough.
Prescription: Jiang Banxia (Ginger-processed Pinellia, 6 qian), Jiao Qu (Charred Medicated Leaven, 2 qian), Xiao Zhishi (Immature Bitter Orange, 1.5 qian), Xingni (Apricot Paste, 3 qian), Xuanfuhua (Inula Flower, 3 qian, wrapped in silk), Suzishuang (Perilla Frost, 1.5 qian), Fuling (Poria, 3 qian), Sheng Baibian Dou (Raw White Hyacinth Bean, 3 qian), Sheng Yiyiren (Coix Seed, 5 qian), Shengjiangzhi (Fresh Ginger Juice, three small spoons mixed each time).
Subsequently, modifications were made to the above prescription:
Added: Guangpi (Broad-leaved Aged Tangerine Peel, 2 qian), Xingren (Apricot Seed, 2 qian), Suzishuang (Perilla Frost, 1.5 qian).
Removed Shenqu (Medicated Leaven).
Total ten doses.
The three young children of the Wu family, aged three, five, and eight, suffered from continuous coughing spells lasting dozens of times. The eight-year-old also experienced epistaxis (nosebleed). Qianjin Weijing Tang (Decoction for Thousand Gold Willow Stem) was prescribed with added Ku Tinglizi (Lepidium Seed, 3 qian). Some recovered after two doses, others after three or four doses. In the third or fourth dose, the amount of Tinglizi was halved; for those with epistaxis, Baimaogen (Imperata Rhizome, 5 qian) was added.
Patient Wen, four years old. A young child suffered from choking cough for dozens of days without effect from various medicines. Qianjin Weijing Tang (Decoction for Thousand Gold Willow Stem) with added Ku Tinglizi (Lepidium Seed) was used, resulting in recovery after two doses.
Ms. Zhou, ten years old. Choking cough induced by spring wind. The physician had used Mahuang (Ephedra) for outward dispersal, combined with Hezi (Chebula Fruit), Baiguo (Ginkgo Seed), and Baihe (Lily Bulb) for inward astringency, causing unresolvable choking cough. The expectorated material consisted entirely of bloody foam. Jinfeicao Tang (Decoction for Golden Fern Grass) was prescribed, resulting in recovery after three doses.
Patient Liu, seventeen years old. On the 24th day of the 5th lunar month in the Yiyu year. In March, Spring Warmth pathogen invaded, causing choking cough and vomiting. Currently, the six pulses are wiry and fine. During the early morning hours of Chou, Yin, and Mao (1:00 AM - 7:00 AM), single-burst coughing is severe, termed "Wood striking Metal" (Liver Wood Qi impacting Lung Metal). Wind originates from Liver Wood. The proposed treatment is to transform wind using acrid-sweet methods and soften the Liver using sweet-cool methods.
Prescription: Ku Jiegeng (Bitter Platycodon Root, 3 qian), Lianqiao (Forsythia Fruit, 3 qian), Yinhua (Honeysuckle Flower, 2 qian), Gancao (Licorice Root, 2 qian), Bohe (Mint, 1 qian), Xian Lugeng (Fresh Reed Rhizome, 3 qian), Sangye (Mulberry Leaf, 3 qian), Maidong (Ophiopogon Tuber, 3 qian), Xi Shengdi (Fine Rehmannia Root, 3 qian), Chaju (Tea Chrysanthemum, 3 qian), Tiandong (Asparagus Tuber, 1 qian).
On the 28th day: Cough decreased, appetite increased, but the pulse remains surging and rapid, with the left pulse larger than the right. Efficacy confirms the method; do not change the formula, administer another four to five doses.
On the 2nd day of the 6th lunar month: Wood striking Metal ringing, treatment with softening Liver and clearing Lung has been effective. The surging and rapid left pulse has decreased. Remove acrid herbs acting on the Qi level from the previous formula, add sweet-moistening agents.
Prescription: Shashen (Glehnia Root, 3 qian), Yuzhu (Polygonatum Rhizome, 3 qian), Maidong (Ophiopogon Tuber, 3 qian), Bingtang (Rock Sugar, 3 qian).
Ms. Li, four years old. On the 10th day of the 2nd lunar month in the Jichou year. Wind-Warmth complicated by Phlegm-Fluids causing wheezing and cough. Abdominal heat is severe, condition critical. Tentatively applied methods to diffuse Lung collaterals and clear Lung heat.
Prescription: Sheng Shigao (Raw Gypsum, 2 qian), Xingren (Apricot Seed, 5 qian), Huangqin (Scutellaria Root, 3 qian, stir-fried), Ku Tingli (Lepidium Seed, 3 qian), Lugeng (Reed Rhizome, 5 qian).
On the 12th day: Wind-Warmth complicated by Phlegm-Fluids causing wheezing and cough.
Prescription: Sheng Shigao (Raw Gypsum, 2 liang), Xingren (Apricot Seed, 4 qian), Dongguaren (Winter Melon Seed, 3 qian), Lugeng (Reed Rhizome, 5 qian), Fulingpi (Poria Skin, 3 qian), Ku Tingli (Lepidium Seed, 1.5 qian).
Boil into three small cups, divide into three doses. After two doses, fever subsided.
Patient Wu, twenty years old. On the 24th day of the 4th lunar month in the Jiazi year. Six pulses are wiry and forceful, showing Yin without Yang. Only coughing without phlegm. Clear Upper Jiao Qi-level heat.
Prescription: Sangye (Mulberry Leaf, 3 qian), Sheng Bian Dou (Raw Broad Bean, 3 qian), Yuzhu (Polygonatum Rhizome, 3 qian), Bingtang (Rock Sugar, 3 qian), Maidong (Ophiopogon Tuber, 3 qian), Shashen (Glehnia Root, 3 qian), Xingren (Apricot Seed, 3 qian), Lianqiao (Forsythia Fruit, 1.5 qian), Chaju (Tea Chrysanthemum, 3 qian).
Four doses.
On the 26th day: From the previous formula, remove Lianqiao (Forsythia Fruit), add:
Danpi (Moutan Bark, 2 qian), Digupi (Cortex Lycii, 3 qian).
Patient Chen, forty years old. On the 13th day of the 1st lunar month in the Bingxu year. Cough started in September of the previous year. Summer injury by dampness, latent Summer-Heat triggered by new coolness caused the cough. The symptoms were originally mild, but later due to mistaken tonification sealing the exterior, pathogens became difficult to expel. Then Sangbai Pi (Morus Bark) powder was used with Digupi (Cortex Lycii) to guide pathogens into the Kidney. According to theory, the Kidney is an organ of storage; pathogens guided in erroneously can never be expelled again. Body heat resolved through sweating induced by tonifying drugs, but heat in the soles of the feet remained unresolved, which is definitive evidence. Currently, cough with vomiting, six pulses wiry, fine, and rapid, indicating both Yin and Yang deficiency. Tentatively follow the method for treating Stomach Cough, first allowing the patient to eat, establishing Middle Jiao function. If Stomach Qi becomes robust, there may be hope for life. Frequent expectoration of one or two mouthfuls of blood containing stasis indicates collateral damage from long-term illness. Pain in the hypochondriac region corresponds to the Liver meridian area; should add methods to diffuse collaterals and descend Qi.
Prescription: Jiang Banxia (Ginger-processed Pinellia, 6 qian), Suzishuang (Perilla Frost, 1.5 qian), Taoren (Peach Kernel, 3 qian), Yunling (Poria, 8 qian, can increase to 1 liang if vomiting persists), Jiangxiangmo (Aquilaria Wood Powder, 2 qian), Guangpitan (Charred Broad-leaved Aged Tangerine Peel, 3 qian), Jiangzhi (Ginger Juice, three small spoons mixed per cup).
Boil into three cups, divide into three doses. This condition is like stirring boiling water to stop it from boiling; it is impossible to remove firewood from under the pot (address the root cause).
Patient Chen, sixteen years old. A youth with inherently weak constitution. Six pulses are wiry, fine, and soft. Coughing at dawn, occasional hemoptysis (coughing up blood). Should be treated according to the theory of Yang deficiency complicated by Phlegm-Fluids causing hemoptysis. Following the principle "depletion should be warmed," Xiaojianzhong Tang (Minor Construct the Middle Decoction) with added Fuling (Poria) and Banxia (Pinellia) was given.
Prescription: Baishao (White Peony Root, 6 qian, stir-fried), Jiang Banxia (Ginger-processed Pinellia, 3 qian), Shengjiang (Fresh Ginger, three large slices), Guizhi (Cinnamon Twig, 4 qian), Yunling (Poria, 5 qian), Jiaoyi (Maltose, 8 qian, dissolved), Zhi Gancao (Honey-processed Licorice Root, 3 qian), Dazao (Jujube, two pieces, pitted).
Taking more is beneficial.
Patient Mou, thirteen years old. Dry cough at dawn, Wood striking Metal ringing. Originally intended to use softening medicine to soften the Liver. Now with pain in both hypochondria, indicating anger-induced depression and stasis. The method should be to invigorate collaterals.
Prescription: Xinjiangsha (New Crimson Gauze, 3 qian), Suzishuang (Perilla Frost, 2 qian), Guangpi (Broad-leaved Aged Tangerine Peel, 2 qian, stir-fried), Xuanfuhua (Inula Flower, 3 qian, wrapped), Jiangxiangmo (Aquilaria Wood Powder, 2 qian), Jiang Banxia (Ginger-processed Pinellia, 5 qian), Guixu (Angelica Sinensis Tail, 3 qian), Yujin (Curcuma Tuber, 2 qian), Qingpi (Green Tangerine Peel, 1.5 qian), Xiangfu (Cyperus Rhizome, 3 qian).
