Abdominal pain that alleviates after defecation, accompanied by tenesmus (a feeling of incomplete evacuation), characterizes dysentery. The treatment principle should be warm purgation (Wen Xia). If one hesitates and delays, the condition will inevitably worsen without a favorable outcome. It is better to seize the moment when pathogenic factors are just gathering and urgently eliminate them, hoping that once pathogens are gone, healthy qi remains, thereby allowing for the resolution of all unresolved issues.
Bai Shao (Radix Paeoniae Alba, 3 qian) Bin Lang (Areca Seed, 3 qian) Sheng Da Huang (Raw Radix et Rhizoma Rhei, 5 qian, stir-fried with wine until half-black) Chao Huang Qin (Scutellariae Radix, stir-fried, 3 qian) Zha Tan (Charred Hawthorn Fruit, 3 qian) Shu Fu Zi (Prepared Aconiti Lateralis Radix, 2 qian) Chao Lian Xin (Coptidis Rhizoma, stir-fried, 2 qian) Shen Qu (Medicated Leaven, 4 qian) Guang Pi Tan (Charred Citri Reticulatae Pericarpium, 3 qian) Xiao Zhi Shi (Immature Bitter Orange Fruit, 3 qian) Chi Gui (Cinnamomi Ramulus, 1.5 qian)
Decoct into three cups. Take one cup first. Wait one to two hours. If upon subsequent defecation there is no abdominal pain, do not take more; if abdominal pain persists, take the second cup. The third cup is administered similarly.
Fifth day: Fourteen days since eruption of variola (smallpox). Scabs have formed on the four limbs, covering about 50%. After taking medicine yesterday, abdominal pain intensified. Stools contained more feces and less accumulation (mucus/blood), with seven to eight bowel movements per day and night. Today, using the previous formula but reducing Fu Zi by 1 qian and Gui Xin by 2 fen. Between Si hour (9-11 AM) and Wei hour (1-3 PM), two episodes of red accumulation were passed. Abdominal pain persisted. Feces were ochre-colored. For the next two cups, add Chi Gui Xin by 8 fen. Approximately half a cup was taken. Abdominal pain ceased immediately after defecation, but red accumulation remained. Feces turned yellow. At midnight, the third cup was taken. In Yin hour (3-5 AM), two consecutive bowel movements occurred. Feces remained ochre-colored with slight red accumulation. Abdominal pain was still mild.
Sixth day: Fifteen days since eruption. Scabs below the knees to the toes are not yet fully formed. One bowel movement at Si hour: dry feces yellow mixed with ochre; loose stool with slight sticky red accumulation; no abdominal pain. First cup taken at Wu hour (11 AM-1 PM). One bowel movement at Hai hour (9-11 PM): yellow feces, no accumulation. One bowel movement at Chou hour (1-3 AM): no accumulation, yellow feces.

Seventh day: Sixteen days since eruption. Scabs are completely formed. Dysentery has recovered. No further medication needed. Eyes show slight swelling. Drink Gu Jing Cao (Euphrasiae Herba) tea.
Eighth day: Cloudy membrane (Yun Yi) appears on the cornea (Qing Jing). Quickly clear heat-toxin from the Gallbladder collaterals.

Ninth day: Variolous pus is insufficient. Toxins flow into the Gallbladder collaterals, causing white opacity on the cornea. Additionally, seasonal dryness transforms into fire, causing Taiyin Jing syndrome (symptoms affecting the eye's white part) on the sclera. Consulting ancient methods, treat with Liu Wei Wan modified as a decoction, making Fu Ling the sovereign herb, plus herbs to clear heat-toxin from the Gallbladder collaterals to reduce opacity.
Yun Ling Kuai (Poria, 4 qian) Shan Yao (Dioscoreae Rhizoma, 1.5 qian) Qing Xiang Zi (Veronicae Semen, 2 qian) Ze Xie (Alismatis Rhizoma, 1.5 qian) Gu Jing Cao (Euphrasiae Herba, 3 qian) Yu Rou (Corni Fructus, 1.5 qian) Zhen Da Sheng Di (Rehmanniae Radix Recens, 2 qian) Cha Ju Hua (Chrysanthemi Indici Flos, 2 qian) Dan Pi (Moutan Cortex, 2 qian) Sang Ye (Mori Folium, 2 qian)

Sheng Gan Cao (Raw Glycyrrhizae Radix et Rhizoma, 1.5 qian) Yin Hua (Lonicerae Japonicae Flos, 5 qian) Lian Qiao (Forsythiae Fructum, 3 qian)
The white opacity in the eyes recedes slightly. Irritability and frequent crying occur due to blood deficiency transforming into dryness after smallpox. Prescribe Gan Mai Da Zao Tang (Licorice, Wheat, and Jujube Decoction).
Sheng Gan Cao (Raw Glycyrrhizae Radix et Rhizoma, 1.5 qian) Xiao Mai (Tritici Fructus, 7 he) Da Zao (Jujubae Fructus, 5 pieces)
Cook as porridge and consume.
Eleventh day: White opacity in the eyes persists. Dozens of large and small sores appear on the body. Fine pustules re-emerge within old scab pits. Due to insufficient pus, toxins flow and form sores. Continue taking the Ninth Day Formula.
Twelfth day: White opacity in the eyes recedes. Taiyin Jing sores remain. Sores have not subsided. Continue taking the original formula.
Thirteenth day: Taiyin Jing sores in the eyes persist. Newly erupted sores/pustules have not resolved. Continue taking the original formula. Apply Zi Cao Gao (Arnebiae Unguent) to sores and add rotten grass ash.
Fourteenth day: Medication same as original formula.
Fifteenth day: Stop medication.
Sixteenth day: Taiyin Jing sores in the eyes recede slightly, but opacity remains. Sores on the body that had scabbed over recur. Small sores that had not fallen off show signs of inverted pus and impending rupture. This is fundamentally due to uneliminated flowing toxins. Continue taking the original formula. Treat ocular opacity with Si Tui San (Four Shedding Powder).
Seventeenth to Twenty-third days: More than a month since eruption. Taiyin Jing sores in the eyes are not fully healed; opacity remains. Continue taking the original formula. Also administer Qian Shi Chan Tui San (Master Qian's Cicada Slough Powder), twice daily.
Chan Yi (Periostracum Cicadae) powdered. Take 1 qian each time, washed down with Sheep Liver Soup. Twice daily.
Si Tui San (Four Shedding Powder) Main indication: Old opacity of the eyeball.

Complete recovery after two months.
Zhao Gu (Aunt Zhao) June 15, Yihyou Year. Constitution is solid, few pox. Originally, medication might not have been necessary. However, fewer pox mean pus must be sufficient. Tongue coating is thick, yellow in center, white at edges. Administer one dose of Qing Du (Clear Toxin) today. Tomorrow, administer one dose of Tuo Jiang (Support Pus).
Ku Jie Geng (Platycodonis Radix, 1 qian) Ren Zhong Huang (Indicum Humanis Excrementum Purificatum, 8 fen) Yin Hua (Lonicerae Japonicae Flos, 3 qian) He Ye Bian (Nelumbinis Folium Margin, 1 corner) Niu Bang Zi (Arctii Fructus, 1.5 qian) Quan Gui (Angelicae Sinensis Radix, Whole, 1.5 qian) Lian Qiao (Forsythiae Fructum, 3 qian)

Seventeenth day: Acrid-cool resolution leading to scabbing is the orthodox method of the ancients. Strengthening Spleen and promoting urination is suitable for dampness cases. Currently in summer month. Tongue coating is thick and white, indicating dampness. Body heat has not fully subsided, indicating heat. Both methods can be combined.
Lian Qiao (Forsythiae Fructum, 3 qian) Yun Ling Kuai (Poria, 3 qian, with skin) Yin Hua (Lonicerae Japonicae Flos, 3 qian) Sheng Yi Ren (Coicis Semen, 3 qian) Lu Gen (Phragmitis Rhizoma, 3 qian)
