For metrorrhagia (uterine bleeding), when blood loss leads to Qi deficiency, it is appropriate to tonify Qi. Initially, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) by removing Danggui (Angelicae Sinensis Radix) and adding Paojiang (Prepared Zingiberis Rhizoma); if accompanied by lower back pain, add Duzhong (Eucommiae Cortex) and Xuduan (Dipsaci Radix). Subsequently, switch to Shiquan Dabu Tang (Ten Substance Great Tonifying Decoction), slightly adding blood-nourishing herbs, and a small amount of Chenpi (Citri Reticulatae Pericarpium) to regulate Qi and open stagnation.
When metrorrhagia presents as heavy flow initially followed by light flow, it indicates Blood Heat; when it starts light and becomes heavy, it indicates Kidney Deficiency. This is because the Kidney governs the two orifices (urination and defecation) and controls opening and closing. Metrorrhagia occurring after an illness belongs to the Spleen rather than the Kidney. Use Renshen (Ginseng Radix et Rhizoma) and Huangqi (Astragali Radix) each 7 fen, Gancao (Glycyrrhizae Radix et Rhizoma) 5 fen, Baishao (Paeoniae Radix Alba) 1 qian, Paojiang (Prepared Zingiberis Rhizoma) and Wuweizi (Schisandrae Fructus) each 4 fen, Shudi (Rehmanniae Radix Preparata) 8 fen, decocted and taken orally. If metrorrhagia persists for two or three days without improvement, use Baoyuan Tang (Protect the Origin Decoction) added with Ejiao (Asini Corii Colla), Xuduan (Dipsaci Radix), Duzhong (Eucommiae Cortex), and Aye (Artemisiae Argyi Folium), decocted and taken orally.
For treating metrorrhagia, Huangqi (Astragali Radix) stir-fried with vinegar until blackened and processed into a paste is effective. Alternatively, take Xiangfu (Cyperi Rhizoma) 2 liang, Huaihua (Sophorae Flos) 4 qian, Lianziqiao (Nelumbinis Nuciferae Epicotyl) charred to retain its properties 3 qian, formed into pills with old rice paste, and taken down with rice water. First, take Shan Yao (Dioscoreae Rhizoma) 3 qian ground into powder, taken down with wine. After consuming five liang, begin taking the pills; this method is extremely effective.
In cases of metrorrhagia leading to syncope, use Can Sha (Bombyx Batryticatus) 1 liang, Ejiao (Asini Corii Colla) 1 liang, Fulonggan (Dragon's Liver) 5 qian, ground into fine powder, and taken down with warm wine in doses of 3 qian, stopping when the patient regains consciousness.
After metrorrhagia, accumulated blood may form masses (abdominal masses). Use Weilingxian (Syneilesidis Herba) 1 liang, Nanting (Arisomatis Rhizoma), Gansui (Euphorbiae Hussonis Radix), Baijiezi (Sinapis Semen) each 5 qian, Fulonggan (Dragon's Liver) 1 liang, and Shexiang (Moschus) 6 li, ground into powder, formed into pills weighing 1 qian each, coated with Zhusha (Cinnabar). When symptoms occur, dissolve in wine and take orally; recovery occurs after three to five doses. This condition causes unbearable pain.
Clinical Case:
A female patient suffered from continuous uterine bleeding due to the Spleen's failure to control blood. Treating only the Spleen would be ineffective. Since the blood flows downward, the Lower Jiao (Lower Burner), including the Small Intestine and Bladder, become insecure, causing Heart Fire to become restless. When the Heart is restless, pathogenic fire overflows into the Small Intestine, preventing blood from ascending and causing it to flow downward. The downward flow occurs because Qi of the Bladder fails to ascend, leading to stagnation of turbid Qi, thus blood circulates in the Lower Jiao. It is suitable to regulate with Guipi Tang (Restore the Spleen Decoction).
Leukorrhea (vaginal discharge) belongs to deficiency of both Spleen and Lung, requiring warming the Lung and nourishing the Spleen. If treatment is delayed, it will inevitably transform into tidal fever and other symptoms, making it difficult to treat. It is suitable to use Baoyuan Tang (Protect the Origin Decoction) to warm Lung Qi; add Paojiang (Prepared Zingiberis Rhizoma) for abdominal pain, and Aye (Artemisiae Argyi Folium) and Ejiao (Asini Corii Colla) for distending pain. For red leukorrhea (bloody vaginal discharge), use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Chishao (Paeoniae Radix Rubra) and Honghua (Carthami Flos); for mixed red and white leukorrhea, use Bawei Wan (Eight-Ingredient Pill). Red sunflower flowers treat red leukorrhea, white sunflower flowers treat white leukorrhea; grind into powder and take down with wine.
Heat entering the Small Intestine results in red leukorrhea, while heat entering the Large Intestine results in white leukorrhea; both indicate deficiency of the Kidney pulse. It is suitable to use Koushenzi (Meliae Toosendan Fructus), Xiaohui (Foeniculi Fructus), and Guishen (Angelicae Sinensis Radix) each 5 qian, formed into pills with wine paste, and taken down with warm wine on an empty stomach.
Clinical Case:
A female patient felt hungry, had variable amounts of leukorrhea, mental dullness and fatigue, and dizziness. This is a disease caused by Phlegm. It requires strong lifting and raising methods. Use raw Baizhu (Atractylodis Macrocephalae Rhizoma) 2 liang, Renshen (Ginseng Radix et Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma), Chuanxiong (Ligustici Chuanxiong Rhizoma), Sharen (Amomi Fructus Rotundus), and Chenpi (Citri Reticulatae Pericarpium) each 5 qian, Banxia (Pinelliae Rhizoma), Baishao (Paeoniae Radix Alba), Fuling (Poria), and Guishen (Angelicae Sinensis Radix) each 1 liang, formed into pills with honey.
