As the saying goes: Yin is often insufficient, while Yang is often in excess. Therefore, when humans fall ill, if it begins with internal injury to the primordial Qi due to the Seven Emotions (Joy, Anger, Worry, Thought, Sorrow, Fear, Fright), and subsequently affects Blood and Qi, this is a progression from the interior to the exterior. In such cases, Yin syndromes account for eight or nine out of ten, while Yang syndromes account for only one or two out of ten. If the illness arises from external pathogenic factors that first damage Qi and Blood, and then affect the interior, then Yang syndromes are more common than Yin syndromes.
The Classics state: Males reach the stage of seminal emission around the age of sixteen. This is the general rule. However, some individuals who have not yet reached this age may have their essence forcibly released by others. Consequently, within the four limbs and body, Yin Qi will be deficient in certain areas, leading to difficult-to-describe symptoms in later years. Some individuals constantly feel aversion to cold. This is because the Kidney stores Essence and belongs to Water; when Kidney Water dries up and fails to control Fire, the Fire blazes and scorches Yin, causing Yin deficiency with hyperactive Fire, which manifests as aversion to cold. This is not true aversion to cold. Rather, extreme Fire resembles Water; thus, the body feels lethargic and sluggish, experiencing shivering chills that mimic aversion to cold. However, this condition has no fixed timing; it may occur during rest and worsen with activity, reflecting increased deficiency. If the mind rests slightly and achieves tranquility, the Fire lies dormant, and the cold sensation resolves upon warming. When physical activity occurs, fever arises; when at rest, the fever ceases. During cessation, the palms and soles feel hot like iron branding, or there may be facial flushing, tinnitus, sore throat, macules and rashes appearing and disappearing rapidly, or sudden heat in the chest and inner aspects of the feet, resembling lightning strikes. These sensations come and go unpredictably. The breath exhaled through the nose and mouth may sometimes carry a burnt, putrid odor. Sleep is restless accompanied by groaning; lips are red, and the tongue coating is scorched, dry, and black. The soles of the feet feel as if roasted. Qi rises from below the navel into the abdomen. There may be burning sensations in the lips, tongue, lower abdomen, and hypochondrium. Lower back pain may feel as if the spine is broken. Bone weakness prevents rising from bed. Generalized body pain occurs. The root of the tongue becomes stiff, rendering speech impossible; this is because the Shaoyin meridian connects to the root of the tongue. Tinnitus may sound like waves, cicadas, magpies, or musical instruments. Ear blockage and deafness may occur. At night, there are many dreams; sleep is disturbed by fright and unrest, with soul and spirit wandering. The patient prefers darkness, fears light, and avoids people. Hallucinations may include seeing rats scurrying, snakes crawling, or children playing. The patient may feel as if limbs are detached, or the head feels cold and falling off. They may perceive companions sleeping beside them, or feel hair being pulled. Various bizarre symptoms vary but are all caused by Kidney deficiency, Yin insufficiency, and hyperactive Fire manifesting abnormally.
This condition should not be mistakenly treated as Phlegm-Fire, nor should harsh, drying, bitter-cold medicines be used hastily. Nor should one rigidly adhere to Zhu Danxi’s method of treating all cases with Si Wu Tang (Four Substances Decoction) plus Zhi Mu (Anemarrhena Rhizome) and Huang Bo (Phellodendron Bark). Zhi Mu, Huang Bo, Huang Qin (Scutellaria Root), and Lian Zi (Coptis Rhizome) drain excess Fire. Used appropriately, they dispel pathogens and restore primordial Qi, thus benefiting the patient. However, if True Yuan (Primordial Qi) is deficient, using these drugs will further drain Tian Zhen (Natural Vitality), leading to diarrhea, poor appetite, weak Stomach Qi, emaciation, and often death, with countless such cases recorded. One must also not rigidly adhere to the theory that "Yang abundance generates Yin" and treat with Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction). Using it for one or two doses may seem effective initially, but long-term adherence is erroneous. For kidney injury due to sexual exhaustion, Bu Shui Yi Yuan Tang (Water-Nourishing and Primordial-Supplementing Decoction) modified is appropriate, leading to recovery. If Yin of Body Fluids is insufficient with hyperactive Fire, fever and aversion to cold occur. If Yin of Marrow and Essence is insufficient with hyperactive Fire, dry mouth and tongue result. If Yin of Ying (Nutritive) Blood is insufficient with hyperactive Fire, nocturnal emission and spermatorrhea occur. The Neijing states: When Heavenly Qi (Yang) is deficient, Earthly Qi (Yin) overcomes it; when Earthly Qi is deficient, Heavenly Qi overcomes it. Aggravation during the day indicates Yang deficiency; aggravation at night indicates Yin deficiency. If Ying Blood is depleted and Wind-Cold is contracted, pathogenic factors enter the Yin level, causing midnight fever. Si Wu Tang (Four Substances Decoction) added with herbs that descend Fire and clear heat is curative. If anger injures the Liver, stirring internal Wind-Heat, causing chaotic Qi and Blood stagnating in the Yin level with fever, soothing the Liver, nourishing Yin, and clearing heat is curative. If diet, fright, and food retention combine to damage Yin Qi, stagnating in the Yin level causing illness, this is Yin level injury presenting with night fever, resembling Yin deficiency with hyperactive Fire, but the syndrome differs. One must investigate the root cause to avoid medical error and premature death. The Classics state: Yang Fire is easy to save, but Yin Fire is hard to support. If True Yin is weak, a hundred doses of medicine and years of effort are required for full recovery. For other excess external pathogens or heat diseases caused by excessive consumption of roasted, greasy, rich foods, Huang Qin, Huang Lian, Zhi Mu, Shi Gao (Gypsum) are usable, even Xiao Bei Mu (Nitratum) and Da Huang (Rhubarb Root and Rhizome) can be considered. Although these can be used for robust individuals, if Yuan Qi is weak, one must first tonify Yuan Qi before dispelling pathogens. If pathogens are strong, simultaneous attack and tonification are needed, assessing deficiency and excess.
Bu Shui Yi Yuan Tang (Water-Nourishing and Primordial-Supplementing Decoction)
Shu Di Huang (Rehmanniae Radix Praeparata, 4 qian), Sheng Di Huang (Rehmanniae Radix), Mai Dong (Ophiopogonis Tuber), Dang Gui (Angelicae Sinensis Radix) (each 2 qian), Bai Shao (Paeoniae Radix Alba), Gan Cao (Glycyrrhizae Radix et Rhizoma) (each 1 qian), Wu Wei Zi (Schisandrae Fructus) (20 grains), Da Zao (Zizyphi Fructus) (3 pieces).
Shu Di Huang greatly tonifies Yin of the Five Zang organs, calms the Shen (Spirit), strengthens the Jing (Essence) vessels, and fills the marrow, serving as the Sovereign herb. Sheng Di Huang nourishes Yin, retreats heat, benefits Jing, and strengthens the Shen. These four herbs are sacred medicines for tonifying primordial Yin. Mai Dong clears the Heart, eliminates irritability, and reduces heat. Combined with Wu Wei Zi, it tonifies primordial Jing, stops thirst, protects Lung Metal, and benefits Kidney Water, preventing Fire pathogens from damaging the Lung. If Fire flares up damaging Metal causing wheezing and cough, Wu Wei Zi should be used cautiously. Gan Cao drains Heart Fire; since the Heart houses the Shen, it descends Fire within the Shen. For palpitations, confusion, and restless sleep, add Suan Zao Ren (Ziziphi Spinosae Semen) 3 qian and Fu Shen (Poria Cum Radix) 1 qian. For severe primordial Yin deficiency, add Shu Di Huang 3 or 5 qian (larger pieces are better). For hyperactive Fire with dryness and heat, add Xi Xin (Asari Radix et Rhizoma) 1-2 fen, Gan Cao 1 qian, Sheng Di Huang 1 qian, and Tong Bian (Child's Urine) half cup. For cough, remove Wu Wei Zi, add Tian Dong (Asparagi Tuber), Mai Dong, Bai He (Lilii Bulbus), Huang Bo, Jie Geng (Platycodonis Radix). If Fire attacks the Pericardium Meridian causing chest stuffiness, double Shu Di Huang and Gan Cao. For dry thirst, double Mai Dong, Wu Wei Zi, and Shu Di Huang. For phlegm discomfort in the chest, reduce Shu Di Huang, remove Wu Wei Zi, add Gua Lou (Trichosanthis Fructus), Bei Mu (Fritillariae Bulbus), Jiang Zhi (Zingiberis Succus Expressus), and Zhu Li (Bambusae Succus). For restlessness, add Bai He and Gan Cao. For abdominal pain and borborygmus due to hyperactive Fire, remove Wu Wei Zi, add Bai Shao and Gan Cao. For mental fatigue, add Shu Di Huang and Suan Zao Ren. For fright, palpitations, and limb ache, add Dang Gui, Di Huang, Suan Zao Ren, Gan Cao, and Fu Ling (Poria). For lower back pain and bone soreness, add Du Zhong (Eucommiae Cortex) and Gu Po Zhi (Astragali Complanati Semen). For hyperactive Fire with rapid digestion, add Xuan Shen (Scrophulariae Radix), Xi Xin, Bai Shao, and Tong Bian. For heat between the Lung acupoint and back resembling cup-fire, coming and going irregularly, add Xuan Shen and Jie Geng. For heat arising from the testicles (Liver Fire), add Chai Hu (Bupleuri Radix) to guide it out. For primordial Yang deficiency, add Ren Shen (Ginseng Radix) and Huang Qi (Astragali Radix); for Jue Ni (Cold Reversal), add Fu Zi (Aconiti Lateralis Radix Praeparata). If Fire attacks the Yang Essence area causing nocturnal emission, add Shan Yao (Dioscoreae Rhizoma), Shan Yu Rou (Corni Fructus), Gou Qi Zi (Lycii Fructus), Xi Xin, Lian Hua Rui (Nelumbinis Stamen), combined with Liu Xian Dan (Six Immortals Pill). If constitution is robust and Zhi Mu is suitable, add it judiciously to prevent Fire from harming Yuan Yang. For post-void boiling drops, use Er Xian Dan (Two Immortals Pill) with Long Gu (Fossilia Ossis Mastodi et Osseum) and Lian Rui (Nelumbinis Stamen). For wandering deficiency Fire with unpredictable macules/rashes and generalized redness/heat, add Xuan Shen and Sheng Di Huang to strengthen the Master of Water to control Yang Light, removing Wu Wei Zi. For shivering chills mimicking aversion to cold, add raw Gan Cao and Tong Bian; do not use cold agents, only nourish Yin, and Fire will subside.
If the Hand-Shaoyin Heart Meridian is congenitally weak, daytime mental labor damages Shen, and nighttime sexual activity damages Jing, Jing-Qi becomes timid and fails to nourish Shen, allowing Fire to attack the Heart Meridian. Symptoms include heat in the palm, tip of the tongue, and sacrum, fluctuating palpitations, restless sleep, fright, waking at Zi hour (11 PM-1 AM), and fatigue at dawn. The center of the tongue is scorched and dry. The little fingers of both hands occasionally feel electric heat, intermittently. Breath feels steaming hot and dry without putrid odor or groaning. Bones and sinews feel steaming hot without wandering sensations, differing slightly from Foot-Shaoyin syndromes. Use Zi Yin Bu Xin Tang (Yin-Nourishing and Heart-Supplementing Decoction).
Shu Di Huang (3 qian), Dang Gui, Guan Gui (Cinnamomi Ramulus), Mai Dong, Sheng Di Huang, Xing Ren (Armeniacae Semen Amarum) (each 1 qian), Bai Shao (2.5 qian), Gan Cao, Fu Shen (each 1 qian), Xiao Mai (Tritici Fructus) (a pinch), Da Zao (3 pieces).
If the Foot-Jueyin Liver Meridian suffers from Fire-Eye swelling suddenly, or [symptoms involving the foot/lower abdomen]...
Shu Di Huang, Sheng Di Huang, Dang Gui (each 1 qian), Bai Shao (1.5 qian), Gan Cao (1 qian), Chai Hu, Xuan Shen (each 8 fen).
Foot-Taiyin Spleen disease may result from excessive thought injuring Shen, irregular diet injuring the Spleen, or uncontrolled anger injuring the Liver, where Liver Wood insults... [Symptoms include feeling limbs detached, visual hallucinations, craving food/easy hunger, or bloating after eating]. All require Yin-nourishing tonification. Even with Fire hyperactivity, avoid bitter-cold herbs; add Pao Jiang (Prepared Zingiberis Rhizoma) as an antagonist in Yin-tonifying formulas. For sallow complexion, lack of muscle luster, mental fatigue, and blood-streaked phlegm, nourish Yin.
Bai Fu Ling (Poria), Fu Shen, Gan Cao, Bai Shao, Sheng Di Huang (each 1 qian), Shan Yao, Dang Gui Shen (Angelicae Sinensis Radix Cortex/Root tip) (each 1.5 qian), Shu Di Huang (3 qian)...
Hand-Taiyin Lung disease may result from sorrow injuring the Lung, causing heat in the back/chest/Lung acupoint, cough, sore throat, hemoptysis, aversion to cold, and heat along the thumb up the shoulder/back to the chest like branding. Use Bai He Gu Jin Tang (Lily Bulb to Preserve the Metal Decoction).
Shu Di Huang, Sheng Di Huang, Dang Gui Shen (each 3 qian), Bai Shao, Gan Cao (each 1 qian), Jie Geng, Xuan Shen (each 8 fen), Bei Mu, Mai Dong, Bai He (each 0.5 qian). For cough, add Wu Wei Zi (20 grains) for the first one or two doses.
For those engaging in sex or excessive mental labor, suffering from month-long insomnia and restless dreams, one must calm the mind and observe inwardly, using Bu Shui Ning Shen (Water-Nourishing and Spirit-Calming) medicines. Or, tonifying Yin may paradoxically increase Fire... This is due to Shen-Thought Fire hyperactivity and True Water insufficiency. One must rescue True Water, and Fire will subside. The Classics say: "When cooling does not cool, it is Water deficiency." Treat with Yuan Shen Gan Cao Tang (Scrophularia and Licorice Decoction) to clear Shen Fire. Once heat subsides slightly, resume Zhuang Shui Yi Yuan Tang (Strengthen Water and Supplement Primordial Qi Decoction). For ear blockage, cough, saliva, groaning, bone weakness, and broken-back pain, use Bu Yuan Yi Yin Tang (Supplement Primordial and Nourish Yin Decoction) assisted by Gui Zhi (Cinnamomi Ramulus). If bone weakness/broken-back pain results from Wind-Damp, Phlegm-Fire, Qi reversal, or Blood stasis, it is outside the scope of Bu Yuan Yi Yin Tang...

For Wind-Heat attacking the Stomach, use Si Wu Tang added with Jing Jie (Schizonepetae Herba), Fang Feng, Sheng Ma, Bai Zhi (Angelicae Dahuricae Radix), Huang Qin... [text missing].
For umbilical bleeding, Fire forces Stomach Qi upward, disrupting transmission, so blood exits from Stomach orifices. Treat with Si Xian Tang added with Gan Cao, Mu Dan Pi, assisted by Bai Zhi, Ou Zhi (Nelumbinis Rhizomatis Succus), Bai Ye (Calami Folium), Mao Gen (Imperatae Rhizoma).
For bleeding from the seven orifices, prolonged stagnation prevents Yin-Blood from returning to its vessels, so it exits through hair follicles. If primordial Yin is weak, Fire attacks Yin level causing boiling blood heat filling skin, use Yin-nourishing and Fire-descending methods. Assess Yuan Qi deficiency/excess, add Blood-cooling herbs.
Ear bleeding often results from anger injuring, disturbing Shaoyin Wind-Heat, causing steaming heat boiling and rushing upward to clear pathways, ascending without descending. Use Sheng Di Huang, Mu Dan Pi, Gan Cao, Chai Hu, Fang Feng, Xiang Fu (Cyperi Rhizoma), Qing Pi (Citri Reticulatae Pericarpium Viride), Zhi Mu, Huang Bo to clear.
For massive hemorrhage, Yuan Yin is inevitably damaged. Use Sheng Di Huang, Mu Dan Pi, Gan Dao (descending Fire) plus Tong Bian.

Gu Zhen San
Shan Yao, Qian Shi (Euryales Semen), Lian Rou (Nelumbinis Semen), Fu Ling equal parts, powdered.
If primordial Yin injury causes vomiting, this is Wood carrying Ministerial Fire attacking Spleen/Stomach. Tonify primordial Yin, suppress Liver, control Fire, clear Spleen. Vomiting stops. Si Wu Tang added with Bai Shao, Gan Cao, assisted by Fu Ling, Qing Pi.
If excessive sexual exertion causes sweat from the Kidney. Classics say: Sweat from Kidney, encountering Wind, traps Wind in the Fu organs. It cannot enter internally nor exit externally, lodging in Xu Fu (pores), causing swelling rooted in the Kidney, named Feng Shui (Wind Edema). Use Zi Shui San Feng (Nourish Water Dispel Wind) with Si Wu Tang added with Qiang Huo (Notopterygii Rhizoma et Radix), Fang Feng, Chai Hu, Jing Jie (Schizonepetae Herba), Fang Ji (Aristolochiae Radix). If someone dies suddenly and revives after a while, with intermittent episodes, this is Heart Fire self-immolation due to mental labor/fright, causing True Shen Qi to lose its position, also Fire attacking Yin. Use Si Xian Tang added with Gan Cao, Tong Bian. Generally, formless Fire is easy to restore, formed Water is hard to support. Thus, True Yin deficiency takes time; how can a cup of water extinguish a cartload of fire? However, if Yuan Qi is deficient, tonify Yuan. Che Che (Human Placenta) cooked and eaten has miraculous effects. Human milk, cow milk can be taken regularly by those with deficiency Fire. Qiu Shi (Mineral Salt) nourishes Yin and descends Fire, also excellent. All Yin deficiency syndromes can select from following formulas.

Classics say: Ying emerges from the Middle Jiao, Wei emerges from the Lower Jiao. Wei deficiency benefits from Acrid, Ying deficiency benefits from Sweet. Acrid and Sweet harmonize Spleen/Stomach; if Spleen/Stomach are strong, what disease remains? Yin Qi: stillness hides the Shen, agitation consumes it. Sweet and Acrid diet is also a way to nourish Yin.
