(Explanation of Seven Symptoms with Diagrams)
Bilateral Tonsillitis (Shuang Rume)
This condition arises from contracting pathogenic factors of the season. The lesions are located in the upper part of the throat gate, with bilateral swellings the size of cherries, representing a disease of the Lung and Stomach. The patient presents with aversion to cold and fever, and the pulse at all six positions is wiry and rapid. Treatment begins with acupuncture at: Shaoshang (LU11), Shangyang (LI11) (four acupoints on both hands). Alternatively, pricking the affected area to induce bleeding is also effective. First, administer Liuwei Tang (Six-Ingredient Decoction) added with: Chenpi (Exocarpium Citri Reticulatae), Haifushi (Pumex), Ziye (Perilla Folium), Qianghuo (Notopterygii Rhizoma et Radix) (each 1.5 qian). Two doses can lead to recovery. If the swelling does not subside and the pulse at all six positions is forceful, Raw Dahuang (Radix et Rhizoma Rhei) (3 qian) may be added.
Unilateral Tonsillitis (Dan Rume)
This condition results from incomplete dispersal following Taiyang Cold Damage (Shanghan). It manifests as body heat, nausea, and fear of appearing sand-rash (shazhen). The pulse at all six positions is floating and rapid. The lesion occurs beside the bilateral tonsils, either on the left or right. Use Liuwei Tang (Six-Ingredient Decoction) added with: Ziye (Perilla Folium) (1 qian), Qianghuo (Notopterygii Rhizoma et Radix) (2 qian), Xianxin Yuansu (Fresh Coriandrum Fructus) (5 qian). If fresh fruit is unavailable, use seeds (3 qian). One dose reduces symptoms by half. On the next day, add: Huangqin (Scutellariae Radix) (wine-fried, 3 qian), Huafen (Trichosanthis Radix) (2 qian), Shanzhi (Gardeniae Fructus) (1 qian), Chishao (Paeoniae Radix Rubra) (1 qian), Mutong (Akebiae Caulis) (1 qian). Full recovery will follow.
Ulcerative Tonsillitis (Lan Rume)
This condition is caused by stagnant heat in the Lung and Stomach, leading to redness, swelling, ulceration, macules, and severe pain, making swallowing difficult. The pulse at all six positions is wiry and tight. Urgently needle: Shaoshang (LU11), Shangyang (LI11) (four acupoints on both sides). Use Liuwei Tang (Six-Ingredient Decoction) added with: Gegen (Puerariae Lobatae Radix) (2 qian), Ziye (Perilla Folium) (1 qian), Xuanshen (Scrophulariae Radix) (salt-water fried, 1 qian), Huangqin (Scutellariae Radix) (wine-fried, 2 qian). Mix with Baizhi Zhi (Cupressi Cacumen Juice) one cup, gargle the throat, and swallow. Then, take one dose of Baxian San (Eight-Immortal Powder), dissolve it in saliva, and swallow. On the next day, remove Ziye (Perilla Folium) and Gegen (Puerariae Lobatae Radix), and add: Shanzhi (Gardeniae Fructus), Mutong (Akebiae Caulis), Shengdi (Rehmanniae Radix), Danpi (Moutan Cortex), Fushi (Pumex), Huafen (Trichosanthis Radix) (each 2 qian). If the pulse is large and forceful, add Raw Dahuang (Radix et Rhizoma Rhei) (3 qian). If the pulse is deficient, use Baxian San (Eight-Immortal Powder) combined with Baizhi Zhi (Cupressi Cacumen Juice) administered as previously described; recovery can be expected in three to four days. If hoarseness and chills in the back occur, add to Liuwei Tang (Six-Ingredient Decoction): Ziye (Perilla Folium) (2 qian), Qianghuo (Notopterygii Rhizoma et Radix) (2 qian), Xixin (Asari Radix et Rhizoma) (0.3 qian).
