Discourse states: There are seven types of Shan disease (abdominal pain and hernia syndromes). If there is Jue-ni (reversal cold), heart pain, cold feet, and vomiting after eating, it is named Jue Shan (Reversal Hernia); if the abdomen is distended with Qi, and there is complete pain below the heart, with Qi accumulation as large as an arm, it is named Jian Shan (Firm Hernia); if drinking cold fluids causes complete pain in the hypochondrium and abdomen, it is named Han Shan (Cold Hernia); if the abdomen is intermittently distended and reduced with pain, it is named Qi Shan (Qi Hernia); if abdominal pain is located beside the umbilicus, it is named Pan Shan (Plate Hernia); if abdominal pain is located below the umbilicus with accumulations, it is named [Missing Character] Shan; if the lower abdomen and genitalia are mutually pulling and painful, accompanied by difficulty in defecation, it is named Ye Lang Shan (Wild Wolf Hernia). All these seven types of Shan are caused by the internal accumulation of Cold pathogen, leading to coagulation and stagnation of Blood and Qi, preventing free flow. When the cold is severe, pain occurs; therefore, they are all called Shan. The *Nan Jing* (Classic of Difficult Issues) states: "When the Ren Mai (Conception Vessel) is diseased, there is internal bitterness from knotting; in men, it manifests as Seven Shans, and in women, as Jia Ju (Masses)." This is likely the reason.
To treat various symptoms of the Seven Shans, where Cold is present with pain beside the umbilicus, rushing up to the chest causing fullness and shortness of breath, use the Jiao Jiang Wan (Pepper and Ginger Pill) formula:
Shu Jiao (Sichuan Pepper, remove eyes and closed-mouth ones, stir-fry until sweat appears, 1 liang 1 fen), Gan Jiang (Dried Ginger, processed), Hou Po (Magnolia Bark, remove rough bark, apply fresh ginger juice and roast), Huang Qin (Scutellaria Root, remove black heart), Xi Xin (Asarum, remove seedlings and leaves), Bai Shao (White Peony Root), Gui Zhi (Cinnamon Twig, remove rough bark, each 1 liang), Jie Geng (Platycodon Root, stir-fry, half liang), Wu Hui (Aconite Lateral Root, processed, crack, remove skin and navel, 1 fen), Chai Hu (Bupleurum Root, remove seedlings), Fu Ling (Poria, remove black skin), Mu Dan Pi (Moutan Cortex, each 1 fen).
The above twelve ingredients are pounded and sifted into powder. Mix with refined honey to form pills the size of Sophora seeds. Take 20 pills per dose, administered with warm wine or rice drink, three times a day.
To treat various Shans, use the Tao Ren Tang (Peach Kernel Decoction) formula:
Tao Ren (Peach Seed, remove skin and pointed ends, double-kernel ones preferred, stir-fry), Wu Zhu Yu (Evodia Fruit, wash with water, bake dry, stir-fry), Chen Pi (Aged Tangerine Peel, soak in water, remove white pith, bake), Hai Zao (Sargassum, wash to remove saltiness), Fu Ling (Poria, remove black skin), Qiang Huo (Notopterygium Rhizome, remove root head), Ji Li Tribulus (Tribulus Fruit, stir-fry, remove spines, each 3 liang).
The above seven ingredients are coarsely pounded and sieved. Take three qian bi (approx. 9-15g depending on era/measurement) per dose. Add one cup of water and one large date-sized slice of fresh ginger. Decoct until 70% remains, strain, and take warm at any time.
To treat the Seven Shans, or if there is reversal cold in the heart and abdomen, inability to breathe smoothly, pain radiating to the back and spine, or firm pain below the heart that cannot be touched, or firm pain below the umbilicus exacerbated by cold food, or firm pain in the hypochondrium as large as a hand, or lower abdominal fullness pulling to urgent bladder pain, or irregular menstruation in women, use the Jiao Fu Wan (Pepper and Aconite Pill) formula:
Shu Jiao (Sichuan Pepper, remove eyes and closed-mouth ones, stir-fry until sweat appears, 1 liang), Jie Geng (Platycodon Root, cut and stir-fry), Bai Shao (White Peony Root), Gan Jiang (Dried Ginger, processed), Hou Po (Magnolia Bark, remove rough bark, apply fresh ginger juice... [Text incomplete in source]), [Source text lists "Eight Ingredients" but omits some names between Hou Po and the summary].
The above eight ingredients (as stated in source) are pounded and sifted into powder. Mix with refined honey to form pills the size of Sophora seeds. Take 20 pills per dose, administered with rice drink or warm wine, three times a day.
To treat Seven Shans with firm pain in the umbilical and abdominal regions, use the Peng E Zhu Wan (Curcuma Pill) formula:
Peng E Zhu (Curcuma Zedoaria, processed and cut), Mu Xiang (Aucklandia Root), Da Huang (Rhubarb Root, cut and stir-fry), Dang Gui (Angelica Sinensis, sliced and stir-fry), Chuan Xiong (Ligusticum Chuanxiong), Jing San Leng (Curcuma Phaeocaulis, processed and cut), Cao Dou Kou (Black Cardamom, remove skin... [Text anomaly "Let smoke emerge 1 liang" appears misplaced]).
The above eleven ingredients are pounded and sifted into powder. Use vinegar and wheat paste to form pills the size of Sophora seeds. Take 20 pills per dose, administered with warm wine or fresh ginger decoction, at any time.
To treat Seven Shans with cold limbs, firm pain and fullness in the umbilical and abdominal regions, use the Hou Po Wan (Magnolia Bark Pill) formula:
Hou Po (Magnolia Bark, remove rough bark, apply fresh ginger juice and roast), Fu Zi (Prepared Aconite Child Root, process until cracked, remove skin and navel), Xiang Zi (Costus Seed, stir-fry), Bai Zhu (Atractylodes Macrocephala Rhizome, cut and stir-fry), Gui Zhi (Cinnamon Twig, remove rough bark), Gan Jiang (Dried Ginger, processed), Zhi Ke (Bitter Orange Fruit, remove pulp, stir-fry with bran), Qing Ju Pi (Green Tangerine Peel, soak in water, remove white pith, bake), Chuan Xiong (Ligusticum Chuanxiong), Wu Tou (Aconite Mother Root, process until cracked, remove skin and navel), Mu Xiang (Aucklandia Root, processed).
The above twelve ingredients are pounded and sifted into powder. Mix with refined honey to form pills the size of Sophora seeds. Take 20 pills per dose, administered with warm wine, or fresh ginger decoction is also acceptable, at any time.
