Commentary states: The *Neijing* (Inner Canon) mentions that if wind pathogen affects the Spleen and transmits to the Kidney, the disease is named Shanxia (Hernia and Mass), characterized by heat accumulation and pain in the lower abdomen, and discharge of white fluid from the urine. This condition is also called Gu (Gu Disease). When the Spleen is invaded by wind pathogen, the evil transmits to the Kidney meridian, where evil heat scorches internally. Consequently, the symptoms include heat accumulation and pain in the lower abdomen. Because the true essence (Jing) fails to remain secure, the symptom is the discharge of white fluid in the urine, and the disease is named Gu. Due to internal scorching by evil heat and the failure of true essence to remain secure, if left untreated for a long time, it will eventually lead to the loss of spirit (Zhi). The essence of water corresponds to Zhi; when Zhi is lost, Jing follows. The *Zuo Zhuan* (Commentary of Zuo) states that losing one's spirit due to bewilderment is called "Gu," which is precisely this meaning.
To treat Gu disease with symptoms of urgent pain in the lower abdomen and loss of essence during urination or defecation, use Da Jianzhong Tang (Great Construct Middle Decoction):
Huangqi (Astragali Radix, sliced) 3 liang, Yuanzhi (Polygalae Radix, heart removed) 3 liang, Danggui (Angelicae Sinensis Radix, radix tip removed) 3 liang, Zexie (Alismatis Rhizoma) 3 liang, Shaoyao (Paeoniae Radix Alba) 2 liang, Renshen (Ginseng Radix) 2 liang, Longgu (Fossilia Ossis Mastodi) 2 liang, Gancao (Glycyrrhizae Radix et Rhizoma, honey-fried) 2 liang.
The above eight ingredients are sliced into particles the size of hemp seeds. Each dose consists of 5 qianbi, boiled with 2 zhan of water, 3 slices of Shengjiang (Zingiberis Rhizoma Recens), and 2 fractured Dazao (Ziziphi Jujubae Fructus) until reduced to 1 zhan. Strain and take warm, at any time.
To treat Gu disease with symptoms of heat accumulation and pain in the lower abdomen, insecurity of essence, and discharge of white fluid in urine, use Zexie Wan (Alisma Pill):

The above eight ingredients are pounded, sifted into powder, and mixed with refined honey to form pills the size of Sophora japonica seeds. Take 30 pills each time, administered with warm wine or salted water, on an empty stomach, at noon, and before sleep.
To treat Gu disease with symptoms of heat accumulation and pain in the lower abdomen, and discharge of white fluid in urine and stool, use Roushoucong Wan (Cistanche Pill):
