Discussion: The condition of tongue stiffness and aphasia (loss of speech) caused by wind-stroke is due to the fact that the spleen meridian connects with the stomach, runs alongside the throat, and attaches to the root of the tongue. This is the pathway through which heart Qi circulates. When pathogenic wind invades and binds here, it causes obstruction and blockage of the Qi and meridians, resulting in a stiff tongue that cannot extend or retract freely, thereby impairing speech function.
For treating wind-stroke causing tongue stiffness and inability to speak, with a flushed face approaching syncope, body flaccidity or spasticity, and restless gaze, use Huangqin Decoction:
Huangqin (Scutellariae Radix, remove black center) Guizhi (Cinnamomi Ramulus, remove rough bark) Xiongjiao (Ligustici Chuanxiong Rhizoma) Renshen (Ginseng Radix et Rhizoma) Fangfeng (Saposhnikoviae Radix, remove forked branches) Fangji (Stephaniae Tetrandrae Rhizoma) Mahuang (Ephedrae Herba, remove roots and joints, decoct first, skim foam)
The above eleven ingredients are cut into pieces the size of hemp seeds. Use six qianbi (approx. 18-20g depending on measure) per dose. Add three zhan (approx. 600ml) of water and ginger crushed to the size of three jujubes. Decoct until one and a half zhan remains. Strain and divide into two warm doses. Take one dose on an empty stomach and one before bed.
For treating wind-stroke causing tongue stiffness and inability to speak, with body flaccidity or spasticity, deviation of mouth and eyes, mental confusion, and restlessness, effective for various wind strokes without causing deficiency, use Mahuang Decoction:

The above nine ingredients are cut into pieces the size of hemp seeds. Use ten qianbi per dose. Add three zhan of water and ginger crushed to the size of three jujubes. Decoct until one and a half zhan remains. Strain and divide into three doses. Take two doses on an empty stomach, then take the third dose after the time it takes a person to walk five li. Cover with clothing to induce sweating. Avoid exposure to external wind.
For treating wind-stroke causing tongue stiffness and inability to speak, with inability to open both eyes and shortness of breath resembling death, use Fangfeng Decoction:
Fangfeng (Saposhnikoviae Radix, remove forked branches) Gancao (Glycyrrhizae Radix et Rhizoma, honey-fried) Huangqin (Scutellariae Radix, remove black center) Baifuling (Poria, remove black skin) Danggui (Angelicae Sinensis Radix, slice and dry-fry, each banliang) Xingren (Prunus Armeniaca Seed, remove skin and tip, stir-fry)
The above eight ingredients are coarsely crushed and sifted. Use ten qianbi per dose. Add two zhan of water and one zhan of clear wine. Add three pitted jujubes and ginger crushed to the size of two jujubes. Decoct together until one and a half zhan remains. Strain and divide into three doses. Take two doses on an empty stomach, then take the third dose after the time it takes a person to walk five li.
