Discussion states: Hematemesis (vomiting blood) or epistaxis (nosebleed) triggered by the administration of mineral drugs (Rushi, a type of ancient mineral-based tonic) occurs because the qi of the minerals becomes obstructed in the Heart and Lungs, generating heat. The Heart governs Blood; when Blood is heated, it surges and overflows. Compounded by the rebellion of Stomach Qi, hematemesis results. The Lung governs Qi and opens into the nose. When Qi and Blood are harmonious, they mutually nourish and protect as Ying (Nutritive) and Wei (Defensive). However, when attacked by Heat, Blood flows recklessly, resulting in epistaxis.
For treating chest oppression and hematemesis caused by the activation of mineral drugs, Mai Men Dong Tang (Ophiopogon Decoction) is prescribed:
Mai Men Dong (Ophiopogonis Radix, heart removed, baked, 1 liang), Sheng Di Huang (Rehmanniae Radix Recens, 2 liang), Gan Cao (Glycyrrhizae Radix et Rhizoma, honey-fried), Qi Gen (Herba Glehniae or similar root, context implies herbal root), Mao Gen (Imperatae Rhizoma, dried ginger, each 1 liang).
The above six herbs are chopped to the size of hemp seeds. Each dose consists of 3 qian bi (approx. 9-10g), boiled with 1 sheng (approx. 200ml) of water and 14 grains of Chi Dou (Fermented Soybean) until reduced to 7/10 volume. Strain and take warm. Administer on an empty stomach and at noon.
For treating epistaxis due to Heat in the Heart and Lungs triggered by the ingestion of mineral drugs, Lian Qiao Yin (Forsythia Drink) is prescribed:
Lian Qiao Jing Ye (Stem and leaves of Forsythiae Fructus, fresh, 1 liang), Sheng Di Huang (Rehmanniae Radix Recens, 2 liang), Cang Er Jing Ye (Stem and leaves of Xanthii Fructus, fresh), Chen Ju Pi (Pericarpium Citri Reticulatae, soaked in hot water, white pith removed), Ji Su Jing Ye (Stem and leaves of Elsholtzia Spicata, fresh, each 1 liang).
The above five herbs are chopped finely, mashed with a small amount of water until pulpy, and the juice is extracted through raw silk gauze. Take 3 he (approx. 60ml) per dose, at any time. If bleeding does not stop, administer again.
For treating epistaxis, headache, high fever, generalized body pain, and restlessness caused by the activation of mineral drugs, Xiao Ji Tang (Cirsii Herba Decoction) is prescribed:
Xiao Ji (Herba Cirsii Japonici, chopped, 2 liang), Ji Su (Herba Elsholtziae Spicatae, chopped, 1 liang), Qing Zhu Ru (Bamboo Shavings from new bamboo, 1.5 liang), Mai Men Dong (Ophiopogonis Radix, heart removed, baked), Sheng Di Huang (Rehmanniae Radix Recens, chopped, each 2 liang).
The above five herbs are chopped to the size of hemp seeds. Each dose consists of 3 qian bi, boiled with 1 sheng of water and 3 slices of Sheng Jiang (Zingiberis Rhizoma Recens) until reduced to 7/10 volume. Strain and take warm at any time. Three times daily.
For treating persistent epistaxis due to severe Heat in the Heart and Lungs triggered by mineral drugs, Chui Bi San (Nose-Blowing Powder) is prescribed:
Hu Fen (Cerussa, ground), Mo (Ink stick, ground), Gan Jiang (Zingiberis Rhizoma, powdered), Fa Hui (Hair Ash, ground), Dang Mo (Soot from cauldron, ground), Fu Long Gan (Caulis Longi Farris, powdered, each half qian).
Grind the above six ingredients together. Use approximately one "yi zi" (a traditional measure for nasal insufflation) and blow into the nose. Alternatively, using powder from just one of the six ingredients, apply one "yi zi" to both nostrils to stop bleeding.
For treating sudden hematemesis of one or two sheng, with blood coming from both mouth and nose, triggered by mineral drugs, Mai Men Dong Yin (Ophiopogon Drink) is prescribed:
Sheng Mai Men Dong (Ophiopogonis Radix Recens, heart removed, juice pressed to 1 sheng), Sheng Di Huang (Rehmanniae Radix Recens, juice pressed to 1 sheng), Xiao Ji (Herba Cirsii Japonici, chopped, juice pressed to 1 sheng), Fu Long Gan (Caulis Longi Farris, powdered, 2 liang).
For the first three ingredients, add a little water and press for juice. Mix evenly. Each dose is 2 he, mixed with 2 qian bi of Fu Long Gan powder and taken orally. Take once on an empty stomach and once at noon.
For treating hematemesis and epistaxis due to excessive Heat triggered by mineral drugs, Sheng Di Huang Tang (Rehmannia Decoction) is prescribed:
Sheng Di Huang (Rehmanniae Radix Recens, chopped, 5 liang), Zhi Zi Ren (Gardeniae Semen, 20 units), Xiao Ji Gen (Radix Cirsii Japonici, chopped, 3 liang), Huang Qin (Scutellariae Radix, black heart removed, 1 liang).
Chop the above four herbs to the size of hemp seeds. Each dose consists of 5 qian bi, boiled with 1.5 sheng of water and 14 grains of Chi Dou until reduced to 1 sheng. Strain and take warm. Take once on an empty stomach and once in the evening.
For treating epistaxis triggered by mineral drugs, Sheng Di Huang Yin (Rehmannia Drink) is prescribed:
Sheng Di Huang (Rehmanniae Radix Recens, chopped, juice pressed to 3 he), Xiao Ji Gen (Radix Cirsii Japonici, chopped, juice pressed to 3 he).
Mix the juices of the above two ingredients evenly. Divide into three warm doses. Take once in the morning and once in the evening.
