Commentary states: When a cold damage disease originates from the Yin aspect, and the physician erroneously applies purgation, the pathogenic factor enters the interior. The stomach becomes deficient, and alien qi rebels upward, causing fullness in the epigastrium without pain, which feels soft upon palpation; this is termed Pi syndrome (Pien Syndrome, [epigastric] fullness). The treatment principle should involve using Heart-draining formulas. However, if the exterior syndrome has not yet resolved, one must not attack the Pi directly; instead, the exterior should be resolved first, followed by treating the Pi. If the exterior is resolved but the interior remains unharmonized, or if the Pi does not resolve after administering a Heart-draining formula, and the patient presents with dry mouth, vexation, and thirst, treatment should be tailored to the specific pattern. If dizziness, vertigo, and tremors of the meridians occur, prolonged duration may lead to Wei syndrome (Atrophy Syndrome). In individuals with both exterior and interior deficiency, where Qi Pi occurs alongside exhaustion of both Yin and Yang Qi, if Fire needle therapy is additionally applied, it may cause vexation, cyanotic or sallow complexion, and withered skin, making it difficult to treat. If the pulse is slightly floating and there is Qi rushing up to the throat causing dyspnea, this is not Pi syndrome but rather Cold in the chest; such cases fall under the category of emetic therapy.
To treat epigastric Pi caused by cold damage, characterized by fullness without pain: Banxia Xiexin Tang (Pinellia Decoction for Draining the Epigastrium) Formula
Banxia (Pinelliae Rhizoma, washed in boiling water seven times, stir-fried until dry, 1 liang) Huangqin (Scutellariae Radix, remove black heart, 3 qian) Huanglian (Coptidis Rhizoma, remove stamens, stir-fried, 0.5 liang) Ganjiang (Zingiberis Rhizoma, processed) Renshen (Ginseng Radix et Rhizoma, each 3 qian) Gancao (Licorice Radix et Rhizoma, honey-fried and chopped, 0.5 liang)
The above six herbs are crushed to the size of hemp seeds. Each dose consists of 5 qianbi, boiled with 1.5 sheng of water and three cracked Jujubae Fructus (Ziziphi Jujubae Fructus). Simmer until reduced to 0.8 sheng, strain, and take warm after meals.
To treat cold damage where severe purgation followed by sweating results in epigastric Pi and aversion to cold, indicating unresolved exterior syndrome: Do not attack the Pi. Resolve the exterior first, then treat the Pi. For resolving the exterior, Guizhi Tang (Cinnamon Twig Decoction) is suitable (see section on Sweating Methods). For attacking the Pi, Dahuang Huanglian Xiexin Tang (Rhubarb and Coptis Decoction for Draining the Epigastrium) is suitable.
Dahuang (Rhei Radix et Rhizoma, wine-washed, 2 liang) Huanglian (Coptidis Rhizoma, remove stamens, 1 liang)
The above two herbs are finely chopped. Each dose of 3 qianbi is soaked in 1 sheng of boiling water (Mafei Tang) for a short while, strained, and taken warm. Another version adds Huangqin (Scutellariae Radix, 1 liang).
To treat epigastric Pi accompanied by aversion to cold and sweating: Fuzi Xiexin Tang (Aconite Decoction for Draining the Epigastrium) Formula
Dahuang (Rhei Radix et Rhizoma, chopped and stewed, 2 liang) Huangqin (Scutellariae Radix, remove black heart) Huanglian (Coptidis Rhizoma, remove stamens, each 1 liang) Fuzi (Aconiti Lateralis Radix Praeparata, one piece, processed until cracked, peel and navel removed, chopped and decocted to extract juice)
Excluding Fuzi, the other four herbs are finely chopped. Each dose of 3 qianbi is soaked in 1 sheng of boiling water for a short while, strained to obtain the liquid, mixed with 0.5 he of Fuzi juice, and taken warm.
To treat post-sweating resolution of cold damage, where disharmony in the stomach leads to hard epigastric Pi, dry belching with food odor, fluid retention in the hypochondrium, borborygmus, and diarrhea: Xiexin Tang (Draining the Epigastrium Decoction) Formula
Renshen (Ginseng Radix et Rhizoma) Gancao (Licorice Radix et Rhizoma, honey-fried, each 3 liang) Ganjiang (Zingiberis Rhizoma, processed) Huanglian (Coptidis Rhizoma, remove stamens, each 1 liang) Huangqin (Scutellariae Radix, remove black heart, 2 liang) Banxia (Pinelliae Rhizoma, washed in boiling water seven times, 2.5 liang)

To treat wind-strike complicated by cold damage, where erroneous purgation causes frequent diarrhea (dozens of times daily), undigested food, borborygmus, hard and full epigastric Pi, dry vomiting, vexation, and insomnia. If the physician sees Pi and assumes incomplete pathogen removal, leading to further purgation, the Pi worsens. This is not heat binding but due to stomach deficiency and rebellious alien Qi causing hardness: Gancao Xiexin Tang (Licorice Decoction for Draining the Epigastrium) Formula
Gancao (Licorice Radix et Rhizoma, honey-fried, 4 liang) Huangqin (Scutellariae Radix, remove black heart) Ganjiang (Zingiberis Rhizoma, processed, each 3 liang) Banxia (Pinelliae Rhizoma, washed in boiling water seven times, 2.5 liang) Huanglian (Coptidis Rhizoma, remove stamens, 1 liang)

To treat cold damage fever where, after vomiting or purgation, the exterior resolves but hard epigastric Pi persists with continuous belching: Xuanshu Daizhe Tang (Inula and Hematitum Decoction) Formula
Xuanshuhua (Flowers of Inulae Flos, 3 liang) Daizheshi (Hematitum, vinegar-processed, 1 liang) Renshen (Ginseng Radix et Rhizoma) Banxia (Pinelliae Rhizoma, washed in boiling water seven times, each 2 liang) Gancao (Licorice Radix et Rhizoma, honey-fried, 3 liang)
