[Discussion] The so-called "Lao Nue" (Chronic Malaria due to Exhaustion) occurs when malaria persists for a long time without resolution, leading to deficiency of both Qi and Blood. Although the disease has intermittent periods, it is triggered by physical exertion, hence the name. At this stage, pathogenic factors deepen daily, while True Qi (Zhen Qi) is increasingly depleted. With both exterior and interior deficient, symptoms such as reduced appetite, emaciation, sallow complexion, and weakness manifest, while chills and fever persist as before.
For treating Lao Nue with symptoms of dry and brittle hair, irregular chills and fever, reduced diet, muscle wasting, bluish-black complexion, weakness in both legs, cold feet at abnormal times, frequent urination, irregular bowel movements, and nocturnal seminal emission, use Biejia Zhu San Formula:
Bie Jia (Turtle Shell, remove skirt edge, vinegar-fried, Ban Liang) Qiang Huo (Notopterygii Rhizoma et Radix, remove radicle) Du Huo (Angelicae Pubescentis Radix, remove radicle) Chai Hu (Bupleuri Radix, remove sprouts, each Yi Fen) Huang Qi (Astragali Radix, sliced, Ban Liang) Chi Fu Ling (Poria Cocos with Red Hosta Root, remove black skin) Sheng Gan Di Huang (Rehmanniae Radix, dried, baked) Mu Xiang (Aucklandiae Radix, each Yi Fen) Niu Xi (Achyranthis Bidentatae Radix, soaked in wine, sliced, baked) Tian Ling Gai (Human Cranium, fried with butter) Wu Zhu Yu (Evodiae Fructus, each Yi Liang).
The above seventeen ingredients are crushed, sieved into powder. Each dose is San Qian Bi (approx. 4.5g), boiled with one Jian (approx. 200ml) of water with a little salt until reduced to seven-tenths. Take warm with dregs on an empty stomach; or refine honey to make pills the size of paulownia seeds, take forty pills with salted water on an empty stomach.
For treating Lao Nue, use Qinjiao Jiu Formula:

The above seven ingredients are chopped like hemp seeds, soaked overnight in three Sheng (approx. 600ml) of unadulterated wine. Place near fire to keep slightly warm. Take one Zhan (approx. 200ml) per dose, at any time before onset. After taking, add one Zhan of wine to the medicinal residue, continuing until three Sheng are added, then stop.
