Commentary states that the pharyngeal gate (yanmen) is the pathway for Stomach Qi, and the larynx (houlou) is the passage for Lung Qi. Within the body, the ascending, descending, exiting, and entering of Qi has no urgency greater than this. A detailed examination of meridian flow and distribution reveals that the connection of the throat and pharynx is not limited to the Lung and Stomach. Therefore, Sun Simiao stated that it is the channel through which the Five Zang and Six Fu organs interact, and the way through which Shen Qi and Yin-Yang pass or are blocked. Human Blood and Qi circulate in harmony with Heaven and Earth, and the throat and pharynx are particularly crucial for intake and excretion. Thus, the *Neijing* states: The larynx governs Heavenly Qi (clear air), and the pharynx governs Earthly Qi (turbid substances). If the Zang organs have heat, the pharyngeal gate closes and Qi becomes obstructed; if the Fu organs have cold, the pharyngeal gate breaks and the voice becomes hoarse. This leads to conditions such as swelling, pain, throat obstruction (Houbi), sores, and palatine tonsil hypertrophy (Xuanyong), as well as sensations of a foreign body, stuffiness, pain, itching, and excessive saliva. These manifestations vary, and one cannot generally treat them all as excess heat. The general principle is: for heat patterns, use unblocking methods; for cold patterns, use tonifying methods; for patterns neither hot nor cold, regulate according to the meridians. Besides purging with decoctions, there are also acupuncture and other methods. The key is to eliminate the pathogen urgently without delay, unlike chronic diseases accumulated in the Zang and Fu organs, which require gradual resolution and conditioning, taking more than a day or night to achieve results.
Commentary states that throat and pharyngeal obstruction and blockage arise from dysfunction of the Spleen and Lung, where Wind pathogens and Heat toxins attack and rush into the throat and pharynx, causing swelling and pain. In severe cases, it leads to complete obstruction and blockage, requiring urgent treatment. This is because the throat and pharynx are the pathways for respiration and the entry and exit of Qi.
