Discussion states: Heat-phlegm (Re Tan) arises from the obstruction of the airway and the failure of body fluids (Jin Ye) to circulate freely. Heat qi (Re Qi) and phlegm-water (Tan Shui) contend with each other, gathering without dispersing. The manifestations include dryness of the throat, a sensation of blockage or congestion in the throat, heaviness and dizziness of the head and eyes, coughing up thick and turbid sputum, and redness and heat in the face and eyes. These constitute the syndrome of heat-phlegm.
To treat heat-phlegm, benefit the chest and diaphragm, relieve irritability and thirst, and transform saliva and phlegm, use the Powder to Transform Saliva (Hua Xian San):
Shui Shi (Calcitritum, 1 liang), Qian Bai Shuang (Lead White Frost, ground), Ma Ya Xiao (Saltpeter), Xiong Huang (Realgar, ground) each 1 qian, Bai Fan (Alumen, boiled until juice is dry), Gan Cao (Licorice Root, slightly roasted and chopped).
The above six herbs are crushed and ground into a powder. Add a small amount of Nao Bing (Borneol), grind evenly. Take 1 qian bi per dose, mixed with honey water. For children's wind-heat causing salivary disorders, mix half a qian bi with sugar water. This medicine is very cold; do not consume too much.
To treat heat-phlegm, cool the heart and lungs, benefit the chest and diaphragm, resolve heat toxicity, tonify primordial qi and boost energy, use the Goose Pear Decoction Pills (E Li Jian Wan):
Da E Li (Large Goose Pear, 20 fruits, peeled, pitted, juice extracted with clean cloth), Zao Jiao (Tamarind Fruit, 10 sturdy pods without black skin, juice extracted by rubbing in Jiang Shui/Vinegar Water), Sheng Di Huang (Fresh Rehmannia Root, half jin, washed, crushed, juice extracted), Sheng Bo He (Fresh Mint, half jin, finely crushed, juice extracted), Mi (Honey, half jin).
The above five ingredients are slowly simmered in a silver or earthenware vessel to form a paste. Add the powders of the remaining herbs: Mu Xiang (Aucklandia Root), Ren Shen (Ginseng Root), Bai Fu Ling (Poria, black skin removed), Bai Ji Li (Tribulus Fruit, stir-fried and thorns removed), Niu Xi (Achyranthes Root, soaked in wine overnight, sliced and dried), Rou Cong Rong (Cistanche Stem, soaked in wine overnight, sliced and dried) each 1 liang; Qiang Huo (Notopterygium Rhizome, rhizome head removed), Fang Feng (Saposhnikovia Root, forked parts removed), Bai Zhu (Atractylodes Rhizome), Qing Ju Pi (Green Tangerina Peel, white pith removed and dried), Jie Geng (Platycodon Root, chopped and stir-fried), Shan Yu (Dioscorea Tuber) each 3 fen; Ban Xia (Pinellia Tuber, washed in hot water seven times, dried and stir-fried) 1 liang; Bin Lang (Areca Seed, baked and chopped) 2 liang; Gan Cao (Licorice Root, roasted and chopped) half liang.
Of the twenty herbs, the first five are made into a paste. The rest are crushed and sieved into powder. Mix with the paste, pound until the consistency is appropriate. Form pills the size of Sophora seeds. Take 15 pills per dose, increasing to 20 pills. Administer with Jing Jie Tang (Schizonepeta Decoction) after meals, twice daily.
To treat severe heat-phlegm obstruction and difficulty in the throat and diaphragm, use the Thousand Gold Powder (Qian Jin San):
Ban Xia (Pinellia Tuber, half liang pounded with 1 liang fresh ginger into cakes, sun-dried), Hua Shi (Talcum, powder, 3 qian), Ning Shui Shi (Halloysite, 3 qian), Qing Ge Fen (Blue Clam Powder, powder, 5 qian).
Grind the above five herbs together into a fine powder. Take 1 qian bi per dose. Mix with one cup of vinegar juice, boil down to 60%, and take warm before retiring.
To treat heat-phlegm with dry throat, irritability, and thirst, use the Borneol Pills (Long Nao Wan):
Long Nao (Borneol, 1 zi), Qian Bai Shuang (Lead White Frost, ground, 1 fen), Gan Cao (Licorice Root, roasted and chopped, half liang), Ning Shui Shi (Halloysite, fired until red-hot and ground, 1 fen).
Grind the above four herbs finely and mix evenly. Use cooked rice as a binder to form pills the size of Sophora seeds. Hold three to five pills in the mouth to dissolve per dose.
To treat heat-phlegm, calm the spirit, relieve headache and nausea, and resolve irritability, use the Cinnabar and Silver Leaf Pills (Dan Sha Yin Bo Wan):
Dan Bo (Gold Leaf? Note: Text says Dan Bo, likely Cinnabar foil or similar preparation, processed by water flotation, 3 fen), Niu Dan Zhi Tian Nan Xing (Arisaema Cum Bile, 2 liang), Xiong Huang (Realgar, ground), Long Nao (Borneol) each 1 fen, Yin Bo (Silver Leaf, 15 pieces), Ma Ya Xiao (Saltpeter).
Grind each of the above six herbs finely, then mix thoroughly. Bind with refined honey to form pills the size of Euryale seeds. Chew one pill per dose, swallow with cooled Ren Shen Tang (Ginseng Decoction). Alternatively, administer with Zhu Ye Tang (Bamboo Leaf Decoction) or fresh well water. Take after meals and before retiring.
To treat heat-phlegm, guide stagnant qi, and moisten the intestines and stomach, use the Ginseng and Rhubarb Decoction (Shen Huang Tang):
Da Huang (Rhubarb Root and Rhizome, baked and chopped, 3 liang), Ren Shen (Ginseng Root), Zhi Ke (Aurantii Fructus, soaked in hot water, pith removed, bran-stir-fried), Bin Lang (Areca Seed, baked and chopped) each 1 liang, Ban Xia (Pinellia Tuber, washed in hot water to remove slipperiness, stir-fried until yellow, 1.5 liang).
Coarsely crush and sieve the above eight herbs. Take 2 qian bi per dose. Add half a piece of crushed fresh ginger to one cup of water. Boil together to 70%. Strain and take warm after meals and before retiring.
To treat severe heat-phlegm, deficiency-fire irritability, and dry thirst, use the Cinnabar Pills (Dan Sha Wan):
Dan Sha (Cinnabar, ground and processed by water flotation), Tian Nan Xing (Arisaema, prepared), Bai Fan (Alumen, boiled until juice is dry) each 1 liang, Mang Cao (Agastache/Patchouli? Note: Text says Mang Cao, likely Agastache or similar aromatic herb, roasted) half liang.
Crush and grind the above four herbs into powder. Additionally, take 2 liang of Ban Xia, wash seven times in hot water, sun-dry, and grind into powder. Boil this powder in water to make a paste. Mix with the herbal powder to form pills the size of Sophora seeds. Take 7 pills per dose, increasing to 10 pills. Swallow with Bo He Cha (Mint Tea) or Sheng Jiang Tang (Ginger Decoction) using warm water. Time is not restricted.
To treat stagnation of heat-phlegm, expectoration resembling sheep fat, benefit the throat and diaphragm, and descend phlegm-saliva, use the Black Dragon Pills (Wu Long Wan):
Zao Jiao (Tamarind Fruit, 4 liang of non-crispy pods, burned to retain character, ground), Bai Fan (Alumen, good quality, half raw half calcined), Pu Xiao (Mirabilite, ground), Qian Bai Shuang (Lead White Frost, ground) each 1 liang.
Mix the above four herbs evenly. Cook with vinegar and wheat flour paste to form pills the size of Sophora seeds. Take 10 pills per dose, administered with strong decoction of Huai Shi (Sophora Fruit). If there is sore throat and swelling, or upper diaphragm obstruction, use Gan Cao (Licorice Root) decoction instead, taken after meals and before retiring.
To treat stagnation of heat-phlegm, use the Alum Stone Pills (Fan Shi Wan):
Bai Fan (Alumen, boiled until juice is dry, 1 liang), Dan Sha (Cinnabar, ground and processed by water flotation, half liang).
Grind the above two herbs evenly. Bind with thin wheat flour paste to form pills the size of Sophora seeds. Chew thoroughly with dried jujube flesh and swallow five pills. Time is not restricted.
To treat heat-phlegm with choking, stifling sensation, dry retching, headache, and dizzy vision as if drunk, use the Rhinoceros Horn Pills (Xi Jiao Wan):
Xi Jiao (Rhinoceros Horn, shaved, half liang), Ban Xia (Pinellia Tuber, boiled thoroughly in vinegar water), Tian Nan Xing (Arisaema, soaked in yellow cattle bile for three nights, dried) each 2 liang, Da Huang (Rhubarb Root and Rhizome, boiled in vinegar for one shi period, dried).
(Note: Text mentions "above seven herbs" but lists fewer explicitly in the ingredient line, implying standard processing additions or missing items in transcription, but we translate what is present). Crush and sieve into powder. Use 15 pods of Soap Pod (Fei Zao Jiao) and two bowls of water, simmer slowly to form a paste. Add the herbal powder to form pills the size of Sophora seeds. Take 20 pills per dose after meals with Sheng Jiang Bo He Tang (Ginger and Mint Decoction).
To treat phlegm-heat, difficulty in the throat and diaphragm, and misaligned pain in the head and eyes, use the Musk and Cinnabar Pills (She Xiang Dan Sha Wan):
She Xiang (Musk, ground, 1 fen), Dan Sha (Cinnabar, ground, 1 liang), Mai Men Dong (Ophiopogon Tuber, heart removed, dried, 3 fen), Long Nao (Borneol, ground, 1 qian), Mu Xiang (Aucklandia Root), Ding Xiang (Clove), Xi Jiao (Rhinoceros Horn, powder), Gan Cao (Licorice Root, roasted and chopped) each 1 fen, Ren Shen (Ginseng Root), Huo Xiang (Pogostemon Herb, stems removed), Tian Nan Xing (Arisaema, prepared in cattle bile), Fang Feng (Saposhnikovia Root, forked parts removed), Huang Qi (Astragalus Root, chopped) each half liang.
Grind the above thirteen herbs into powder, mix evenly. Bind with refined honey to form pills the size of Euryale seeds. Take one pill per dose, chew thoroughly, and swallow with Jing Jie Tang (Schizonepeta Decoction). Take after meals and before retiring.
To treat heat-phlegm in the chest, loss of appetite, use the Ophiopogon Decoction (Mai Men Dong Tang):
Mai Men Dong (Ophiopogon Tuber, heart removed, dried), Ge Gen (Kudzu Root), Ren Shen (Ginseng Root), Qian Hu (Peucedani Radix, rhizome head removed), Xi Jiao (Rhinoceros Horn, shaved) each 1 liang, Jie Geng (Platycodon Root, half liang), Lu Gen (Reed Rhizome, 2 liang).
Chop the above seven herbs uniformly like hemp seeds. Take 5 qian bi per dose. Add one and a half cups of water. Boil down to 80%. Strain and take warm.
