Commentary states: Whenever arrowheads or metal sharp objects penetrate the flesh, treatment should aim to remove them quickly. If there are broken bones, they must also be completely removed before applying various topical medicines; otherwise, the wound will certainly not heal. Even if it temporarily improves slightly, pain will often persist. If fright causes blood and qi to become chaotic and vital energy to be depleted, death may ensue.
Formula for treating metallic wounds where arrowheads are lodged in bone and have remained for many years without being expelled: Mudan San (Moutan Powder).
Mudan Cortex (Paeoniae Moutan Cortex), with the heart removed, 1 liang; Bai Lian (Ampelopsis Radix), 1 liang; Sang Gen Bai Pi (Mori Cortex Radicis), chopped, 2 liang; Huo Xiang Ye (Agastaches Folium); Ding Xiang (Caryophylli Flos); She Xiang (Moschus), ground, each 1 fen.
The above six herbs are crushed, sifted into a powder. Take 2 qian bi per dose, mixed with warm wine, three times daily. For shallow wounds, it takes ten days; for deep wounds, twenty days, after which the arrowhead is naturally expelled.
Formula for treating arrowheads that do not expel:
Ci Shi (Magnetitum), raw, crushed and ground extremely fine; Xiong Huang (Realgar), ground, each 3 fen.
The above two minerals are ground together uniformly. Take 2 qian bi per dose, mixed with mung bean juice, on an empty stomach. After ten days, gentle manipulation will cause expulsion. Applying this medicine to the hands and feet also facilitates natural expulsion.
Formula for treating metallic wounds to relieve pain, as well as Ji Feng (acute wind syndrome) and Po Shang Feng (tetanus/trismus caused by injury), and promoting arrowhead expulsion: Jifeng San (Jifeng Powder).
Cao Wu Tou (Aconiti Kusnezoffii Radix), 3 liang total: one liang calcined to retain its nature and quenched in vinegar until cool; another liang calcined according to the previous method and covered with fresh yellow earth until cool; one liang used raw. Sheng Hei Dou (Glycines Soja Seed, raw); Dan Sha (Cinnabar); She Xiang (Moschus).
The above ingredients (processed as described) are crushed and sifted into a powder. Add Dan Sha and She Xiang, and grind finely together. To expel an arrowhead, first take half a qian bi mixed with wine orally, then apply the medicine topically to the wound. For tetanus, take half a qian bi or one qian bi mixed with wine orally. For mild conditions, take only "yi zi" (a very small measure), adjusting dosage according to the patient's capacity.
Formula for treating metallic wounds and using topical application to promote spontaneous arrowhead expulsion: Hushé Wan (Tiger Tongue Pill).
Gan Hu She (Dry Tiger Tongue), half liang, pounded in a stone mortar into powder; Sheng Cao Wu Tou Jian (Tip of Aconiti Kusnezoffii Radix, raw), powdered, 3 qian; Ci Shi (Magnetitum), half liang (dense quality), pounded in a stone mortar into powder; Liu Huang (Sulfur), ground; Chu Zi (Broussonetiae Fructus), pounded into powder. [Note: Text mentions ten ingredients but lists fewer explicitly in this segment, implying others from context or standard preparation].
The above ten ingredients are placed in a mortar and ground extremely fine. Form pills using Shi Nao You (Petroleum Ether/Naphtha), sized like millet seeds. Store in horn containers with pointed ends. For each arrowhead to be expelled, use one pill. Insert the medicated tip into the wound using a copper stick. Rub the surrounding area with a little good wine, then massage the wound with Hong San Zi (Red Powder). Soon, intense heat and itching will be felt, and the arrowhead will be expelled on the same day. Subsequently, apply Shengji Jinhua San (Flesh-Regenerating Golden Flower Powder) into the wound and seal it with Zijin Gao (Purple Gold Ointment).
Formula for treating arrowheads, applied to the wound opening: Hong San Zi (Red Powder).
Man Tuo Luo Zi (Daturae Fructus); Cao Wu Tou Jian (Tip of Aconiti Kusnezoffii Radix); Qi Jie (Dragon's Blood); Qie Zi Hua (Solanis Melongena Flos); Bi Ma Zi (Ricini Semen), shelled and finely ground, each half liang.
The above five herbs are crushed and sifted into a fine powder. Mix with good wine to form a paste. Apply and massage onto the wound opening; the arrowhead will spontaneously expel.
Formula for treating metallic wounds where knives/arrows enter flesh, broken bones remain, causing redness, swelling, and pain: Er Hui San (Two Ashes Powder).
Ma Jiang Hui (Ash from horse reins), 1 liang; Gong Xian Hui (Ash from bowstrings), 1 liang.
The above two ashes are ground together uniformly. Take 1 qian bi per dose, mixed with Liao Lan Zhi (Polygoni Tinctorii Juice), three times daily.
Formula for treating arrowheads entering flesh with redness and swelling, avoiding wind and promoting healing: Danggui Xuduan Gao (Angelica and Teucrium Ointment).
Dang Gui (Angelicae Sinensis Radix); Xuduan (Dipsaci Radix); Gu Sui Bu (Drynariae Rhizoma); Gui Zhi (Cinnamomi Ramulus), coarse bark removed; Fu Zi (Aconiti Lateralis Radix Praeparata); Ze Lan (Lycopersici Herba); Shao Yao (Paeoniae Radix Alba); Bai Ji (Bletillae Rhizoma); Niu Xi (Achyranthis Bidentatae Radix); Qiang Huo (Notopterygii Rhizoma et Radix); Xiong (Ligustici Chuanxiong Rhizoma); Mu Xiang (Aucklandiae Radix); Qi Jie (Dragon's Blood); Sheng Gan Di Huang (Rehmanniae Radix); Bai Jiang Can (Bombyx Batryticatus); Bai Fu Zi (Typhonii Rhizoma), each 1 liang; Chen Xiang (Aquilariae Lignum Resinatum); Ding Xiang (Caryophylli Flos), each half liang; Gua Lou (Trichosanthis Fructus), two large fruits.
The above twenty-five herbs are finely chopped. Add 4 jin of Sesame Oil (Mayou), 1.5 jin of Pig Fat (Zhuzhi), and 3 liang of Camel Fat (Tuozhi). Decoct over mild and strong fire for three days. Filter out the herbs. Add 3 liang of Ru Xiang (Olibanum) and 6 liang of Song Zhi (Colophonium). Decoct for another day. Filter through raw silk to remove coarse dregs. Use a large iron pot (five dou capacity). Add 3 jin of finely sifted Qian Dan (Lithargyrum), stir-fry until purple. Slowly add the previous herbal oil while stirring with a willow branch until the mixture turns purple. Remove from heat. Test by dropping a little oil into water; when it forms beads, the processing is complete. Store tightly in porcelain vessels. Use according to the aforementioned methods.
Formula for treating arrowheads that do not expel: Xionghuang Wan (Realgar Pill).
Xiong Huang (Realgar), ground; Du Jiao Xian (Dynastes hercules beetle); Sha (Cinnabar), ground; Bu Hui Mu (Asbestus); Wei Ling Xian (Clematidis Radix et Rhizoma), soil removed; Mu Jin Hua (Hibisci Mutabilis Flos), each 1 liang; Shu (Rattus norvegicus), one piece, using the heart.
The above seven ingredients, except for the rat blood, are crushed and ground into a powder. Mix uniformly and bind with refined honey to form pills sized like yellow rice grains. Insert into the wound; the arrowhead will spontaneously expel.
Formula for treating arrowheads that do not expel: Jiegu Wan (Bone-Resolving Pill).
Xiong Huang (Realgar), ground; Qiang Lang (Scarabaeus sisyphius), ground; Xiang Ya Mo (Elephant Ivory Powder). These three ingredients are in equal proportions.
Grind these three ingredients into a powder. Bind with refined honey to form pills sized like millet seeds. Insert into the wound opening. Chew sheep kidney fat finely and apply/massage over the site. When itching is felt, the arrowhead will spontaneously expel.
Formula for treating arrowheads lodged in bone, unable to be pulled out by any means: Badou Gao (Croton Ointment).
Badou (Crotonis Tiglii Semen), five kernels, shelled and raw-ground; Qiang Lang (Scarabaeus sisyphius), one piece, legs and wings removed, raw.
The above two ingredients are ground together uniformly into an ointment. Form pills sized like green beans for use. Apply inside the arrow wound. Pain will subside shortly, followed by slight itching; endure this. Wait until the itching becomes unbearable, then shake the arrowhead to extract it. The effect is miraculous.
Formula for treating arrowheads that do not expel:
Qiang Lang (Scarabaeus sisyphius), preferably collected on the fifth day of the fifth lunar month; Ban Mao (Mylabris), seven pieces each, pricked onto thorns and sun-dried, finely ground; Pi Huang (Arsenic Trioxide/Yellow Arsenic), finely ground, half liang.
Mix the above three ingredients uniformly. Place inside a three-cun long green bamboo tube. Seal the mouth with wax paper, and further seal with wax paper. Immerse in a latrine for twenty-one days (three seven-day periods). Remove, wash, and dry. Use a amount the size of a bean. Wait for itching to occur, then press to facilitate expulsion.
Formula for treating arrowheads that do not expel: Bailian San (Ampelopsis Powder).
Bai Lian (Ampelopsis Radix), 2 fen; Mudan (Paeoniae Moutan Cortex), with heart removed, 3 fen.
The above two herbs are crushed and sifted into a powder. Take 3 qian bi per dose, mixed with warm wine. Administer on an empty stomach, at noon, midnight, and before sleep.
