Commentary states: The character dictionary interprets "scabies" (Jie, 疥) as having the meaning of "guarding firmly" (Jie Shou, 介守), implying that although the condition is minor, it adheres stubbornly to the body and is difficult to cure. Therefore, scabies has five names, all caused by wind-heat pathogenic factors invading and hiding within the muscle tissue, thus often involving parasites. Some believe these are pinworms emerging from within; this reasoning may be plausible. Treatment should focus on dispersing wind and purging heat. It is appropriate to employ fumigation, bathing, external application, and smearing methods, which yield better results when combined with internal decoction therapy.
For treating widespread skin sores and scabies accompanied by itching due to wind-toxin: Kushen San (Bitterroot Powder).
Ingredients: Sophorae Radix (Kushen, 苦参); Dendrobii Caulis (Baohuashe, 白花蛇 - processed with wine, skin and bones removed, roasted, 1 liang each); Coptidis Rhizoma (Huanglian, 黄连 - remove stamens); Angelicae Sinensis Radix (Danggui, 当归 - sliced and dried, 3 qian each); Ginseng Radix (Renshen, 人参); Scrophulariae Radix (Xuanshen, 玄参); Salviae Miltiorrhizae Radix (Danshen, 丹参); Glehniae Radix (Shashen, 沙参); Paeoniae Radix (Shaoyao, 芍药); Tribuli Fructus (Jilizi, 蒺藜子 - stir-fried, spines removed); Saposhnikoviae Radix (Fangfeng, 防风 - forked parts removed, 0.5 liang each).
Procedure: Grind the above eleven ingredients into a fine powder. Take 2 qianbi (a small measure) mixed with warm wine, twice daily, at any time.
For treating persistent widespread scabies: Kuaiji Wan (Muscle-Rapid Pill).
Ingredients: Clematidis Radix et Rhizoma (Weilingxian, 威灵仙 - cleaned of soil, 1.5 liang, ground to fine powder); Porcine Bile (Zhudan, 猪胆 - three pieces).
Procedure: Mix the bile with the powder, roll into pills the size of lucerne seeds. Take 20 pills per dose, washed down with Schizonepeta Decoction, at any time.
For treating all wind-sores, scabies, and tinea, characterized by skin itching and wheals upon scratching: Fangfeng Wan (Saposhnikovia Pill).
Ingredients: Saposhnikoviae Radix (Fangfeng, 防风 - forked parts removed); Cryptotympanae Pergamen (Chanke, 蝉壳); Gleditsiae Sinensis Fructus (Zhuyazaojia, 猪牙皂荚 - roasted with lard, skin and seeds removed, 1.5 liang each); Gastrodiae Rhizoma (Tianma, 天麻 - 2 liang).
Procedure: Grind into fine powder. Cook lean mutton until soft, mash it, and boil with wine to form a paste. Roll into pills the size of mung beans. Take 30 pills per dose, washed down with Schizonepeta Wine or tea water.
For treating wind-toxin scabies and tinea, Jiajian Bafeng San (Modified Eight-Wind Powder):
Ingredients: Angelicae Pubescentis Radix (Duohuo, 独活 - rhizome removed); Saposhnikoviae Radix (Fangfeng, 防风 - forked parts removed); Scutellariae Radix (Huangqin, 黄芩 - sliced); Glycyrrhizae Radix et Rhizoma (Gancao, 甘草 - roasted until red, sliced, 1.1 liang each); Scrophulariae Radix (Xuanshen, 玄参); Sophorae Radix (Kushen, 苦参); Ligustici Chuanxiong Rhizoma (Xiong, 川芎); Gentianae Macrophyllae Radix (Qinjiao, 秦艽 - seedlings and soil removed, 1 liang each); Atractylodis Macrocephalae Rhizoma (Baizhu, 白术 - stir-fried until purple); Pinus Resin (Songzhi, 松脂 - 1.1 liang each); Cnidii Fructus (Shechuangzi, 蛇床子 - 3 qian); Coptidis Rhizoma (Huanglian, 黄连 - remove stamens); Cinnamomi Ramulus (Guizhi, 桂枝 - roots and joints removed); Armeniacae Semen Amarum (Xingren, 杏仁 - skin and tips removed); Rehmanniae Radix Preparata (Dihuang, 地黄 - baked, 2 liang); Polygalae Radix (Yuanzhi - heart part, 1 liang).
Procedure: Grind the above thirty-five ingredients into a powder. Take 2 qianbi per dose, mixed with ginger-honey water on an empty stomach, and again in the evening. Gradually increase to three doses.
For treating widespread skin sores and scabies with wind-toxin, manifesting as itching or pain: Kushen San (Bitterroot Powder):

For treating persistent itching and pain from scabies: Dahuang Wan (Rhubarb Pill):
Ingredients: Rhei Radix et Rhizoma (Dahuang, 大黄 - sliced and stir-fried, 2.5 liang); Saposhnikoviae Radix (Fangfeng, 防风 - forked parts removed); Scutellariae Radix (Huangqin, 黄芩 - sliced); Coptidis Rhizoma (Huanglian, 黄连 - remove stamens, 1.5 liang each); Euphorbiae Humifusae Radix (Loulu, 漏芦 - rhizome removed, 1 liang); Gentianae Macrophyllae Radix (Qinjiao, 秦艽 - seedlings and soil removed); Sophorae Radix (Kushen, 苦参); Natrix Humpbacked (Wushe, 乌蛇 - soaked in wine, roasted yellow, skin and bones removed, 2 liang each).

Procedure: Grind the above ten ingredients into powder, mix with refined honey to make pills the size of lucerne seeds. Take 20 pills with warm wine on an empty stomach, morning and evening, until cured.
For treating persistent scabies and tinea: Wushe San (Black Snake Powder):
Ingredients: Natrix Humpbacked (Wushe, 乌蛇 - soaked in wine, roasted yellow, skin and bones removed, 2 liang); Euphorbiae Humifusae Radix (Loulu, 漏芦 - rhizome removed, 1.5 liang); Rhei Radix et Rhizoma (Dahuang, 大黄 - sliced, stir-fried until fragrant); Notopterygii Rhizoma et Radix (Qianghuo, 羌活 - rhizome removed); Salviae Miltiorrhizae Radix (Danshen, 丹参); Glehniae Radix (Shashen, 沙参); Scrophulariae Radix (Xuanshen, 玄参); Acanthopanacis Cortex (Wujiaopi, 五加皮 - sliced); Glycyrrhizae Radix et Rhizoma (Gancao, 甘草 - roasted red, sliced); Bombyx Batryticatus (Baijiangcan, 白僵蚕 - stir-fried); Scorpio (Ganye, 干蝎 - stir-fried, cleaned of soil, 1 liang each).

For treating various scabies, wind-deficiency, and malignant sores: Wushen San (Five-Root Powder):
Ingredients: Ginseng Radix (Renshen, 人参); Scrophulariae Radix (Xuanshen, 玄参); Salviae Miltiorrhizae Radix (Danshen, 丹参); Glehniae Radix (Shashen, 沙参); Sophorae Radix (Kushen, 苦参 - 1 liang each); Tribuli Fructus (Jilizi, 蒺藜子 - stir-fried, pounded, spines removed); Gentianae Macrophyllae Radix (Qinjiao, 秦艽 - seedlings and soil removed, 0.5 liang each); Gardeniae Fructus (Zhizi, 栀子); Angelicae Pubescentis Radix (Duohuo, 独活 - 2 liang each).
Procedure: Grind the above seventeen ingredients into powder. Take 3 qianbi mixed with boiled water on an empty stomach, morning and evening, until cured.
For treating dry and wet scabies: Chixiaodou San (Red Bean Powder):
Ingredients: Phaseoli Semen Rubrum (Chixiaodou, 赤小豆 - 3 he, stir-fried dry, soaked in vinegar seven times); Ginseng Radix (Renshen, 人参); Glycyrrhizae Radix et Rhizoma (Gancao, 甘草 - roasted red, sliced); Diantherae Herba (Qumai Sui, 瞿麦穗); Ampelopsis Radix (Bailian, 白蔹); Angelicae Sinensis Radix (Danggui, 当归 - sliced and dried); Scutellariae Radix (Huangqin, 黄芩 - black hearts removed); Polyporus (Zuling, 猪苓 - black skin removed); Saposhnikoviae Radix (Fangfeng, 防风 - forked parts removed, 0.5 liang each); Scutellariae Radix (Huangqin, 黄芩 - sliced); Coicis Semen (Yiyiren, 薏苡仁); Cimicifugae Rhizoma (Shengma, 升麻 - 3 qian each).
Procedure: Grind the above twelve ingredients into fine powder. Take 3 qianbi mixed with gruel drink on an empty stomach, twice daily and once at night.
For treating dry scabies with persistent itching: Huangqi Wan (Astragalus Pill):
Ingredients: Astragali Membranacei Radix (Huangqi, 黄芪 - 2 liang sliced); Natrix Humpbacked (Wushe, 乌蛇 - 4 liang, soaked in wine, skin and bones removed, roasted yellow); Aconiti Lateralis Radix Preparata (Chuanwutou, 川乌头 - 3 liang, processed, cracked, skin and navel removed); Aconiti Radix Preparata (Fuzi, 附子 - 2 liang, processed, cracked, skin and navel removed); Ilicis Cornuti Folium (Yinyu, 茵芋 - 2 liang); Pyri Radiatae Caulis (Shinan, 石南 - 1 liang); Gentianae Macrophyllae Radix (Qinjiao, 秦艽 - 2 liang, seedlings removed).
Procedure: Grind into powder, mix with refined honey and pound 300-200 times. Make pills the size of lucerne seeds. Take 30 pills after meals, washed down with Schizonepeta Decoction, until cured.
For treating widespread dry scabies itching, where scratching raises the skin: Qinjiao Wan (Gentian Pill):

Ingredients: Arecae Semen (Binglang, 槟榔 - sliced); Notopterygii Rhizoma et Radix (Qianghuo, 羌活 - rhizome removed); Angelicae Pubescentis Radix (Duohuo, 独活 - rhizome removed); Aurantii Fructus (Zhiqiao, 枳壳 - pulp removed, bran-roasted); Pharbitidis Semen Album (Baiqianniu, 白牵牛 - lightly stir-fried); Pharbitidis Semen Nigrum (Heiqianniu, 黑牵牛 - lightly stir-fried, 0.5 liang each).
Procedure: Pound and sieve into powder. Use one large Gleditsia sinensis pod (over 1 chi) to extract juice, boil into a paste, and mix with the powder to make pills the size of lucerne seeds. Take 30 pills before bed with warm wine or boiled water, adjusting dosage based on deficiency or excess. For chronic scabies, immediate cure is expected.

Procedure: Grind into fine powder. Use ginger juice and wheat flour paste to make pills the size of mung beans. Take 10 pills on an empty stomach and before bed, washed down with stir-fried salt water.
For treating sores and scabies lasting years without healing: Wulong Wan (Black Dragon Pill):
Ingredients: Pharbitidis Semen (Qianniuzi, 牵牛子 - arbitrary amount, spread on tile, slow-fire dried without stirring until fragrant, take half raw half cooked, cool and grind to fine powder).
Procedure: Use two large Gleditsia pods, soak in one bowl of water, squeeze out juice, filter, and boil in a silver/ceramic vessel into a paste. Mix with Pharbitis powder to make pills the size of lucerne seeds. Take 20 pills after meals and before bed with warm wine. If mild diarrhea occurs, stop taking. Sores will cease immediately, usually cured within 3-5 doses.
For treating widespread dry and wet scabies sores that are untreatable: Heshouwu Wash Decoction:
Ingredients: Polygoni Multiflori Radix (Heshouwu, 何首乌); Artemisiae Argyi Folium (Ai, 艾) - equal parts, finely sliced.

Ingredients: Evodiae Fructus (Wuzhuyu, 吴茱萸 - 1 liang); Phaseoli Semen Rubrum (Chixiaodou, 赤小豆 - 49 grains); Tribuli Fructus (Baijili, 白蒺藜 - 1 liang); Alpiniae Katsumadai Semen (Baiwanyi Ren, 白芜荑仁 - 0.5 liang); Calomelas (Qingfen, 轻粉 - 5 qianbi); Sulphur (Shiuhuang, 石硫黄 - a little, ground).
Procedure: Pound and grind into a uniform powder. Take half a qianbi mixed with raw sesame oil, rub between palms until warm, then wipe over all scabies-affected areas before sleeping. Sores will heal upon waking.
For treating various scabies: Pi Huang Fumigation:
Ingredients: Realgar (Pi Huang, 砒黄 - ground); Realgar (Xionghuang, 雄黄 - ground); Sulphur (Shiuhuang, 石硫黄 - ground, 0.5 liang each); Prepared Mugwort (Shuai, 熟艾 - 5 liang).
Procedure: Grind and mix uniformly. Spread on paper, roll tightly like a cake, seal with flour paste, cut into four sections. Patient covers [affected area] in the morning [text implies fumigation or covering].
For treating various scabies: Wutou San (Aconite Powder):
Ingredients: Aconiti Radix (Wutou, 乌头); Evodiae Fructus (Wuzhuyu, 吴茱萸); Sulphur (Shiuhuang, 石硫黄); Datura Seed (Langdangzi, 莨菪子 - 1 liang each).
Procedure: Grind into powder, mix with raw sesame oil to form a paste. Apply to affected areas, 3-5 times daily, until cured.
For treating malignant scabies: Ji Zi Smear:
Ingredients: Chicken Egg (Jizi, 鸡子 - 7 eggs, boiled, yolks extracted and fried in a pan into a paste); Calomelas (Nifen, 腻粉); Ash of tangled hair (Luankuihui, 乱发灰); Alum Ash (Baijianhui, 白矾灰 - 1 fen each); Sulphur (Shiuhuang, 石硫黄 - 0.5 liang, ground).

Ingredients: Veratris Radix (Lilu, 藜芦 - rhizome removed, 1.5 liang); Sulphur (Shiuhuang, 石硫黄 - ground); Gleditsiae Sinensis Fructus (Zhuyazaojia, 猪牙皂荚); Boswelliae Resina (Ruxiang, 乳香 - 0.5 liang each, ground); Aconiti Radix Preparata (Fuzi, 附子 - 1 fen); Armeniacae Semen Amarum (Xingren, 杏仁 - skin removed, ground, 0.5 liang); Calomelas (Nifen, 腻粉 - 1 fen); Crotonis Fructus (Badou, 巴豆 - shell removed, ground, 0.5 fen); Porcine Bile (Zhudan, 猪胆 - 2 pieces); Lard (Su, 酥 - 2 liang); Alum Ash (Baijianhui, 白矾灰 - 1 fen, ground); Pig Fat (Zhu Zhi, 猪脂 - 2 liang).
Procedure: Grind nine ingredients into powder. Boil lard and bile, add powders, mix well. Store in ceramic jar. Apply/rub 3-5 times daily.
For treating various scabies: Sulphur Application:
Ingredients: Sulphur (Shiuhuang, 石硫黄 - 1 qian, ground); Sichuan Pepper (Shujiao, 蜀椒 - remove eyes and closed ones); Evodiae Fructus (Wuzhuyu, 吴茱萸); Phellodendri Cortex (Huangbai, 黄柏 - 1 liang each).
Procedure: Grind into powder, mix with raw sesame oil to form a paste. Apply to scabies, 2-3 times daily.
For treating dry scabies not healing for long, with skin itching: Formula:
Mercury (Shuiyin, 水银 - 1 fen, mixed with Lead Carbonate/Hufen, add little water, grind until stars disappear); Lead Carbonate (Hufen, 胡粉 - 1 liang); Cnidii Fructus (Shechuangzi, 蛇床子 - 0.5 liang, ground to powder); Coptidis Rhizoma (Huanglian, 黄连 - 3 qian, remove stamens, ground to powder).
Procedure: Grind finely and mix. Blend with raw hemp seed oil into a thin paste. Before application, wash with salt brine, then apply medicine. Replace when dry. Cured in 3-5 applications.

Ingredients: Gleditsiae Sinensis Fructus (Zhuyazaojia, 猪牙皂荚); Calomelas (Nifen, 腻粉); Sulphur (Liuhuang, 硫黄 - finely ground); Odorous Yellow (Chouhuang, 臭黄 - finely ground); Alum Ash (Baijianhui, 白矾灰); Beeswax (Huangla, 黄蜡); Crotonis Fructus (Badou, 巴豆 - peeled); Aconiti Radix (Wutou, 乌头 - raw use); Evodiae Fructus (Wuzhuyu, 吴茱萸) - 1 fen each.
Procedure: Grind into powder, re-grind to mix. Add 2-3 he of hemp seed oil, melt wax over low fire, mix well. Apply twice daily.
Another Formula:
Alum (Baijian, 白矾 - 1 liang, burned to ash); Sulphur (Liuhuang, 硫黄 - 1 liang, finely ground); Coptidis Rhizoma (Huanglian, 黄连 - 1.5 liang, remove stamens, ground); Orpiment (Cihuang, 雌黄 - 1 liang, finely ground); Cnidii Fructus (Shechuangzi, 蛇床子 - 3 qian, ground).
Procedure: Grind finely and mix. Blend with rendered pork fat and honey-like consistency. Wash thoroughly with salt brine first, dry, then apply.
For treating scabies with dry crusts and unceasing itching: Formula:

Procedure: Boil pig fat, add Badou, cook until color changes, add wax to melt, then add sulphur powder. Mix well, store in ceramic jar. Apply 3-5 times daily.
Another Formula:
Sulphur (Liuhuang, 硫黄 - 1 liang); Niter (Xiaoshi, 硝石 - 0.5 liang).
Procedure: Grind to powder, mix with raw hemp seed oil and apply.
For treating all scabies, wind-itch, and sores: Wushe San (Black Snake Powder):
Ingredients: Natrix Humpbacked (Wushe, 乌蛇 - coated with wine, roasted, skin and bones removed, 2 liang); Notopterygii Rhizoma et Radix (Qianghuo, 羌活 - rhizome removed); Dictamni Cortex (Baixianpi, 白鲜皮); Sophorae Radix (Kushen, 苦参); Zanthoxyli Fructus (Zhishi, 枳实 - pulp removed, bran-roasted); Tribuli Fructus (Jilizi, 蒺藜子 - stir-fried); Bombyx Batryticatus (Baijiangcan, 僵蚕 - stir-fried); Saposhnikoviae Radix (Fangfeng, 防风 - forked parts removed); Scutellariae Radix (Huangqin, 黄芩 - black hearts removed); Coptidis Rhizoma (Huanglian, 黄连 - remove stamens); Angelicae Sinensis Radix (Danggui, 当归 - sliced and dried); Ginseng Radix (Renshen, 人参); Salviae Miltiorrhizae Radix (Danshen, 丹参); Glehniae Radix (Shashen, 沙参); Scrophulariae Radix (Xuanshen, 玄参); Glycyrrhizae Radix et Rhizoma (Gancao, 甘草 - roasted); Atractylodis Macrocephalae Rhizoma (Baizhu, 白术 - stir-fried); Gentianae Macrophyllae Radix (Qinjiao, 秦艽 - seedlings and soil removed).
Procedure: Grind the above eighteen ingredients into fine powder. Take 5 qianbi mixed with wine on an empty stomach.
For treating all tinea, scabies, and leprosy-like sores: Qufeng Wan (Wind-Expelling Pill):
Ingredients: Sophorae Tree Teeth (Huaiya, 槐牙 - baked dry); Gleditsiae Sinensis Fructus Teeth (Zaojia Ya, 皂荚牙 - baked dry, 1 jin each); Sophorae Radix (Kushen, 苦参 - 3 liang); Saposhnikoviae Radix (Fangfeng, 防风 - forked parts removed); Notopterygii Rhizoma et Radix (Qianghuo, 羌活 - rhizome removed, 1.3 liang each); Natrix Humpbacked (Wushe, 乌蛇 - one strip, wine-roasted, skin and bones removed); Quisqualis Fructus (Shijunzi, 使君子 - 1.5 liang).
Procedure: Grind into powder, mix with refined honey to make pills the size of lucerne seeds. Take 30 pills with wine on an empty stomach, and with honey water at night.
For treating spleen-lung wind-toxin attacking and causing scabies/tinea: Formula:
Cimicifugae Rhizoma (Shengma, 升麻); Pruni Persicae Cortex (Taobaipi, 桃白皮); Sophorae Radix (Kushen, 苦参 - 0.5 liang each).
Procedure: Finely slice. Boil in 2 dou of water until reduced to 1 dou. Filter dregs, wait until warm, and wash.
For treating tinea and scabies: Formula:
Alum Ash (Baijianhui, 白矾灰); Sulphur (Shiuhuang, 石硫黄).
Procedure: Equal parts, grind to powder, mix with raw sesame oil and apply.
For treating all malignant scabies: Formula:
Phellodendri Cortex (Huangbai, 黄柏 - powder); Calcined Lime (Duanshi, 锻石 - ground).
Procedure: Equal parts, grind extremely fine. Mix with raw hemp seed oil into a thin batter. Wash sores cleanly, apply 4-5 times until cured.
For treating severe sores and scabies: Wudou Decoction:
Aconiti Radix (Wutou, 乌头 - 1 liang, each piece split into four); Soybean (Dadou, 大豆 - 1.5 liang).
Procedure: Put together in a clay pot, boil until very soft. Take one piece of Aconite and a few soybeans with wine on an empty stomach.
