[Discussion on the Third Chapter of the Cold Damage Example] states: A pulse that is vigorous accompanied by cold body indicates contraction of cold damage (Shanghan); a pulse that is deficient accompanied by fever indicates contraction of heat damage (Shangshu).

Lin's commentary: This is a statement from Wang Shuhe's Preface, and it should not be indiscriminately attributed to Zhang Zhongjing. It is correct that a vigorous pulse corresponds to cold damage, but it must be vigorous in the left position; it is correct that a deficient pulse with fever corresponds to heat damage, but the deficient and large pulse must be vigorous in the right position. Moreover, there is no case of cold damage without fever; this preface only gives a general outline and should not be taken literally.

[Discussion on Dampness, Fourth Chapter] states: Taiyang heat stroke refers to Zhu Xia (a type of summerheat syndrome characterized by fatigue and low-grade fever). The patient sweats, fears cold, has a fever, and feels thirsty.

It also states: Taiyang heatstroke (Zhuyue) presents with fever, aversion to cold, heaviness and pain in the body, and a pulse that is string-taut, fine, hollow, and slow. After urination, the patient feels chilly with gooseflesh on the skin, and experiences cold extremities. Slight exertion causes the body temperature to rise. Breathing through an open mouth leads to dryness of the anterior teeth. If sweating-inducing methods are used, the aversion to cold will worsen; if warm needle therapy is applied, the fever will intensify; if purgatives are used repeatedly, strangury (lin syndrome) will become severe.

Commentary: For this syndrome, using only cooling herbs to clear internal fire, acrid herbs to disperse, and sour herbs to astringe, combined together, is excellent. Both sweating and purging methods are contraindicated. If warm needles are added, death will ensue rapidly.

Lin's commentary: Zhang Zhongjing treats heatstroke with three distinct methods—sweating, warming, and purging—which have been explained previously. The note stating that clearing internal fire with cooling herbs, dispersing with acrid herbs, and astringing with sour herbs is optimal is likely incomplete. Heaviness and pain in the body, along with a string-taut, fine, hollow, and slow pulse, indicate dampness excess. Can cold-cooling and sour-astringing methods treat this? This view is too superficial.

[Discussion on Differentiating Taiyang Disease Pulses, Fifth Chapter] states: If two or three days after contracting cold damage, symptoms of Yangming and Shaoyang do not appear, it means the disease has not transmitted.

Lin's commentary: This section demonstrates Zhang Zhongjing's flexible approach to disease transmission. Heaven possesses six qi (cold, heat, dryness, dampness, wind, fire), and human organs possess the six qi of Three Yang and Three Yin. When heaven's cold and heat affect humans, humans respond with their own cold qi, known as "same qi seeking each other." The *Lingshu · Ben Cang* chapter states: "The Sanjiao and Bladder correspond to the interstices and body hair." This indicates that Taiyang Cold Water governs the skin and hair, being the first layer of the body surface. Therefore, wind-cold first attacks Taiyang. However, some cases bypass Taiyang and directly affect Yangming, Shaoyang, or even the Three Yin channels. This occurs because whichever channel is deficient receives the pathogen. As stated in the *Bing Xing* chapter: "Attacks on the face descend to Yangming; attacks on the nape descend to Taiyang; attacks on the cheeks descend to Shaoyang; attacks on the chest, back, and flanks also strike their respective meridians." It also says: "Attacks on the Yin channels often begin at the arms." Regarding the transmission theory in the *Suwen · Re Lun*, which describes Day 1 as Taiyang, Day 2 as Yangming, Day 3 as Shaoyang, Day 4 as Taiyin, Day 5 as Shaoyin, and Day 6 as Jueyin, this refers to the sequential transmission of the six qi within the body, representing the transmission of qi rather than disease. However, where the body's qi is deficient, the pathogen enters there, as the saying goes, "Where the pathogen gathers, the qi must be deficient." Zhang Zhongjing feared that practitioners would rigidly adhere to day-by-day transmission, so he stated, "If two or three days after cold damage, Yangming and Shaoyang symptoms are absent, it has not transmitted." If transmission to Yangming occurred on the second day, symptoms such as fever, eye pain, nasal dryness, and insomnia would appear. On the third day, involving Shaoyang, chest/hypochondriac pain and deafness would appear. If these symptoms are absent, transmission has not occurred, and the same applies to the Three Yin. Thus, transmission should not be rigidly calculated by days; instead, treatment should follow the specific channel exhibiting symptoms. The omission of Hand channels when discussing Foot channels is merely for brevity. One must understand the mutual root of Yin and Yang: cold pathogens first attack the Foot Taiyang, while heat pathogens first attack the Hand Taiyin. This principle of mutual rooting is immutable. Later theories claiming cold damage transmits strictly by day are fundamentally wrong. Conversely, asserting that warm diseases and summerheat only affect the Sanjiao and do not transmit to other channels is also incorrect. How else can one explain the day-by-day transmission described in the *Re Lun*? Cold damage can directly invade the Three Yin; warm-heat latent qi can emerge from Shaoyin into Shaoyang; summerheat pathogens can enter the Heart, Liver, or Spleen. These variations cannot be treated with rigid rules.

It also states: Taiyang disease presenting with fever, thirst, and no aversion to cold is called Warm Disease (Wenbing).

Commentary: Warm disease does not belong to a single specific channel; it depends on where the pathogen strikes, differentiated by pulse. Some cases do not involve Taiyang and initially lack headache and aversion to cold. Quack doctors may mistake this for internal injury due to phlegm-fire, leading to immediate disaster. This condition does not transmit; it affects only one or two channels. Caution is essential.

Lin's commentary: Warm disease arises from the latent yang-heat stored in the bones and muscles during winter, suppressed by severe cold. In spring, this suppressed heat is triggered by external pathogens and erupts. When latent heat emerges to the skin and meets the warm air of spring, the interaction of the two heats creates Warm Disease, consistent with the *Suwen · Shengqi Tongtian Lun*: "Winter injury by cold leads to Warm Disease in Spring." Since the external pathogen is light, there is no aversion to cold, only fever and thirst. Mild aversion to cold followed quickly by fever and thirst indicates a heavier external pathogen. If there is a sticky mouth coating, greasy tongue coating, body pain, loose stools, sweating without relief, and a soggy-rapid pulse, it is not Wind Strike but Warm Disease complicated by Dampness. Internal emission of warm pathogens typically shows a stronger pulse on the right side, as heat damages Qi. External wind-heat first injures the Hand Taiyin Lung meridian, while wind-cold first injures the Foot Taiyang Bladder meridian, because both the Lung and Bladder govern the exterior. Mahuang Tang, used for cold damage, acts to drain the Lungs; thus, mentioning Taiyang implicitly includes Hand Taiyin, again for brevity. Warm-heat should be treated by examining the Sanjiao. Transmission follows the same principles as cold damage: treat based on the affected channel, differing only in the hot/cold nature of the herbs. Those who claim warm-heat does not transmit and focuses solely on the Sanjiao contradict the *Re Lun*'s description of day-by-day transmission. The statement that warm disease has no specific channel comes from Yu Ren's *Nan Jing* Difficulties No. 58 referring to epidemic diseases, and should not be confused with winter latent qi warm-heat diseases.

It also states: If after inducing sweating, the body becomes burning hot, it is named Wind-Warmth (Fengwen). Wind-Warmth presents with floating pulses at both cun and chi positions, spontaneous sweating, heavy body, excessive sleepiness, snoring breath, and difficulty speaking. If purged incorrectly, there will be difficulty urinating, fixed staring, and incontinence. If treated with fire therapy, mild cases turn yellow; severe cases resemble convulsions or epilepsy with periodic spasms. If scorched by fire, one error may prolong life, but a second error will hasten death.

Commentary: These cases are all Spring Warmth, caused by internal fire combined with external wind-cold, leading to rapid onset. Sleepiness and difficulty speaking result from internal fire scorching, not severe external wind-warmth. Spontaneous sweating indicates wind injuring the Wei (defensive) qi; absence of aversion to cold suggests mild external symptoms. Only acrid-cool herbs for internal resolution and mild exterior release are appropriate.

Lin's commentary: Wind-Warmth consists of internally accumulated latent heat with externally contracted wind pathogen. Aversion to wind and fear of cold are possible symptoms, but fever, thirst, and cough are necessary. Burning heat after sweating results from mistakenly using acrid-warm exterior-releasing herbs. Wind herbs are drying; they trigger latent qi, causing wind and fire to fan each other, resulting in spontaneous sweating, heaviness, somnolence, snoring, and speech difficulties. These are complications from fluid depletion due to erroneous sweating, not inherent to early-stage Wind-Warmth. Further errors with purging or fire therapy lead to fixed staring, incontinence,惊惕 (startle/fear), and spasms. One error may delay death, but a second accelerates it. This proves that initial mismanagement of the exterior, followed by erroneous purging and fire therapy, leads to incurable conditions. This confirms Wind-Warmth is a dry-heat disease. Clearing heat and nourishing yin is the definitive treatment. Even if slightly complicated by exterior pathogens, only light acrid-cool agents like Cong Bai (Scallion Stem), Dou Chi (Fermented Soybean), Niu Bang Zi (Arctium Seed), and Bo He (Mint) should be used to vent kidney qi and disperse wind-heat. It is absolutely wrong to recklessly use Ephedra Twig and Cinnamon Twig Decoctions.
