(Treatise One)
The treatise states: The Spleen governs "Yi" (Intent/Memory). The Spleen organ is the residence of Yi. Yi refers to the will of memory and retention. In official terms, it corresponds to the Remonstrance Official, and receives influences from the other four Zang organs. What arises in the Heart is called Yi; what is retained by Yi is called Zhi (Will); thinking and changing based on Zhi is called Si (Reflection); far-reaching planning based on Si is called Lv (Deliberation); handling matters based on Lv is called Zhi (Wisdom). Yi is the spirit stored by the Spleen. The lips are the external manifestation of the Spleen. Spleen Qi connects to the mouth; when the mouth functions harmoniously, one can distinguish the flavors of the five grains. Thus, it is said that the mouth belongs to Wu (Yang Earth), and the tongue and lips belong to Ji (Yin Earth), circulating in the Central Palace (Spleen/Stomach), emerging upwards to the cheeks, then observing the lips, and returning downwards to the Spleen. Glory manifests in the tongue. Externally, it governs muscles; internally, it governs taste. The Spleen weighs two jin and three liang, is flat and wide three cun, and long five cun. It also has a half-jin loose pancreas (San Gao), which primarily wraps blood and warms the five Zang organs. Its spiritual name is Bibi, governing the storage of Ying (Nutritive) Qi (or Yi in some texts). Its title is the Organ of Intent. It follows seasonal rhythms and gatherings, hence it is said that the Spleen stores Ying, and Ying houses Yi. In sound, it manifests as belching (Yi); in fluid, it manifests as saliva (Xian). When Spleen Qi is deficient, the limbs cannot be used freely, and the five Zang organs are unsettled. When Spleen Qi is excess, there is abdominal distension and difficulty with urination and defecation. Spleen Qi deficiency leads to dreams of insufficient food; if it occurs during its appropriate season, it leads to dreams of building walls and houses. Spleen Qi excess leads to dreams of singing and joy, heavy body, and inability to lift hands and feet. If Jue (Rebellious) Qi lodges in the Spleen, one dreams of hills, large marshes, ruined houses, and storms. Generally, the Spleen corresponds to Earth and combines with the Stomach to form a Fu organ. Its meridian is the Foot Taiyin, which is interior-exterior related to the Foot Yangming. Its pulse is slow (Huan), corresponding to Summer, and flourishing in Late Summer (Ji Xia). The Spleen is Earth, solid and benevolent. Solid implies density. All things have diverse colors, hence named "De Fu" (Receiving Blessings/Broad). All things hang by roots and stems, with leaves at the top. Flying insects and crawling creatures, those breathing and panting, all receive the grace of Earth, becoming slow/harmonious. Grace implies slowness, thus making the Taiyin pulse slow and delayed. The Cun, Guan, Chi positions differ. Sour, salty, bitter, pungent flavors arise wondrously, each operating in its own time without mixing, and can be taken regularly for regulation. If Earth is cold, warm the Earth; if hot, cool the Earth. There is a son named Jin Huai, held close to the body. Metal fears Fire, fearing heat scorching it, so it abandons its mother (Earth) and flees to Water. Water, as the child of Metal, hides the Fire Spirit, closing doors and windows, blocking internal and external communication. This is called Winter, when Earth loses its son (Metal), its Qi becomes weak. Water overflows and infiltrates the ground, striking the skin, causing facial and eye swelling, returning to the limbs. Foolish doctors see edema and use purgatives directly, causing Spleen deficiency and Stomach emptiness, allowing Water to occupy the space. Lung becomes asthmatic and floating due to water. Liver, fearing Lung, sinks deep. Below are thorns, fearing injury, avoiding to the side, like flowing water. Heart deficiency makes the pulse hidden (Fu); Liver weakness makes it sunken (Chen), thus the pulse is Fu and Chen. Wise doctors look far ahead, adjusting acupoints, promoting urination, thus unblocking water pathways. Sweet fluids flow down, stopping Yin-Yang imbalance. Asthma becomes slight, sweat flows normally. Liver takes root, Heart Qi rises. Yang moves through the limbs, Lung Qi stands tall, asthma settles. Kidney produces calm sounds, its flavor is salty. Leaning on the mother's decay, the smell is fishy. When Earth obtains its son (Metal), it forms the order of the four seasons and variations of reversal and smoothness. But what does the Spleen pulse alone govern? The Spleen pulse is Earth. The solitary organ irrigates the four directions. Its healthy state is invisible; its pathological state is visible. How does the pathological state arrive? Like flowing water, this is called Excess (Tai Guo), disease is external. Like a bird's beak, this is called Deficiency (Bu Ji), disease is internal. Excess causes heavy limbs unable to lift. Deficiency causes obstruction of the nine orifices, named Chong Qiang (Heavy Rigidity). A Spleen pulse that comes and goes with harmony and softness, like a chicken stepping on the ground, is a Normal Pulse. Late Summer takes Stomach Qi as the root. A Spleen pulse that is solid, full, and rapid, like a chicken lifting its foot, indicates Spleen Disease. A Spleen pulse that is firm and sharp like a chicken's beak, a bird's spur, leaking like a roof, or flowing rapidly, indicates Spleen Death. The True Spleen pulse arrives weak, alternating between sparse and rapid (or numerous), with yellow-green color lacking luster, hair breaking leads to death. Late Summer with slight Ru Ruo (Soft and Weak) Stomach Qi is Normal. More Ruo than Stomach Qi is Spleen Disease. Only Dai (Intermittent) pulse without Stomach Qi is Death. Ru Ruo combined with Shi (Kidney/Water) pulse indicates Winter Disease; strong Shi indicates current illness. Spleen stores Ying, Ying houses Yi. Unresolved worry injures Yi; injured Yi causes confusion and inability to lift limbs, with withered hair and dull complexion, leading to death in Spring. When Foot Taiyin Qi perishes, the pulse fails to nourish the lips. Lips are the root of muscles. If the pulse does not nourish, muscles rot. Rotting muscles cause fullness in the philtrum. Fullness in the philtrum causes lip retraction. Lip retraction means flesh dies first. Jia day severe, Yi day dead, because Wood conquers Earth. Spleen dying organ pulse is floating, large, and slow (or firm). Pressed, it feels like an inverted cup, clean and shaking, indicating death. June, Late Summer, builds Wei. Between Kun's end and Earth's position is the time of Spleen flourishing. Pulse large, grand, and slow is Normal. Reversely obtaining floating, large, and洪 (Surging) is Heart overcoming Spleen. Mother returning to son is Empty Evil; though sick, easy to treat. Reversely obtaining slight, choppy, and short is Lung overcoming Spleen. Son overcoming mother is Solid Evil; though sick, self-healing. Reversely obtaining dizzy and long is Liver overcoming Spleen. Wood conquering Earth is Thief Evil, Great Reversal, ten deaths, untreatable. Reversely obtaining sunken, soft, and slippery is Kidney overcoming Spleen. Water flooding Earth is Minor Evil; though sick, quickly recovers. Right hand Guan position Yin absolute means no Spleen pulse. Suffering from shortness of breath, diarrhea, abdominal fullness, body heaviness, limbs unwilling to move, frequent vomiting, puncture Foot Yangming to treat Yang. Right hand Guan position Yin excess means Spleen Excess. Suffering from internal hardness like stones, difficult defecation, puncture Foot Taiyin to treat Yin.
Spleen pulse long and weak, coming sparse and going rapid (or numerous), two beats is Normal, three beats is Li Jing Disease, four beats depletes essence, five beats death, six beats life ends. This is the Foot Taiyin pulse.
Spleen pulse urgent extreme causes convulsions; slightly urgent causes diaphragm fullness, food/drink entering and returning out, profuse salivation after. Slow extreme causes flaccid paralysis; slightly slow causes wind flaccidity, limbs unusable, mind clear as if healthy. Large extreme causes being struck/fallen; slightly large causes Spleen hernia wrapping large pus/blood outside intestines/stomach. Small extreme causes cold/heat; slightly small causes Xiao Dao (Wasting-Thirst). Slippery extreme causes strangury; slightly slippery causes worm/toxin roundworm, intestinal borborygmus, heat/astringency. Extreme causes intestinal abscess; slightly slippery causes internal ulceration, passing much pus/blood.
Spleen pulse beating firm and long, with yellow color, indicates disease of shortness of breath. Soft and scattered with lack of luster indicates swelling of feet, resembling water-like yellow pulse arrival. Large and empty indicates accumulated Qi in abdomen, rebellious Qi named Jue Hernia. Women follow same method. Caused by illness, sweating limbs exposed to wind.
Bian Que says: Spleen disease shows sallow yellow complexion. Excess causes rigid tongue base. Deficiency causes many masses, frequent swallowing/diarrhea. If Yang Qi is strong, dreams of food/drink occur. Spleen sound is song, movement is belching, emotion is thought (Si). Thought injures Spleen. Essence/Qi merging into Spleen causes hunger. Sound governs Late Summer. Pathology in sound, take from meridians. Fear unresolved injures Essence. Injured Essence causes bone soreness, flaccid paralysis, spontaneous leakage of essence. Thus, five Zang store Essence. Do not injure. Injury leads to loss of guarding, Yin deficiency. Deficiency leads to no Qi. No Qi leads to death.
Disease starting in Spleen, blockage, body pain, weight, one day transmits to Stomach causing abdominal distension. Two days transmit to Kidney causing lower abdomen, waist/spine pain, shin soreness. Three days transmit to Bladder causing back/epaxial muscle pain, urinary closure. Ten days not improving, death. Winter at Ding hour, Summer late eating.
Disease in Spleen, improves at dusk, worsens at dawn, holds at noon, quiet at afternoon. (Su Wen says worsens at sunrise. Wang Bing says holding at noon is error). Suppose Spleen disease moves East. If eating pheasant/rabbit meat and wood fruits, failing to obtain them, it should manifest in Spring, getting disease on Jia/Yi days.
Generally, Spleen disease symptoms must include body heaviness, frequent hunger, foot flaccidity unable to gather (Su Wen says frequent muscle flaccidity, foot unable to gather; Jia Yi says suffering hunger, muscle flaccidity, foot unable to gather). Walking often causes convulsions, foot sole pain. Deficiency causes abdominal fullness, intestinal borborygmus, undigested stool diarrhea. Take its meridians: Foot Taiyin, Yangming, Shaoyin blood.
Spleen pulse pressed is Ru (soft), floating is Xu (deficient). Suffering abdominal distension, vexation/fullness, heat in stomach, poor appetite, undigested food, difficult defecation, limbs suffering bi-obstruction and occasional numbness. Caused by sexual activity, amenorrhea, or frequent menstruation.
Spleen disease shows yellow color, indigestion, abdominal distress/fullness, body weight/joint pain, difficult defecation. Pulse slight slow and long, this is treatable. Suitable to take Ping Wei Wan, Xie Pi Wan, Zhu Yu Wan, Fu Zi Tang. Spring puncture Yin Bai, Winter puncture Yin Ling Quan, both reducing. Summer puncture Da Du, Late Summer puncture Gongsun, Autumn puncture Shang Qiu, all tonifying. Also moxa Zhang Men 50 moxibullae, back 11th vertebra 100 moxibullae.
Pathogen in Spleen/Stomach, muscle pain. Yang excess, Yin deficiency, causes Heat Middle, frequent hunger. Yang deficiency, Yin excess, causes Cold Middle, intestinal borborygmus, abdominal pain. Both Yang/Yin excess, or both deficiency, causes cold and heat, all regulate San Li.
Being struck/fallen, or drunk/full entering bedchamber, sweating exposed to wind injures Spleen. Spleen injury causes middle Qi Yin-Yang separation, Yang not following Yin, thus judge life/death by three parts.
Spleen struck by Wind:翕翕 (He He) fever, shape like drunk person. Abdominal vexation/heaviness, skin/muscle distension and shortness of breath. Spleen struck by Cold.
Spleen Water: Person has large abdomen, limbs suffer heaviness, no generation of fluids, only suffering shortness of breath and difficult urination.
Spleen Distension: Frequent hiccups, limbs urgent (or solid), body heavy unable to dress (or gather). Duzhong pulse floating and choppy. Floating means Stomach Qi strong. Choppy means frequent urination. Floating/choppy contend, stool becomes hard. Spleen is constrained. Spleen Constrained: Person has hard stool, free urination but no thirst.
Spleen Qi weak, disease diarrhea passing white intestinal mucus, hard stool unable to change clothes, continuous sweating, named Spleen Qi Weakness. Or five fluids pouring down green/yellow/red/white/black.
Cunkou pulse string-like and slippery. String-like means pain. Slippery means excess. Pain means urgency. Excess means protrusion. Pain/protrusion contend, chest/hypochondrium snatching urgency. Duzhong pulse floating and choppy. Floating means Stomach Qi slight. Choppy means Spleen Qi decline. Slight/decline contend, breathing impossible. This is Spleen family losing measure.
Cunkou pulse double tight means entering. Qi does not exit. No exterior, has interior. Epigastric blockage firmness.
Duzhong pulse slight and choppy. Slight means no Stomach Qi. Choppy means injuring Spleen. Cold in diaphragm, yet purged. Cold accumulation not消散. Stomach slight, Spleen injured. Grain Qi does not move. After eating, auto-belching. Cold in chest/diaphragm. Upper empty, lower solid. Grain Qi blocked, is obstruction disease.
Cunkou pulse slow and delayed. Slow means Yang, Wei Qi grows. Delayed means Yin, Ying Qi urges (or insufficient). Ying/Wei both harmonious, Gang/Rou both obtained. San Jiang inherits. Qi must be strong.
Duzhong pulse slippery and tight. Slippery means Stomach Qi solid. Tight means Spleen Qi injured. Eating but not digesting, this is Spleen not treating. Able to eat but abdomen not full, this is Stomach Qi excess. Abdomen full but unable to eat, epigastria like hunger, this is Stomach Qi not moving, Heart Qi deficiency.
