To treat *Pifu* fistula (a fistula caused by the toxin of *Pifu*, an ant), which initially manifests on the neck and resembles *Shanghan* (Cold Damage disease) at onset. This condition arises from the failure to eliminate toxins from *Pifu* ingested in food, with its root in the Kidney (*Shen*). The principal herb is Shi (Stone/Mineral), assisted by Fangfeng.
Shi (Stone/Mineral), Fangfeng (Saposhnikovia Root), Zhimu (Anemarrhena Rhizome), Cihuang (Realgar), Taobaipi (Peach Bark), Gan Dihuang (Prepared Rehmannia Root), Duhuo (Angelica Pubescens Root), Qingdai (Indigo Naturalis), Banmao (Mylabris), Baizhi (Angelica Dahurica), Songzhi (Pinus Resin) [alternative: Baizhi (Cupressus Resin)], Shaoyao (Peony Root), Haizao (Sargassum), Danggui (Angelica Sinensis) [each 2 fen], Baizhu (Atractylodes Macrocephala Rhizome), Pi (Bark) [each 4 fen], Shujiao (Sichuan Pepper) [100 grains].
The above seventeen ingredients are crushed, sifted, and taken orally with water, one *qianbi* (approx. 1-2 grams) per dose, three times daily.
To treat *Qicao* fistula (a subcutaneous nodular fistula), which initially appears below the neck without distinct head or tail, resembling a jujube pit-sized mass migrating within the skin, causing alternating chills and fever along with chest and epigastric fullness. This condition results from excessive anger, joy, or weeping, with its root in the Heart (*Xin*). The principal herb is Fanshi (Alum), assisted by Baizhu.
Fanshi (Alum), Baizhu (Atractylodes Macrocephala Rhizome), Kongqing (Azurite), Danggui (Angelica Sinensis) [each 2 fen], Xixin (Asarum) [1 liang], Pi (Bark), Banmao (Mylabris), Gouqi (Lycium Fruit), Di Dan (Euphorbia Kansui Root) [each 1 fen], Gan Wunao (Dried Toad Venom/Gland) [size of three large soybeans].
The above ten ingredients are ground into powder, taken orally with vinegar solution, one *cunbi* (approx. 3-5 grams) per dose, three times daily. If the lesion is located superiorly, lie on one side; if inferiorly, use a high pillow, to facilitate the downward flow of the medicine.
To treat *Fuju* fistula (a fistula accompanied by chills and fever), which initially appears on the neck like two fingers wide, causing chills, fever, and drowsiness. This condition results from excessive worry and overthinking, with its root in the Gallbladder (*Dan*). The principal herb is Di Dan, assisted by Gancao.
Di Dan (Euphorbia Kansui Root), Xionghuang (Realgar), Ganjiang (Dried Ginger), Xuduan (Dipsacus Root), Shijueming (Concha Haliotidis), Gen (Root), Longdancao (Gentiana Root) [each 3 fen], Gancao (Licorice Root) [1 fen], Xixin (Asarum) [2 fen], Dahuang (Rhubarb Root) [half fen].
The above ten ingredients are crushed, sifted, and applied topically to the ulcer, four to five times daily. (In *Gujin Luyan*, Xionghuang is omitted and Liuhuang [Sulfur] is included).
To treat *Luoli* fistula (fistula resulting from ruptured cervical lymphatic tuberculosis), which initially appears on the neck with a fixed base, causing initial pain and chills/fever. This condition results from wet hair being tied up after washing, with sweat flowing onto the neck, with its root in the Kidney (*Shen*). The principal herb is Cihuang, assisted by Shaoyao.
Cihuang (Realgar), Shaoyao (Peony Root), Fuling (Poria), Xuduan (Dipsacus Root), Gan Dihuang (Prepared Rehmannia Root), Kongqing (Azurite), Shi (Stone/Mineral), Ganjiang (Dried Ginger), Jiegeng (Platycodon Root), Shujiao (Sichuan Pepper), Hengshan (Polygala Root), Hushen (Tiger Kidney), Li Rou (Fox Meat), Wunao (Toad Venom/Gland), Banmao (Mylabris), Fanshi (Alum) [each 1 fen], Fuzi (Prepared Aconite Root) [1 liang].
