The treatise states: When cold pathogen suddenly invades the five zang organs and six fu organs, it manifests as sudden heart pain and chest bi syndrome. Contracting cold, if mild, results in cough; if severe, it results in pain and diarrhea. Jué xīn tòng (Rebellious Heart Pain) draws from the back, prone to spasms, with a sensation as if an object touches the heart from behind. Body stooping indicates Shèn xīn tòng (Kidney Heart Pain). Jué xīn tòng accompanied by abdominal distension and fullness. Severe heart pain indicates Wèi xīn tòng (Stomach Heart Pain). Jué xīn tòng feeling like being pierced by a needle or awl into the heart, with extreme pain, indicates Pí xīn tòng (Spleen Heart Pain). Jué xīn tòng with a complexion ashen blue like dead ash, unable to take deep breaths all day, indicates Gān xīn tòng (Liver Heart Pain). Jué xīn tòng starting from the heart center when lying down, worsening with movement, but without color change, indicates Fèi xīn tòng (Lung Heart Pain). Zhēn xīn tòng (True Heart Pain) presents with cyanosis of hands and feet extending to the joints, severe heart pain, death if onset is in the morning and evening, or death if onset is in the evening and morning. Huí chóng xīn tòng (Roundworm Heart Pain) involves pain in the heart and abdomen where masses gather and disperse, moving up and down, with intermittent pain, heat in the abdomen, and frequent salivation, indicating roundworm biting. Press firmly with hand to prevent movement, needle with a large needle, retain for a long time until the worm stops moving, then withdraw the needle. The lower heart region must not be needled; lesions with accumulation inside should not be treated via shu points; intestinal roundworm biting must not be treated with small needles.
