Chapter: Seven Formulas for Cold Damage and Baihe Disease
Category: Formulas
[English Translation]
Regarding the etiology of Cold Damage and Baihe Disease (Lily Disease), it is stated that the meridians and all vessels share a single origin, thus disease affects them collectively. This condition typically arises after suffering from Cold Damage or consumptive diseases where recovery is incomplete, leading to this ailment. The symptoms include: desiring to eat but being unable to do so; often remaining silent and wishing to sleep, yet unable to sleep; desiring to go out but lacking the strength to walk; sometimes finding food palatable, at other times having no appetite, and even feeling aversion to the smell of food; the physical state may appear healthy, yet insomnia persists; sometimes feeling cold, then not cold; sometimes feeling hot, then not hot; suffering until dawn, with dark yellow urine. Baihe Disease is difficult to cure with various medications; improper medication exacerbates the condition, causing vomiting and diarrhea, as if afflicted by spirits. The body shape appears normal, but the pulse is faint and rapid (Wei Shu). Headache occurs with each urination; the disease resolves in sixty days. If there is no headache during urination but rather a sensation of chilliness (Xi Xi Ran), it resolves in forty days. If urination brings relief but is accompanied by dizziness, it resolves in twenty days. These manifestations may appear before the onset of illness, emerge four to five days after onset, or recur after twenty days or a month. If the symptoms include aversion to cold and vomiting, the pathology is in the Upper Jiao (Upper Burner), and resolution is expected in twenty-three days. If the symptoms include abdominal fullness, slight wheezing, hard stool, defecation only once every three to four days, occasionally with loose stools, the pathology is in the Middle Jiao (Middle Burner), and resolution is expected in sixty-three days. If the symptoms include difficult, dribbling urination, the pathology is in the Lower Jiao (Lower Burner), and resolution is expected in forty-three days. Treatment should follow the specific syndrome patterns. (Excerpted from Volume 8)
Zhang Zhongjing’s *Shanghan Lun* (Treatise on Cold Damage) states regarding the treatment of Baihe Disease: Various drugs are ineffective; if administered, the condition worsens with vomiting and dysentery, as if possessed. The body remains physically calm, the pulse is faint and rapid, and headache accompanies each urination; resolution takes sixty days. If there is no headache during urination but a sensation of chilliness, resolution takes forty days. If urination brings relief but only dizziness remains, resolution takes twenty days. All these syndromes, whether appearing pre-onset, emerging four to five days post-onset, or recurring after twenty days to a month, are treated accordingly.
Furthermore, for cases relapsing after sweating therapy, Baihe Zhi Mu Tang (Lily and Anemone Decoction) is the primary formula.
Baihe (Lilii Bulbus, 7 bulbs, split) Zhi Mu (Anemarrhenae Rhizoma, 3 liang)
Wash the two ingredients with spring water. Soak the Lily bulb overnight; white foam will appear on the surface. Discard this liquid. Add 2 sheng of good spring water, decoct to obtain 1 sheng of liquid, remove the dregs, and set aside. Separately, add 2 sheng of spring water to Anemarrhena, decoct to obtain 1 sheng of liquid, and remove the dregs. Mix the two liquids, decoct together to obtain 1.5 sheng, and take half warm, divided into two doses. (Identical to *Duanju Fang* and *Qianjin Fang*)
Furthermore, for cases relapsing after purgative therapy, Baihe Huashi Daizhe Tang (Lily, Talcum, and Hematite Decoction) is the primary formula.
Baihe (Lilii Bulbus, 7 bulbs, split, soaked in spring water overnight, discard the white turbid liquid) Huashi (Talcum, 3 liang, crushed) Daizhe (Hematitum, one pill-sized piece, crushed)
First, use 2 sheng of spring water to decoct the Lily bulb to obtain 1 sheng of liquid, remove the dregs, and set aside. Then, use 2 sheng of spring water to decoct the remaining two ingredients to obtain 1 sheng of liquid, remove the dregs. Combine all liquids and decoct to obtain 1.5 sheng, divide into two doses. (Identical to *Qianjin Fang* and *Duanju Fang*)
Furthermore, for cases relapsing after emetic therapy, Baihe Jizi Tang (Lily and Egg Yolk Decoction) is the primary formula.
Baihe (Lilii Bulbus, 7 bulbs)
Follow the previous method: use 2 sheng of spring water to decoct the Lily bulb to obtain 1 sheng of liquid, remove the dregs. Take one egg, extract the yolk, add it to the Lily decoction, stir well, and take half warm, divided into two doses. (Identical to *Qianjin Fang*)
Furthermore, for cases where neither emesis, purgation, nor sweating was used, and the condition remains unchanged, Baihe Shengdihuang Tang (Lily and Rehmannia Decoction) is the primary formula.
Baihe (Lilii Bulpus, 7 bulbs)
Follow the previous method: soak in 2 sheng of spring water, decoct to obtain 1 sheng of liquid. Add 1 sheng of fresh Rehmannia juice (Sheng Dihuang Zhi), mix the two liquids, decoct to obtain 1.5 sheng, and take half warm, divided into two doses. If the disease is cured after one dose, do not take more. Evil frothy substances may be excreted. (Identical to *Qianjin Fang* and *Duanju Fang*)
Furthermore, for Baihe Disease persisting for one month without resolution, transforming into thirst:
Use the water in which the Lily bulb was soaked to wash the patient's body. This is a method found in later editions of *Qianjin Fang*. Subsequently, consume Gualou Muli San (Trichosanthes Root and Oyster Shell Powder); this is the secondary method. (Excerpted from Volume 17)
*Duanju Fang* states: If Baihe Disease presents with Yin characteristics but is treated with Yang methods (such as sweating), the Yin pathogen cannot be resolved; repeating sweating is contraindicated, making the disease difficult to treat. If it presents with Yang characteristics but is treated with Yin methods (such as purgation), the Yang pathogen cannot be resolved; repeating purgation leads to non-resolution.
*Qianjin Fang* formula for Baihe Disease persisting for one month and transforming into thirst:
Baihe Gen (Radix Lilii, cut, 1 sheng)
Soak one dou of water with the Radix Lilii overnight. Use this water to wash the patient's body. After washing, eat plain wheat noodles (Note: Do not add salt or fermented soybeans). If thirst does not subside, use equal parts of Trichosanthes Root (Gualou Gen) and Oyster Shell (Muli) ground into powder. Take one fangcun bi (a square-inch measure) mixed with water, three times daily. (Identical to *Duanju Fang* and Zhang Zhongjing's formulas)
Furthermore, a formula for treating Baihe Disease complicated by fever:
Huashi (Talcum, 3 liang) Baihe Gen (Radix Lilii, 1 liang, roasted)
Grind the two ingredients into powder. Take one fangcun bi mixed with water, three times daily. Stop administration if mild diarrhea occurs; the heat will then dissipate. Another version states it treats Baihe Disease with red, scanty, and painful urination, and hardness/urgency below the umbilicus.
Furthermore, a formula for treating Baihe Disease complicated by abdominal fullness and pain:
Consume only Radix Lilii. Roast an appropriate amount until yellow, grind into powder. Take one fangcun bi mixed with water, three times daily. Discontinue when fullness subsides and pain stops. (Identical to *Duanju Fang*, excerpted from Volume 10)
