*Suwen* (Source of Diseases) states that the Qi of the five Zang organs is derived from the Lung. When the Lung is affected by slight cold, coughing ensues. Cold combats with Qi, causing Qi to gather and return to the Lung. As pathogens have periods of activity and rest, when pathogens move, Qi rushes upward in rebellion. When Qi rebels upward, it injures the five Zang organs. If it disturbs the Stomach Qi, Stomach Qi rebels, leading to vomiting. This is pulmonary cough, where the condition lingers, disturbing Qi and affecting the Stomach, resulting in vomiting. Furthermore, during the late summer when the Spleen is in command, if Spleen Qi is deficient and cannot fulfill its function, cold pathogenic factors may injure it, causing coughing; this is called Spleen Cough. Its manifestation includes pain in the right hypochondrium during coughing, dull pain radiating to the shoulder and back, severe cases being unable to move, as movement triggers coughing. The Spleen and Stomach are exteriorly-interiorly related. If Spleen Cough persists, the disease transmits to the Stomach. Its symptoms include coughing accompanied by vomiting; severe vomiting may result in the expulsion of roundworms. Generally, severe coughing leads to vomiting; one should determine the affected Zang or Fu organ based on the syndrome patterns. (From Volume XIV)
