In the *Shanghan Lun* (Treatise on Cold Damage Diseases) by Zhang Zhongjing, the description of Chest Bi syndrome (Chest Obstruction Syndrome): Chest Bi is characterized by a sensation of fullness in the chest, a feeling of obstruction in the throat accompanied by paroxysmal itching, dryness of the throat, and sticky, foamy saliva. The primary formula for treating this condition is Jupi Zhishi Tang (Tangerine Peel and Immature Bitter Orange Decoction).
Jupi (Citri Reticulatae Pericarpium, half jin) Zhishi (Aurantii Fructus Immaturus, four pieces, roasted) Shengjiang (Zingiberis Rhizoma Recens, half jin)
The above three herbs are cut into pieces. Boil with five sheng of water until two sheng remain. Divide into two doses for administration. (This formula is also recorded in *Zhouhou Beiji Fang* (Prescriptions Kept at the Ready Hand), *Duanju Fang*, *Wenzhong Shen Fang*, *Fan Wang Fang*, *Gujin Luyan Fang*, *Jingxin Lu*, and *Qianjin Fang*, appearing in Volume 15.)
Qitong Tang (Unblocking Qi Decoction) from *Qianjin Fang*: A formula for treating chest fullness, shortness of breath, and dysphagia/obstruction.
Banxia (Pinelliae Rhizoma, eight liang, washed) Shengjiang (Zingiberis Rhizoma Recens, six liang) Guixin (Cinnamomi Cortex, three liang) Wuzhuyu (Evodiae Fructus, forty pieces)
The above four herbs are cut into pieces. Boil with eight sheng of water until three sheng remain. Strain the dregs. Divide into three warm doses. Avoid mutton, maltose, and raw green onions. (Another version excludes Guixin and uses Jupi instead, appearing in Volume 13.)
In the *Shanghan Lun*, another description of Chest Bi syndrome: Chest Bi presents with wheezing, coughing up phlegm, pain in the chest and back, and shortness of breath. The pulse is described as sunken and rising at the Cun position, and small, tight, and rapid at the Guan position. The primary formula for treating this condition is Gualou Xiebai Baijiu Tang (Trichosanthes Fruit, Allium Macrostemon, and White Wine Decoction).
Gualoushi (Trichosanthis Fructus, one piece) Xiebai (Allii Macrostemonis Bulbus, cut, half sheng)
The above two herbs are boiled with seven sheng of white wine until two sheng remain. Strain the dregs. Divide into two warm doses for administration. (Recorded in *Shen Fang* and *Fan Wang Fang*, appearing in Volume 15.)
The *Zhouhou Beiji Fang* states: Chest Bi causes a hard, obstructive mass in the heart region, urgent pain, severe bi-sensation in the muscles, cramping pain like stabbing, making it impossible to bend forward or backward. Both the anterior chest and the back are painful, and the area cannot be touched. There is chest fullness, shortness of breath, coughing up phlegm that triggers pain, restlessness, and spontaneous sweating. The pain may radiate through to the back and spine. If not treated immediately, it can be fatal within a few days. Formula for sudden onset of Chest Bi:
Xionghuang (Realgar) Badou (Crotonis Fructus, skin and heart removed, roasted)
The above two herbs: First, grind Xionghuang finely and sieve it. Add Badou. Thoroughly pound them together until well mixed. Form pills the size of small beans. Take one pill per dose. If no effect is felt, slightly increase the dosage. Avoid wild boar meat and Asparagus cochinchinensis shoots.
Another formula:
Zhishi (Aurantii Fructus Immaturus, roasted)
The above single herb is pounded and sieved. Administer one fangcun bi (a measure approx. 2-3 cm square) with rice juice before meals. Three times daily, once at night.
Another method for cases where the disease has subsided but recurs:
Take Xiegen (Allii Macrostemonis Radix, five jin)
The above single herb is pounded and squeezed to extract the juice. Drink it; recovery will be immediate. (All extracted from Volume 1.)
