Yin Xun (Fungus-like Vulvar Hypertrophy): The vulvar margins are hypertrophic, swollen, and painful, with a central protrusion resembling a mushroom or cockscomb. It is caused by Liver Qi stagnation and Spleen Qi deficiency. Yi Bu Zhong Yi Qi Tang (Supplement the Middle and Augment the Qi Decoction) and Gui Pi Tang (Restore the Spleen Decoction) should be used, with increased doses of Qing Pi (Tangerine Peel) and Zhi Zi (Gardenia Fruit).
Yin Zhong (Vulvar Swelling): Sudden swelling and pain of the vulva is called Bang Ju (Clam Carbuncle). It is caused by labor injury affecting the Blood aspect. Yi Si Wu Tang (Four Substances Decoction) added with Dan Pi (Moutan Cortex), Ze Xie (Alisma Rhizome), Hua Fen (Trichosanthes Root), and Chai Hu (Bupleurum Root) should be used. Alternatively, Qin Jiao Tang (Gentiana Macrophylla Decoction) may be employed.
Yin Shi (Erosive Genital Condition): Papules appear on the exterior of the genitalia, while small worms are generated internally, causing unbearable itching. Also known as Te Chuang (Hidden Sore). It is caused by Stomach Qi deficiency and accumulated stagnation. Yi Si Wu Tang (Four Substances Decoction) added with Shi Chang Pu (Acorus Rhizome), Long Dan Cao (Gentian Root), Huang Lian (Coptis Rhizome), and Mu Tong (Akebia Caulis) should be used. If chills and fever resemble those of Xu Lao (Consumptive Disease), it indicates that the pathogenic worms have entered the Zang organs. Yi Xiao Yao San (Free and Easy Wanderer Powder) taken with Lu Hui Wan (Aloe Pill) morning and evening is indicated. External treatment includes fumigation and washing, followed by inserting Yin Xing San (Ginkgo Powder) into the vagina to kill parasites and relieve itching. If this condition occurs in widowed women who endure it in silence, waiting until stabbing pain, dry mouth, fever, emaciation, and anorexia develop, it becomes a Shi He Zheng (Disease of Sexual Deprivation). This condition is incurable.
Yin Tuo (Vulvar Prolapse/Relaxation): The vulva remains open and cannot close. Accompanied by itching, pain, and exudation. It is caused by excessive worry and overthinking. Yi Xiao Yao San (Free and Easy Wanderer Powder) and Gui Pi Tang (Restore the Spleen Decoction) should be used, both added with Chai Hu (Bupleurum Root), Zhi Zi (Gardenia Fruit), Bai Shao (White Peony Root), and Dan Pi (Moutan Cortex). If contracted postpartum, Yi Bu Zhong Yi Qi Tang (Supplement the Middle and Augment the Qi Decoction) added with Wu Wei Zi (Schisandra Berry) and Cu Chao Bai Shao (Vinegar-fried White Peony Root) is indicated. In all cases, large doses of Jing Jie (Schizonepeta Spike), Zhi Ke (Bitter Orange), He Zi (Chebula Fruit), and Wen Ge (Clam Shell) decocted for fumigation and washing should be used.
Yin Ting (Uterine Prolapse): The uterus prolapses outside the body. Commonly known as "Hu Lu" (Gourd). It is caused by Qi and Blood deficiency. Yi Bu Zhong Yi Qi Tang (Supplement the Middle and Augment the Qi Decoction) should be used, with Chai Hu (Bupleurum Root) removed, Sheng Ma (Cimicifuga Rhizome) doubled, and Yi Mu Cao (Motherwort Herb) added. Externally, Bi Ma Zi Rou (Castor Bean Flesh) should be applied to the Bai Hui acupoint (GV20) on the vertex. Then, half a jin of Zhi Ke (Bitter Orange) decocted for fumigation and washing should be used. If caused by strenuous effort and heavy lifting, moxibustion at four cun five fen below the umbilicus, seven moxa cones, yields immediate results. If caused by unfulfilled sexual desire leading to Liver Qi stagnation, there is typically initial obstruction during urination, followed by prolonged straining, which causes the descent. A midwife should manually reduce the prolapsed uterus. The patient should lie supine with a pillow under her waist. Snuff powder should be blown into the nose to induce sneezing, which helps retract the organ. Once retracted, the vulva closes tightly, and the legs should be bound together with cloth. Urination must also be controlled. Internal administration of strong tonifying and ascending herbs can provide temporary relief. For widowed women, resuming sexual activity is necessary to eliminate the root cause.
