When an ulcer first breaks open, or after it has been incised with a needle, fearing that the puncture wound might close easily, apply Xuanzhu Ointment (Mysterious Pearl Ointment) to a cotton paper wick and insert it. Alternatively, melt yellow beeswax, add a little sesame oil, and form it into a wick. If it is a Ding sore (a type of severe carbuncle), wrap a thread around the center of the wick, insert the wax wick into the puncture wound, leave the thread end outside, and cover it with ointment. Change and reinsert as before the next day.
For wounds where necrotic flesh does not discharge, make a wick from cotton paper. Use the frayed tip of the wick to twist and wring out the necrotic tissue on the surface, capturing the dead flesh. Slowly pull it out. If not completely removed, repeat the process. Then apply Xuanzhu Ointment. Once the necrotic tissue is clean, apply Jiangzhu Ointment (Crimson Pearl Ointment) to the swollen area surrounding the wound, and cover it with Ganzhu Ointment (Azure Pearl Ointment). For stagnant pus and swelling that do not discharge, crush garlic and mix it with pine rosin powder to apply as a poultice.
If the ulcer is already ulcerated with a black Ding inside, protruding like a sinew head and causing unbearable hard pain, use Zijiang Zhenxiang (Purple Crimson Real Fragrance), grind it into bean-sized pieces, stir-fry until charred black, and powder it. Sprinkle this powder onto the Ding. Apply Lanxue San (Orchid Snow Powder) to the surrounding ulcerated areas. Cover with Hei Gao (Black Plaster). After two or three applications, the Ding will be completely removed; then switch to Shengji Medicines (Flesh-Generating Medicines).
For stubborn necrotic tissue that does not fall off: in robust patients, use powder of raw Arisaema Rhizoma (Sheng Nanxing) mixed with Borneol (Bingpian), sprinkle it on, cover with plaster overnight. The necrotic tissue will fall off like a well being cleaned. Then switch to Shengji medicines. In deficient patients, sprinkle Ginseng Radix et Rhizoma (Renshen) powder, wash it off overnight. After two or three times, the necrotic flesh will detach naturally and healing will be easy.
There are four causes for a wound opening resembling a snake's head: exposure to Wind, Water, Anger, and squeezing by hand. All can cause this. If caused by Wind Evil invasion, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with Fangfeng (Saposhnikovia Root) and Tianma (Gastrodia Rhizome). If caused by Cold-Wind stagnation, use Shixuan San (Ten Acupoints Powder) with Qianghuo (Notopterygium Root) and Tianma. If caused by Liver Fire and Blood Dryness generating Wind, use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) with Qianghuo and Tianma. For children, if the mother has Liver Fire generating Wind, use Jiawei Xiao Chaihu Tang (Modified Minor Bupleurum Decoction), followed by Jiawei Xiaoyao San with Loulu (Euphorbia Root) and Tianma. Additionally, use external warming methods. Or use Wumei (Mume Fruit) preserved with its original nature, mixed with a little Qingfen (Calomel), sprinkle it on, and it will heal immediately.
For sores resembling pomegranate flowers, use a silver needle to prick the red part to drain dark blood. After three to five times, it will flatten and heal.
For stubborn sores, Ding sores, unspecified sores, and beating injuries that fail to generate flesh for a long time, there is likely a membrane over the sore. Crush the root of Tricyrtis Rhizoma (Yuzanhua Gen) in a stone mortar and apply it. Bind tightly. Change every three days. It heals after five to seven applications.
If a wound fails to close due to deficiency, it requires strong tonification. Externally, use equal parts of Ginseng Radix et Rhizoma (Renshen) and Pearl (Zhenzhu) powdered and sprinkled on, which promotes scabbing. All Shengji medicines must be soaked in Ginseng decoction, dried, then soaked in human milk again. This yields miraculous speed.
Although the wound has closed, if the skin is tender and sensitive to wind and cold externally, cook millet rice, place it hot in a bowl, cover with silk fabric, and apply over the affected area. Steam it three to five times, and it will heal.
If toxic water enters the ulcer during breakdown, or if a splinter penetrates to the bone causing swelling and pain, use seven seeds of Gleditsia Sinensis (Zaojiaozi), one large dried toad, and fifteen grains of Piper Longum (Hujiao). Place them in a pot, cover with a tile, and heat gently until smoke ceases. Grind into powder. First, wash with warm water, dry, and sprinkle the powder. Cover with plaster for a long time. The toxic water will drain out, and any splinters will become visible. Alternatively, use equal parts of Lithargyrum (Huandan) and Tridacna Shell (Ge Fen), stir-fried until color changes, and sprinkle. For pain caused by entering any toxic water, soap pod water, or donkey/horse urine and feces, use three taels of Glutinous Rice Flour (Nuomifen), three taels of Rock Sugar (Shatang), and one fen of Raw Licorice Root (Sheng Gancao), made into a plaster and applied. The toxic water will drain out.
If horse sweat enters a sore causing swelling and pain, take the handle part of a horse whip, burn it to ash while preserving its nature, powder it, and mix with various plasters to apply. Or mix with lard. Or dry winter melon peel in the shade, powder it, and apply.
If a chronic sore smells foul, rots, and breeds maggots, powder Sea Cucumber (Haishen) and sprinkle it, or powder Alum (Zaofan) and sprinkle it. The maggots will turn into water.
Method for bandaging back ulcers: When the ulcer has broken and is discharging pus, regardless of season, place four to five layers of silk gauze over the plaster. Outside this, use a square piece of cotton cloth eight inches wide. Attach thin snake-skin straps to the four corners. Cover the silk with the cloth. Tie the straps across the chest to tighten the bandage firmly over the ulcer. This prevents warmth from escaping and facilitates suppuration and decay. Prepare two sets of bandages for replacement when washing.
Method for internal flesh non-adhesion: When necrotic tissue is gone and a purulent crust forms, but the internal flesh does not adhere, place seven to eight layers of soft silk gauze on the affected area. Bind tightly with silk. Have the patient lie still. After several times, the flesh will naturally adhere as if formed by nature. If the wound mouth is not yet complete, apply Yuhong Gao (Jade Red Ointment) and it will level itself.
