The meridian pathways are the same as those of the back.
Faxia Bei (Lower Back Carbuncle) arises in the center of the spine, corresponding anteriorly to the umbilicus. Also known as Duiqi Fa (Umbilical Opposite Carbuncle). This condition is extremely perilous. Since the Kidney (Zang organ responsible for storing essence and governing water metabolism) is the root of life, treatment should prioritize protecting the Kidney.
Shenshu Fa (Kidney-Shu Carbuncle) arises on both sides of the waist, between the deep muscles. In cases caused by alcohol consumption, sexual excess, and damp-heat, it often occurs unilaterally; in cases caused by sexual exhaustion and anger/fire, it occurs bilaterally.
If these two conditions have sufficient True Yin to control fire, the sore will be red and vibrant, swelling high and suppurating. Use Renshen Yangrong Tang (Ginseng Decoction for Nourishing Ying and Wei), adding Shanzhuyu (Cornus Officinalis Fruit), Wuweizi (Schisandra Chinensis Fruit), Zhimu (Anemarrhena Asphodeloides Rhizome), and Huangbai (Phellodendron Amurense Bark). Alternatively, add Rushen (Frankincense), Moyao (Myrrh), and Ejiao (Donkey-hide Gelatin). Also use Jiajian Bawei Wan (Modified Eight-Ingredient Pill) to restore the source. If the sore is purple-black, dry, hard, and does not suppurate, or if the pus is thin and damages the fascia, it indicates internal failure of True Yin, with death occurring around fifteen days later. This is not of the same category as Xiadashou. Moxibustion at Heyang Acupoint (BL58) with five moxa cones.
Xiadashou (Lower Reaching Hand) arises beside the Shenshu Acupoint (BL23), between the fourteenth vertebral spinous process. Also known as Lianshen Fa (Kidney-Connecting Carbuncle). It is caused by kidney damage due to sexual excess. It leads to dry mouth, alternating chills and fever, and pain in all joints. The treatment is the same as for Faxia Bei. If coughing, vomiting, hiccups, cold extremities, and inability to eat occur, along with thin pus and a sensation that the lumbar bones are broken, it is a adverse sign (Ni). Pre-existing thirst followed by carbuncle indicates exhaustion of Kidney Water. Pus that is foul, dark in color, and painless, or a sore mouth that is wide open, indicates failure of Spleen Qi. Urination resembling strangury, phlegm obstruction, panting, dry mouth, and cracked tongue indicate failure of Spleen and Lung. A Hong-shu pulse (rapid and forceful) at the left Chi position indicates the Kidney has no generation source. A Fu-se pulse (floating and choppy) at the left Guan position indicates Metal overcoming Wood. Difficult to treat in autumn. All are incurable.
Shiliuzhu (Dampness Flowing Deposit) also arises between the waist and Kidneys. However, it swells without a head, does not change color, and is painless. It does not ulcerate for three to five months, causing loss of appetite and emaciation. Treatment should tonify Qi and Blood, and resolve stagnation. For example, Shiquan Dabu Tang (Ten Substance Great Tonifying Decoction) with Xiangfu (Cyperus Rotundus Rhizome) and Chenpi (Citrus Reticulata Pericarpium), taken for dozens of doses. Wait until Yuan Qi (Primal Qi) recovers slightly before needling. Still double the dosage of Renshen (Panax Ginseng Root) and Huangqi (Astragalus Membranaceus Root). Add Yuanzhi (Polygala Tenuifolia Root), Beimu (Fritillaria Cirrhosa Bulb), and Bailian (Paeonia Anomala Root). Externally, apply Douchi Bing (Fermented Soybean Cake) and Fuzi Bing (Aconite Carpinus Cake) for moxibustion. Avoid using Shixuan San (Ten Acupoints Powder) and Liqi Yin (Moving Qi Decoction).
Shiju (Stone Abscess) arises between the waist and hips. It swells without a head, skin color remains unchanged, and it is hard as stone. It is caused by accumulated heat in the Shaoyin and Yangming meridians. Treatment is the same as Shiliuzhu. It is also due to deficiency of Yuan Qi leading to solidification of pathogenic factors. If it becomes black, sunken, numb, accompanied by vomiting, inability to eat, coma, and a scattered or intermittent pulse, death ensues.
Whenever evil blood stagnates, slow treatment results in Liuzhu (Flowing Deposit) or Guju (Bone Abscess).
A woman had swelling in her waist, worse in both legs. This was stasis of blood in the meridians. Two doses of Taoren Tang (Peach Kernel Decoction) brought slight improvement. She recovered completely after taking Moyao Wan (Myrrh Pill). In another case where evil blood was not fully cleared, there was stabbing pain in the umbilical and abdominal region. If it flows into the limbs or causes swelling and pain in the inner thighs, Moyao Wan cures the disease. In another case caused by contact with water-dampness, where meridians do not flow leading to swelling and pain, Danggui Wan (Angelica Sinensis Pill) is appropriate.
Chanyao Huodan (Fire Belt Snake) is also known as Huodai Chuang (Fire Belt Sore). Colloquially called Shechuan Chuang (Snake String Sore). Initially arising on the waist, it appears purple-red like a rash, or forms blisters. Pain feels like burning fire. It is caused by non-interaction between Heart and Kidney, intense internal Liver Fire flowing into the Bladder and wrapping around the Dai Mai (Girdle Vessel). Internal administration of Shuhuanglian Tang (Bupleurum and Coptis Clearing Decoction) is suitable. For robust constitutions, Guijin Wan (Precious Gold Pill) can be used to purge. Externally apply Qingliang Gao (Cooling Ointment) for self-healing. If neglected, it spreads all over the waist, poison enters from the umbilicus, causing distension and anorexia, which is incurable.
Qingliang Gao (Cooling Ointment): Take one piece of calcined gypsum (Duan Shi), dissolve in cool water, add water and stir turbidly. Let settle briefly, take one cup of clear water, mix with half a cup of sesame oil, stir hundreds of times to form an ointment. Add a small amount of powdered Xionghuang (Realgar), stir evenly. Apply to the affected area with a goose feather brush for immediate effect.
A seventy-year-old woman had a carbuncle on her waist, itching excessively. Qi Shi said: A floating and rapid pulse with aversion to cold indicates a sore. If the pulse becomes weak before the sore ulcerates, how can healing occur? Treatment was ineffective, and she indeed died.
