Meridians: The center of the neck belongs to the Du Meridian (Governing Vessel). The large tendons on both sides of the neck belong to the Foot-Taiyang Bladder Meridian. Beside the large tendons, behind the ears, belongs to the Hand-Shaoyang Sanjiao Meridian. Below the ears belongs to the Foot-Shaoyang Gallbladder Meridian. Below the cheeks belongs to the Hand-Taiyang Small Intestine Meridian. One cun in front of the cheeks belongs to the Hand-Yangming Large Intestine Meridian. The pulsating arteries on both sides of the throat belong to the Taiyang and Yangming meridians.
Neck Carbuncle (Xiang Ju): Occurs in the center of the neck. Also known as Xiangzhong Ju. Commonly called "Duikou" (Opposite Mouth), because it is opposite the mouth. This condition belongs to the excessive Yang fire of the Du Meridian. Those with redness, swelling, and pain are easy to treat. Those with a large and hard root are difficult to heal. Those that are flat and sunken with coma are fatal. At the initial stage, if the pulse is surging, rapid, and forceful, Gan Zhu Dan, Weisheng San, Guijin Wan, Meihua Wuqi Dan (Plum Blossom Five Qi Powder), or Huanglian Jiuku Tang can be selected. Combined with garlic-separated moxibustion. Or moxa-convulsion at Tianzong (SI 11) acupoint seven times, with moxa cones the size of green beans. For those already formed, Jiedu Tianjiang San (Detoxifying Heavenly Pulp Powder) and Tuoli Xiaodu San can be used. For those already ulcerated, Da Xiao Bao'an Tang (Great and Small Security Decoction) is used to tonify Yuan Qi (Original Qi), and Jiadi Dihuang Wan (Modified Rehmannia Pill) guides fire back to its source. External treatment includes applying medicaments. Or moxa-convulsion at Shenque (CV 8) acupoint fourteen times. This disease has a head and tail; the head points upward, the tail downward, shaped like a honeycomb. Prevent toxic vapors from attacking the heart and eroding the brain. Avoid exposure to wind; convulsions caused by wind invasion are difficult to treat. If phlegm stagnation and toxin flow to both shoulders, sudden profuse bleeding, abdominal distension, and delirium occur, these are untreatable.
Silkworm Carbuncle (Wocan Ju): Occurs below Duikou Ju and above Tianzhu Bone. Treatment is the same as Brain Carbuncle.
Tianzhu Carbuncle (Tianzhu Ju): Occurs between Tianzhu bones. The shape is sunken, color is black, and there is much itching. Caused by depression and anger in the Sanjiao Meridian. Initially, use Weisheng San. For strong constitutions, use Guijin Wan. For weak constitutions, use Shenxiao Gualou San (Miraculous Trichosanthes Powder). For those already formed, use Bazhen Tang (Eight Treasure Decoction) added with Chai Hu (Bupleurum Root), Xiang Fu (Cyperus Rhizome), and Yuanzhi wine. Severe cases present with stiffness and tension in the shoulder and back, vomiting, and nausea. Use garlic-separated moxibustion, stopping when itching ceases. Or direct moxibustion. Blisters after moxibustion indicate a favorable prognosis; no blisters indicate an unfavorable one. Taking diaphoretic agents to induce sweating is treatable; no sweat is difficult to cure. The *Ling Shu* states: Failure to treat allows heat qi to descend into Yuanye, damaging the Ren Meridian anteriorly, and internally steaming the Liver and Lungs, leading to death within ten days. Therefore, coma, retching, and uncontrolled bleeding are untreatable.
Lateral Brain Carbuncle (Pian Nao Ju): Occurs beside Duikou. Caused by accumulated heat in the Foot-Taiyang Bladder Meridian. Since the Taiyang Meridian governs cold water, originating from the vertex, passing through both sides of the neck and descending smoothly, it is often cold and sinking, hence often flat, hard, difficult to suppurate, and prone to metastasis. In the initial stage, before suppuration and within seven days, use a Bi Needle (Surgical Knife) to puncture directly at the head. Stop when pain is felt. Drain the bad blood, open the sore aperture, insert Chansu strips (Toad Venom Strips) down to the bottom of the sore, two strips daily. Apply external plaster. After three days, add insertion medicines. Internally take Gan Zhu Dan, or Chansu Wan to induce sweating and detoxify. Subsequently take interior-supporting and great-tonifying medicines to restore Yang Qi. Continue until the top and root are high, protruding, swollen, and painful. Use Divine Lamp Illumination method externally to assist Yin transforming into Yang. By the seventh day, cracks appear around the sore with pus, then stop inserting medicines. Mild cases shed necrotic tissue or suppurate after nine days. Severe cases shed roots spontaneously by the thirteenth day. If the root remains firmly attached beyond the expected time, remove it with a Bi Needle. Apply Yuhong Gao (Jade Red Ointment) externally and take great-tonifying medicines internally to support expulsion. This is indeed an excellent method for turning heavy conditions light and moving deep conditions shallow. If there is no swelling or pain, and even moxibustion causes no pain, this method is especially suitable. However, if the anterior brain carbuncle presents as a Yang syndrome, this method is not used.
Fish Tail Toxin (Yuwéi Du): Occurs between the posterior hairline, above Piankou, and beside the posterior head hair. Treatment is the same as Piankou. Hemorrhage, asthma, and phlegm emergence make it difficult to treat.
Hundred Vessels Carbuncle (Baimai Ju): Swelling surrounds the neck and nape, with pain, fever, anorexia, rebellious Qi, and cough. The onset pulls towards the ear, causing swelling. It can be pricked after fifteen days. Pus discharge indicates a favorable prognosis; blood indicates an unfavorable one.
Lock-Throat Toxin (Suohou Du): Sudden swelling and pain of the neck and nape, resembling stiff neck. Later, redness spreads around the shoulders and back, becoming hard and difficult to resolve. Caused by obese individuals who drink excessively, combined with external invasion of Wind-Heat. Treatment should clear summerheat and disperse Wind.
Throat Knot Carbuncle (Jie Hou Yong): Occurs in the exact center outside the throat. Also named Meng Ju (Fierce Carbuncle). Belongs to the Ren Meridian (Conception Vessel) and the Hand-Taiyang and Hand-Shaoyin meridians. Caused by sorrow and indignation. Use Huanglian Xiaodu Yin (Coptis Detoxifying Decoction) and Weisheng San. The *Ling Shu* states: If pus is not drained, the throat will be blocked and death will occur in half a day. Therefore, needle drainage of toxin is necessary, followed by taking tonic and supporting agents. Even if ulcerated and patent, flowing out serous fluid leads to recovery. Moxa-convulsion at Shaohai (HT 3) acupoint seven times.
Qi Abscess (Qi Yong): Occurs above and below Jie Hou Yong.
Clamping Throat Abscess (Jia Hou Yong): Occurs on both sides of Jie Hou Yong.
The treatment for these two conditions is the same as for Jie Hou Yong. Moxa-convulsion at Lingdao (HT 4) acupoint seven times.
