Upper Dasha (Upper Back Carbuncle) arises at the Feishu (BL13) acupoint. Although the names for left and right are the same, their meridians differ: the right belongs to the Lung Meridian, while the left belongs to the Liver Meridian. This condition is generally caused by Qi stagnation and phlegm-heat accumulation.
[Note] This lesion occurs at the Feishu (BL13) acupoint of the Foot-Taiyang Bladder Meridian, located in the movable area between the two scapulae. Whether it is Left Dasha or Right Dasha, although the name is identical, lesions on the left belong to the Liver, and those on the right belong to the Lung; hence it is said that "the meridians differ." It is invariably formed by the coagulation of Qi stagnation and phlegm-heat. Initially, Shenshou Weisheng Tang should be used for simultaneous release of exterior and interior pathogens. Subsequently, Xiaoyao San (Free and Easy Wanderer Powder) should be used for clearing, accompanied by Liuyu Tang (Six Stagnations Decoction) for regulation. All other internal and external treatment methods follow the chapters on Carbuncles and Ulcers (Swelling/Ulceration).
Xiaoyao San (Free and Easy Wanderer Powder)
Danggui (Radix Angelicae Sinensis, wine-washed), Baishao (Radix Paeoniae Alba, wine-washed), Baifuling (Poria, dried), Baizhu (Rhizoma Atractylodis Macrocephalae, earth-fried), Xiangfu (Rhizoma Cyperi Rotundi, wine-fried, each 1 qian), Chaihu (Radix Bupleuri, 8 fen), Huangqin (Scutellariae Radix, 5 fen), Chenpi (Pericarpium Citri Reticulatae, 1 qian), Bohe (Menthae Haplocalycis Herba, 5 fen), Gancao (Glycyrrhizae Radix et Rhizoma, raw, 6 fen).
Add two cups of water, decoct until eight-tenths remain, and take after meals.
[Formula Song] Xiaoyao San harmonizes Qi and Blood, opens depression, moves stagnation, and dissipates nodules. It contains Danggui, Baishao, Fuling, Baizhu, Xiangfu, Chaihu, Huangqin, Chenpi, Bohe, and Gancao to clear toxic heat.
Liuyu Tang (Six Stagnations Decoction)
Xiangfu (Rhizoma Cyperi Rotundi, wine-fried), Fuling (Poria, dried), Chenpi (Pericarpium Citri Reticulatae), Banxia (Rhizoma Pinelliae Ternatae, processed), Chuanxiong (Rhizoma Chuanxiong), Shanzhi (Gardeniae Fructus, each 1 qian), Cangzhu (Rhizoma Atractylodis Lanceae, fried), Suasharen (Fructus Amomi Rotundus, raw), Gancao (Glycyrrhizae Radix et Rhizoma, raw, each 5 fen).
Add three slices of Shengjiang (Zingiberis Rhizoma Recens) and two cups of water, decoct until eight-tenths remain, and take.
[Formula Song] Liuyu Tang opens the six types of stagnation, eliminating phlegm and moving Qi. It contains Chuanxiong, Suasharen, Erchen components (Chenpi, Banxia), Cangzhu, Shanzhi, Xiangfu, and Shengjiang to resolve stagnation.
Shenshou Weisheng Tang (See Chapter on Swelling/Ulceration)
Middle Dasha (Middle Back Carbuncle) arises near the Gaohuang (BL43) acupoint. The meridian involved is the Bladder Meridian along the side of the spine. It is caused by disharmony of the Seven Emotions, where anger leads to fire accumulation. One must carefully differentiate between deficiency/excess and cold/heat patterns.
[Note] This lesion occurs on both sides of the spinal column, belonging to the Gaohuang (BL43) acupoint of the Foot-Taiyang Bladder Meridian, also known as Long Ju (Dragon Carbuncle). It arises from disharmony of the Seven Emotions, specifically the coagulation of fire due to anger. If the pattern involves excess cold with dry stool and no thirst, Yili Jindan (One Pill Golden Elixir) should be used for warm purgation. If the pattern involves excess heat with dry stool and extreme thirst, Neishu Huanglian Tang (Internal Draining Coptis Decoction) should be used for cold purgation. If there is Qi and Blood deficiency causing the ulcer to fail to erupt or grow, Neituo Huangqi San (Internal Supportive Astragalus Powder) should be used for supportive tonification and toxin expulsion. All other internal and external treatment methods follow the chapters on Carbuncles and Ulcers (Swelling/Ulceration).
Yili Jindan (One Pill Golden Elixir)
Muxiang (Aucklandiae Radix), Rushxiang (Olibanum, each 5 fen), Badoushuang (Seminar Crotonis Tiglii, prepared frost, 1.5 qian), Chenxiang (Aquilariae Lignum Resinatum, 5 fen).
Grind all into fine powder and mix evenly. Take half a large, fleshy Jujube (Ziziphi Spinosae Fructus), remove skin and pit, mash it, and mix with the powder to form pills the size of an Euryale Seed (Qianshi). Take one pill per dose, chew thoroughly, and swallow with one mouthful of plain boiled water. Shortly after, drink another mouthful of plain boiled water to induce one bowel movement. If two mouthfuls are drunk, two bowel movements occur. For patients with strong Stomach Qi and severe toxin stagnation, drinking three to four mouthfuls of boiled water after medication may induce three to four bowel movements; do not overdo this. Once toxin stagnation is fully expelled, immediately replenish with Rice Water (Mi Yin) to support the Stomach Qi.
[Formula Song] Yili Jindan treats malignant sores, suitable for cold-excess patterns without thirst and dry stool. It contains Muxiang, Rushxiang, Chenxiang, and Badou meat, formed into pills with Jujube paste for recovery.
Neituo Huangqi San (Internal Supportive Astragalus Powder)
Danggui (Radix Angelicae Sinensis), Baishao (Radix Paeoniae Alba, fried), Chuanxiong (Rhizoma Chuanxiong), Baizhu (Rhizoma Atractylodis Macrocephalae, earth-fried), Chenpi (Pericarpium Citri Reticulatae), Chuanshanjia (Squama Manis, fried and ground), Zaocispina (Spina Gleditsiae Sinensis), Huangqi (Radix Astragali Membranacei, each 1 qian), Binglang (Arecae Semen, 3 fen), Rougui (Cinnamomi Cortex, purple, 5 fen).
Add two cups of water, decoct until eight-tenths remain, and take before meals.
[Formula Song] Neituo Huangqi San treats ulcer deficiencies; it is most suitable for internal support therapies for various ulcers. It contains Danggui, Baishao, Chuanxiong, Baizhu, Chenpi, Rougui, Chuanshanjia, Binglang, and Zaocispina.
Neishu Huanglian Tang (Internal Draining Coptis Decoction) (See Chapter on Swelling/Ulceration)
