Niukou Feng (Button-shaped Wind) arises between the chest and neck, caused by the accumulation of wind-dampness, resulting in unbearable itching. The condition spreads into patches and exudes fluid; it is named according to its location and should be treated as a tinea disease (fungal skin infection).
[Note] This condition occurs between the Tianchu point (CV22) below the neck. After sweating, pathogenic wind invades the space between the skin and muscle, presenting as millet-grain-sized papules with extreme itching. Scratching breaks the skin, causing serous discharge. If mistakenly washed with water, it spreads into extensive patches. For mild cases, apply Dusheng San (Supreme Victory Powder) or Bingsu San (Ice and Sulfur Powder) externally; for severe cases, administer Xiaofeng San (Wind-Expelling Powder) internally for recovery.
Dusheng San (Supreme Victory Powder)
Grind mustard flower (Sinapis alba) into fine powder, mix with vinegar, and apply to the affected area.
[Prescription Verse] Dusheng San treats Niukou Feng, effective whether broken or not; containing only mustard flower, it stops itching and reduces swelling with miraculous efficacy.
Bingsu San (Ice and Sulfur Powder)
Lithii Sulfas (Sulfur, 1 liang), Camphora (Chao Nao, 2 qian), Zanthoxyli Fructus (Chuan Jiao, 2 qian), Alumen Crudum (Sheng Bai Fan, 2 qian).
Grind all ingredients into fine powder. Take one white radish (Raphanus sativus), hollow it out, fill it with the medicine, cover with the radish peel, wrap in three to four layers of paper, and roast in ash fire for about half an hour. After cooling, remove the medicine and mix with rendered lard (Porci Adeps) to a thick consistency. Apply to the affected area for self-healing.
[Prescription Verse] In Bingsu San, Lithium Sulfate (Sulfur) is primary; Camphor, Zanthoxylum, and Alum are excellent. Fill the hollowed radish with medicine, mix with oil, and apply specifically to Niukou sores.
Xiaofeng San (Wind-Expelling Powder) Treats Niukou Feng, characterized by extreme itching, serous discharge upon scratching, and sometimes serosanguineous discharge.
Schizonepetae Herba (Jingjie), Saposhnikoviae Radix (Fangfeng), Angelicae Sinensis Radix (Danggui), Rehmanniae Radix (Shengdi), Sophorae Flavescentis Radix (Kushen), Atractylodis Rhizoma (Cangzhu, stir-fried), Cicadae Periostracum (Chantui), Sesami Semen (Huma Ren), Arctii Fructus (Niubangzi, stir-fried and ground), Anemarrhenae Rhizoma (Zhimu, raw), Gypsum Fibrosum (Shigao, each 1 qian), Glycyrrhizae Radix et Rhizoma (Gancao, raw), Akebiae Caulis (Mutong, each 5 fen).
Decoct two cups of water until reduced to eight-tenths. Take after meals.
[Prescription Verse] Xiaofeng San stops itching and dispels wind-dampness; Mutong, Cangzhu, Kushen, and Zhimu assist. Jingjie, Fangfeng, Danggui, Niubang, Chantian, Shigao, and Gancao are decocted with Huma and Shengdi in water.
San Fa (Three Types of Back Carbuncles: Upper, Middle, Lower) involve Fire-Toxin affecting the Du Meridian (Governor Vessel). The Middle Fa belongs to the Liver and corresponds to the Heart; the Upper Fa belongs to the Lung and arises below the Tianzhu bone (GB10); the Lower Fa belongs to the Kidney and arises behind the umbilicus.
[Note] The Upper, Middle, and Lower Back Carbuncles all belong to the Du Meridian and are caused by Fire-Toxin. Upper Back Carbuncle: Fire-Toxin injures the Lung, arising below the Tianzhu bone. Also known as Pidufa (Spleel-Belly Carbuncle), it is wide and flat like a belly. Middle Back Carbuncle: Fire-Toxin injures the Liver, arising at the center of the back. Also known as Duixinfa (Opposite-Heart Carbuncle), it is wide in the middle and pointed at both ends, like a melon. Lower Back Carbuncle: Fire-Toxin injures the Kidney, arising in the waist. Also known as Duiqifa (Opposite-Umbilicus Carbuncle), it is flat and spreading like a turtle. Initially, they appear like millet grains, accompanied by pain, numbness, itching, general stiffness, and alternating chills and fever. After several days, sudden massive swelling occurs. Those with excess Qi experience more pain; those with deficient Qi experience more numbness and itching. Initial treatment, regardless of deficiency or excess, should involve moxibustion with garlic slices. If ineffective, direct moxibustion on the peak of the lesion should be performed until pain is felt, indicating efficacy. This shrinks large lesions and moves deep pathology to the surface. After moxibustion, puncture the center of the carbuncle with a needle, then apply suction methods to ensure thorough drainage of toxins, preventing internal invasion. If exterior symptoms exist (fever, aversion to cold, absence of sweat), administer Jingfang Baidu San (Schizonepeta and Saposhnikovia Decoction to Overcome Toxicity) to induce sweating. If interior symptoms exist (fever, aversion to heat, dry stool), administer Neishu Huanglian Tang (Internal Draining Coptis Decoction) to purge. If both exterior and interior symptoms coexist, administer Shenshou Weisheng Tang (Divine Grant Health-Preserving Decoction) to resolve both, thereby reducing the severity of the sore. As pus forms, employ Tonifying Interior methods. If the ulcer opens but necrotic tissue remains, apply Bagao (Ba Plaster) externally to dissolve it. Other treatments follow the protocols for Yang Sores (Yong Ju) in the Swelling/Ulceration sections. These three conditions, regardless of age, are considered favorable (Shun) if high, swollen, red, vibrant, and painful; they are unfavorable (Ni) if flat, sunken, scorched, dry, purple, or black.
