舒肝溃坚汤(见瘰 门)
Shiju (Stone Abscess) arises on both sides of the neck and nape. The mass is hard as stone, while the skin color remains normal. This condition results from Liver Qi stagnation causing Qi and Blood to coagulate in the collaterals; after ulceration, the treatment principles follow those for Luoli (Scrofula).
[Note] This abscess occurs on both sides of the neck, resembling a plum or peach in shape, with normal skin color, hard as stone, painful but not hot. It is caused by stagnation in the Liver Meridian, leading to the stasis of Qi and Blood within the collaterals. Initially small, it gradually enlarges, is difficult to dissipate, difficult to suppurate, and even after ulceration, difficult to heal; it is a stubborn and chronic condition. In the early stage, if the patient's constitution is robust, Shugan Kuijian Tang (Liver-Soothing Hardness-Dispersing Decoction) is indicated; if the constitution is deficient, Xiangbei Yangrong Tang (Fragrant Fritillaria Nourishing Nutrient Decoction) is indicated. Externally, apply a paste made from scallion white and honey. If the mass does not resolve after a long time, moxa-burn daily using Yangsui Ding (Yang-Sun Plaster), continuing until the mass dissolves, softens, or is about to suppurate. Once ulcerated, the treatment follows that of Luoli.
Xiangbei Yangrong Tang (Fragrant Fritillaria Nourishing Nutrient Decoction)
Bai Zhu (Stir-fried with soil, 2 qian), Ren Shen, Fu Ling, Chen Pi, Shu Di Huang, Chuan Xiong, Dang Gui, Bei Mu (heart removed), Xiang Fu (stir-fried with wine), Bai Shao (stir-fried with wine), each 1 qian; Jie Geng, Gan Cao, each 0.5 qian.
Add three slices of fresh ginger and two dates. Decoct in two sheng of water until reduced to eight fen. Take after meals.
If there is chest and diaphragm fullness and oppression, add Zhi Qiao and Mu Xiang. If appetite is poor, add Hou Po and Cang Zhu. If there are alternating chills and fever, add Chai Hu and Di Gu Pi. If thirst occurs after suppuration, double the dosage of Ren Shen, Dang Gui, and Bai Zhu, and add Huang Qi. If pus is excessive or thin, double the dosage of Dang Gui and Chuan Xiong. If there is pain or fullness in the hypochondrium, add Qing Pi and Mu Xiang. If muscle regeneration is slow, add Bai Lian and Rou Gui. If there is excessive phlegm, add Ban Xia and Ju Hong. If there is dry mouth, add Mai Dong and Wu Wei Zi. If there is fever, add Chai Hu and Huang Qin. If thirst persists, add Zhi Mu and Chi Xiao Dou. If pain returns after ulceration, add Shu Fu Zi and Chen Xiang. If pus discharge continues, double the dosage of Ren Shen and Dang Gui, and add Huang Qi. If there is deficiency-related irritability and insomnia, double the dosage of Ren Shen and Shu Di Huang, and add Yuan Zhi and Suan Zao Ren.
[Formula Song] Xiangbei Yangrong uses Si Jun [Tang], combined with Si Wu, Bei Mu, Jie Geng, Xiang Fu, and Chen Pi; suitable for prolonged use in cases of dual Qi and Blood deficiency, effective as magic for Jinluoli and Shiju.
