The general name for Gan Chuang (Chancre) encompasses three primary causes: unfulfilled sexual desire leading to difficult and dribbling urination; the application of aphrodisiac drugs causing itching, purplish discoloration, and bright red swelling due to Syphilis.
[Note] This condition is generally called Gan Chuang (Chancre), also known as Du Jing Chuang (Jealousy Sperm Chancre). It occurs on the external genitalia. The Classics state: "The external genitalia are the gathering place of the Zong Jin (Tendons), governed by the Du Mai (Governing Vessel) meridian network, circulating through the Yin Qi (genitals) and converging at the Cuan Jian (perineum)." It is also stated: "The Kidney opens into the two Yin orifices (anterior and posterior)." Therefore, sores arising here belong to the Liver, Du Mai, and Kidney meridians. Although named differently, their forms vary: those occurring below the Ma Kou (urethral meatus) are named Xia Gan (Lower Chancre); those above the penis are named Zhu Gan (Moth-eaten Chancre); sores on the penis with swollen skin wrapping around are named Xiu Kou Gan (Cuff Chancre); chancre that has ulcerated extensively over time is named La Zhu Gan (Candlestick Chancre); pain radiating to the testicles with a swollen and hanging scrotum is named Ji Yan Gan (Chicken Eye Chancre); painful and itchy, superficially ulcerated resembling peeled rotten apricots is named Sao Gan (Itching Chancre); occurring beside the urethral meatus, with holes like palm eyes, itchy inside, yielding slight pus upon pressure, is named Xuan Gen Gan (Rotating Root Chancre); when Chancre Mei Yang (Syphilitic Chancre) occurs, or due to mistaken use of fumigation and topical applications causing rot resembling a mortar, it is named Mei Yang Gan (Syphilitic Chancre); furthermore, when Chancre Mei Yang occurs, and one ingests Jie Yao (drastic medicines made from Calomel/Mercurous Chloride and Mercury), causing stinging pain in the urethra during urination, it is named Mei Yang Nei Gan (Internal Syphilitic Chancre). The origins of these various chancres are threefold: First, due to aroused sexual desire and rampant lust not being discharged, leading to stagnation of spoiled essence and turbid blood forming swellings; initially presenting with dribbling and painful urination, followed by discharge of yellow turbid spoiled semen, gradual damage to the penis, and eventually swelling, pain, and rotting; treatment should focus on draining damp-heat from the Liver and Kidney, primarily using Ba Zheng San (Eight Righteousness Decoction) and Qing Gan Dao Zhi Tang (Clear Liver and Guide Stagnation Decoction). Second, due to the application of hot aphrodisiac drugs to the penis and washing the genitals, seeking prolonged potency without ejaculation, leading to fire stagnation and swelling; initially presenting with itchy, painful, and hard penis, gradually developing nodules that are purple, rotting, with dripping blood and fluid, and frequent erections; treatment should focus on draining fire and resolving toxicity, primarily using Huang Lian Jie Du Tang (Coptis Decoction for Resolving Toxicity) and Lu Hui Wan (Aloe Pill). Third, due to intercourse with prostitutes whose vaginal secretions contain stagnant essence and turbid qi, transmitting Syphilis; initially presenting with red, shiny, swollen skin, transparent like crystal, breaking to flow foul-smelling fluid, intermittent numbness and itching, and increasing daily swelling and pain; treatment should focus on resolving toxicity, primarily using Long Dan Xie Gan Tang (Gentian Decoction to Drain the Liver), followed by Er Zi Xiao Du San (Two Seeds Detoxification Powder), and externally washing with Da Dou Gan Cao Tang (Soybean and Licorice Decoction); for redness, swelling, heat, and pain, apply Carp Bile; for broken and rotting lesions, apply Feng Yi San (Phoenix Skin Powder), Han Luo San (Land Snail Powder), Zhen Zhu San (Pearl Powder), Yin Fen San (Silver Powder), and Hui Chun Tuo Gan San (Revitalize and Remove Chancre Powder) according to the syndrome.
