Baihui Ju (Baihui Carbuncle) occurs at the vertex of the head, belonging to the Du Meridian (Governor Vessel) at the Baihui acupoint. Initially, it resembles a millet grain, gradually enlarging to the size of a coin; in severe cases, it resembles a grape cluster and is as hard as iron. If presenting with high swelling, heat, and excess syndrome, clear the toxic fire; if presenting with flat collapse and Yang deficiency, apply warming tonification and avoid harsh purgatives. If the swelling extends to the ears and neck, accompanied by rattling phlegm sounds, and fails to suppurate and ulcerate within seven days, the patient's life will be lost.
[Note] This condition, known as Baihui Ju or Yuding Fa (Jade Top Eruption), arises at the center of the vertex, belonging to the Du Meridian at the Baihui acupoint. It is caused by excessive consumption of rich and greasy foods, leading to the coagulation of fire-toxin. Initially, the shape resembles a millet grain; it gradually swells with a broad base like a coin. In severe cases, it resembles a grape cluster, hard as iron, with high, pointed redness and swelling, burning pain, a gathered root, aversion to cold with high fever, extreme thirst where drinking provides immediate relief but dryness returns quickly, bitter taste in the mouth, scorched lips, constipation, irritability, and a surging and rapid pulse. This belongs to Qi Excess syndrome. It is appropriate to take Huanglian Xiaodu Yin (Coptis Decoction for Disinfecting Toxins) to clear toxic fire, and externally apply Chonghe Gao (Harmonizing Ointment). If presenting with diffuse swelling and flat collapse, purple-dark hardness, scattered root of pain, aversion to cold, and diarrhea, with a fine and rapid pulse, this belongs to Yang Deficiency syndrome. It is appropriate to take Shiquan Dabu Tang (Ten-Ingredient Great Tonifying Decoction) to warm and tonify, and externally apply Huiyang Yulong Gao (Restore Yang Jade Dragon Ointment). If presenting with excessive facial redness, extreme irritability, dry mouth without thirst, and moist lips, this belongs to floating and superficial Yang Deficiency. It is appropriate to take Guifu Dihuang Wan (Cinnamon and Aconite Rehmannia Pill) to guide fire back to its source, and place raw Fuzi Bing (Aconite Cake) on the Yongquan acupoints (KI1) of both soles, moxibustion five cones each, to drain the toxin. At the initial stage, apply Hupo Gao (Amber Ointment); after ulceration, sprinkle Huang Ling Yao (Yellow Spirit Medicine) and cover with Taiyi Gao (Supreme Unity Ointment); after all necrotic tissue is removed, change to muscle-regenerating medications for treatment. If the swelling connects to the ears and neck, with phlegm sounds like sawing wood, no pus formation or ulceration after seven days, and coma, death is inevitable!
