Baimai Ju (Abscess of Hundred Vessels) presents with swelling, specific color and shape. The lesion extends to the ear and encircles the neck, appearing purple-red. Patients experience pain, fever, inability to eat, qi rebellion, and coughing. If puncture yields pus, it is a favorable sign; if blood flows, it is an unfavorable sign.
[Note] When this Ju first appears, there is diffuse swelling in several patches surrounding the neck. The color is purple-red, accompanied by pain, fever, loss of appetite, qi rebellion, and coughing, with the onset radiating to the ear. Puncture may be performed after fifteen days; delay allows pathogenic toxins to attack the throat. Pus discharge indicates a smooth course (Shun), while blood discharge indicates a reversed course (Ni). Other treatment methods refer to the sections on Yong (Carbuncle) sores and ulcers.
Jiehou Yong (Abscess at the Thyroid Cartilage) occurs at the center of the anterior neck (at the thyroid cartilage). If liver and lung accumulated heat block the throat, the condition is critical. If abscesses form and are not rapidly punctured, rupture into the throat makes survival difficult.
[Note] This Yong occurs above the Jiehou at the anterior neck, also called Meng Ju (Fierce Abscess) due to its fierce toxic nature. This area belongs to the Ren Mai (Conception Vessel) and is caused by accumulated heat and indignation in the Liver and Lung meridians. Severe swelling blocks the throat, preventing swallowing of liquids, which is terrifyingly dangerous. If abscesses form but are not needled, internal rupture through the throat leads to death. Initially, administer Huanglian Xiaodu Yin (Coptis Decoction for Detoxification) and apply Erwei Baidu San (Two-Ingredient Toxin-Pulling Powder) externally. Treatment for impending rupture refers to the sections on Yong sores and ulcers.
Jiahou Yong (Abscess Flanking the Throat) arises on both sides of the throat, caused by toxic heat in the Liver and Stomach. Its treatment is the same as Jiehou Yong, but particular attention must be paid to phlegm obstruction causing frequent choking.
[Note] This Yong, also known as Jia Ju, arises on both sides of the Jiehou, resulting from fire-toxic ascending from the Foot-Jueyin Liver Meridian and Foot-Yangming Stomach Meridian. Its treatment is identical to that of Jiehou Yong.
Xiao Luo Da Li (Small Luo and Large Li) involves the Three Yang Meridians and can occur at the front, back, or sides of the neck. Although named differently such as Tan Luo (Phlegm Luo), Shi Luo (Dampness Luo), Qi Li (Qi Li), and Jin Li (Tendon Li), they all originate from anger and depression transforming into heat. One must carefully examine lingering symptoms; if prolonged without healing, it evolves into consumption (Lao), making cure difficult.
[Note] Small lesions are called Luo, large ones Li. Differentiate by meridian: those at the anterior neck belong to the Yangming Meridian, named Tan Luo; those at the posterior neck belong to the Taiyang Meridian, named Shi Luo; those on the left and right sides belong to the Shaoyang Meridian, soft and swelling upon anger, named Qi Li; hard with tendon contraction, named Jin Li; continuous like beads, named Luo Li; or long like clams, red and hard, burning pain, severe swelling, named Ma Dao (Horse Knife). Luo Li has mother-child variations in size. Chongtai Li involves stacking three to five nodules. Shepan Li wraps around the neck like beans in a basket, also called Suoxiang Li. Those at the left ear root are Fengwo Li; at the right ear root, Huidai Li. Small and itchy ones are Feng Li. Red, swollen, painful submandibular areas are Yanwo Li. Extending to chest and axilla are Gouteng Li. At soft flesh beside breasts or groin are Yi Yu. Widespread, diffuse, soft with hard cores inside are Liuzhu. Single nodule at fontanelle is Danke Li. One sac with ten-plus nodules is Lianzi Li. Hard as brick is Menzhuan Li. Lychee-shaped is Shi Li. Rat-shaped is Shu Li, also Shu Chuang. Above all, movable nodules have no root, belong to Yang, and treat externally with acupuncture, poultices, or cauterization. Immobile, deep-rooted nodules belong to Yin and are incurable. Avoid needles and corrosive drugs; misuse prevents healing.
Luo Li varies in form but stems from Phlegm, Dampness, Wind, Heat, and Qi-Toxin accumulation, invariably combined with anger, depression, stagnation, and unfulfilled plans. External wind invading with internal phlegm-damp binding meridians causes initial chills then fever, mild swelling/heat, normal skin color, easy resolution/ulceration/healing—this is Wind-Toxin, treat with Fangfeng Qianghuo Tang or Haicai Wan. Heavenly heat, summer-damp affecting Three Yang, plus rich food causing heat-toxin results in red, mildly hot, hard slow-swelling nodules, difficult to resolve/ulcerate/heal—this is Heat-Toxin, treat with Shengyang Tiaojing Tang, Chaihu Lianqiao Tang, or Jiming San based on severity. Four-season pestilential qi causes sudden swelling in ears/neck/chest/axilla, red hot skin, chills/fever, dizziness/stiff neck—this is Qi-Toxin, treat with Li Gao's Lianqiao Sanjian Tang or Sanzhong Kuijian Tang. Liver injury from anger with blood deficiency failing to nourish tendons causes hard nodes, tendon contraction, immobile—this is Jin Li, initially treat with Shugan Kuijian Tang, then Xiangbei Yangrong Tang. Ingestion of sweat, insects, rat remains, stale water/tea causes small-to-large bead-like chains, no chills/fever, late pain—this is Poisonous Food Toxin, treat with Yang Family's methods or Zhi Lingji Dan based on excess/deficiency.
Posterior lateral Shi Luo Li belongs to Bladder Cold Water, external wind-damp binding causes diffuse swelling/pain, normal skin. Impending rupture shows red skin, mild heat/severe pain. Treatments should avoid cold/cool herbs. Initial swelling: Fuzi Baidu Tang, Shen Gong San externally. Impending/ulcerated stages follow standard Yong/Li protocols. Warmth throughout treatment is effective; cold/cool herbs cause neck/back stiffness, sunken sores, internal toxin attack, diarrhea—these are adverse signs. Men with Luo Li should not exhibit exposed Taiyang blue veins, tidal fever/cough, spontaneous/dream sweating. Women should not exhibit red eye threads, amenorrhea/bone-steaming fever, five-palm heat. These indicate transformation into Sore Consumption, all adverse and difficult to cure.
