Scrotal eczema (Shennang Feng) originates from the Liver meridian, caused by external invasion of wind and dampness. Symptoms include itching and numbness; scratching breaks the skin leading to exudation of serous fluid. In severe cases, nodules appear, causing burning pain.
[Note] This condition is also known as "Embroidered Ball Wind" (Xiuqiu Feng), characterized by scrotal pruritus. The etiology involves internal damp-heat in the Liver meridian combined with external wind pathogen invading the skin and interstices. Initially, the skin is dry with extreme itching, and patients prefer hot water baths. In severe stages, nodules resembling red millet seeds appear. Numbness, itching, breaking upon scratching, oozing serous fluid, along with hot, painful skin feeling like a burn, indicate interior heat syndrome. All such cases should be treated internally with Longdan Xiegan Tang (Gentian Liver-Draining Decoction) and externally washed with Shechuangzi Decoction (Cnidium Monier Decoction). After washing, applying Yelangdu Ointment (Stellera Root Ointment) is highly effective.
Shechuangzi Decoction (Shechuangzi Tang)
Weilingxian (Clematis Root), Shechuangzi (Cnidium Fruit), Dangguiwei (Angelica Sinensis Tail) [each 5 qian], Suoshaqiao (Amomum Villosum Shell) [3 qian], Tudahuang (Rheum officinale or similar), Kushen (Sophora Flavescens) [each 5 qian], Laocong tou (Old Scallion Bulb) [7 pieces].
Boil in 5 bowls of water until boiling several times. Pour into a basin. First fumigate, then wash when warm.
[Formula Song] Shechuangzi Tang washes scrotal wind, stopping itch and dispelling wind/dampness effectively; Weiling, Danggui tail, Amomum shell, plus Rheum, Sophora, and Scallion.
Yelangdu Ointment (Yelangdu Gao)
Yelangdu (Stellera Chamaejasme), Chuanjiao (Sichuan Pepper), Liuhuang (Sulfur), Binglang (Areca Seed), Wen Ge (Clam Shell), Shechuangzi (Cnidium Fruit), Dafengzi (Croton Seed), Ku Bai Fan (Alumen Calcined) [each 3 qian].
Grind all into fine powder. Boil one teacup of sesame oil, add one bile from a male pig, mix evenly with the powder, and apply to the affected area.
[Formula Song] Yelangdu Ointment rubs Embroidered Ball Wind, treating wet itch and burning pain; Pepper, Sulfur, Areca, Clam, Cnidium, Croton, Alumen mixed with Pig Bile Oil.

Female Yin Sores (Yin Chuang) is a general term; each has specific manifestations and belongs to different meridians. Yin Ting (Prolapse, resembling a snake) is due to Spleen deficiency. Yin Zhong (Swelling) results from labor injuring the Blood aspect. Yin Shi (Erosion/Ulcer) is caused by Stomach deficiency and accumulated depression. Yin Tuo (Relaxation/Openness) arises from excessive worry and thought. Yin [Missing Character] (likely Pruritus) is due to dual deficiency of Qi and Blood. Treatment must be symptomatic to resolve these conditions.
[Note] These conditions occur on the genitalia. If a snake-like structure protrudes from the vagina, it is called Yin Ting, caused by Spleen meridian weakness or postpartum exposure to wind after anger. Initially, treat with Xiaoyao San (Free and Easy Wanderer Powder) plus Jingjie (Schizonepeta) and Fangfeng (Saposhnikovia). Subsequently, take Guozhong Yiqi Tang (National Middle Qi-Boosting Decoction, i.e., Buzhong Yiqi Tang) with increased Shengma (Cimicifuga) in the morning, and Longdan Xiegan Tang in the evening; externally wash with Shechuangzi decoction. If the vulva suddenly swells and hurts, it is called Yin Zhong, also known as Bang Ju (Clam Abscess), caused by labor injuring the blood. Treat with Siwu Tang (Four Substance Decoction) plus Danpi (Moutan Bark), Zexie (Alisma), Huafen (Trichosanthes Root), and Chaihu (Bupleurum); or Qin Jiao Tang (Gentian Macrophylla Decoction). Externally, fumigate/wash with Aiye (Mugwort Leaf) 1 liang, Fangfeng 6 qian, Daji (Euphorbia Lathyris) 5 qian boiled in water. If nodules grow outside the genitals and small insects cause itching inside, it is called Yin Shi, also known as [Missing Character] Chuang, caused by Stomach deficiency and stagnation. Treat with Siwu Tang plus Shichangpu (Acorus), Longdancao (Gentian Root), Huanglian (Coptis), Mutong (Akebia). If chills/fever resemble consumptive disease, worms have entered the viscera; treat with Xiaoyao San swallowing Lubin Wan (Aloe Pill), twice daily. Externally fumigate/wash with [Missing Character] Yang Tang, then insert Yinxin San (Ginkgo Powder) to kill worms and stop itching. If the vulva opens and does not close, with itching, pain, and discharge, it is called Yin Tuo, caused by excessive worry. Treat with Xiaoyao San or Guipi Tang (Restore the Spleen Decoction) both added with Chaihu, Zhizi (Gardenia), Baishao (White Peony), and Danpi; if postpartum, use Buzhong Yiqi Tang plus Wuweizi (Schisandra) and Cu Chao Baishao (Vinegar-fried White Peony). Externally, all cases use Jingjie, Zhike (Citrus Aurantium), Hezi (Chebula), Wen Ge in large doses for fumigation/washing. If the uterus prolapses, named Yin [Missing Character] (commonly "Gourd"), caused by Qi and Blood deficiency, use Buzhong Yiqi Tang removing Chaihu, doubling Shengma, adding Yimucao (Motherwort). Externally, apply mashed Castor Bean flesh to the Baihui acupoint (vertex), then fumigate with half a jin of Zhike decoction. If caused by unfulfilled desires and Liver Qi stagnation, there is initially a sensation of urinary blockage, leading to straining; over time, the organ descends with straining. Have a midwife reduce the prolapse, let the patient lie supine with a pillow under the waist, blow sneezing medicine to induce sneezing for contraction. Once reduced, tightly bind the legs with cloth, and continue internal Buzhong Yiqi Tang for self-healing.
Qin Jiao Tang (Gentian Macrophylla Decoction)

[Missing Name] Itch-Killing Decoction
Kushen, Yelangdu, Shechuangzi, Dangguiwei, Weilingxian [each 5 qian], Heshicao (Nemesis Herb) [1 liang].
Boil in 10 bowls of river water, strain dregs, store in basin. Fumigate while hot, then add 2-3 male pig gallbladders when warm, mix and wash. Highly effective.
[Formula Song] [Missing Name] Decoction kills worms and treats Yin Shi, fumigation/washing should not be delayed; Sophora, Stellera, Cnidium, Angelica tail, Pig Gall, Clematis, Nemesis applied.
Yinxin San (Ginkgo Powder)
Qingfen (Calomel), Xionghuang (Realgar), Shuiyin (Mercury, lead-processed), Xingren (Apricot Kernel, raw) [each 1 qian].
Grind each separately, mix uniformly. Use 5 fen per dose. Form pill with one jujube flesh, wrap in silk cotton, tie tightly with thread. Insert into vagina, leaving thread end outside. Remove during urination, reinsert after. Change once daily. Self-heals in 4-5 days.
