Tianshe Du (Snake Poison), also known as Shetou Ding (Snake-Head Carbuncle), is a condition caused by excessive Heart Fire attacking and accumulating locally. The affected finger swells like a snake's head, presenting with redness, swelling, and pain that may radiate to the heart region. In severe cases, alternating chills and fever occur, with swelling and pain spreading upward. Apply five moxa cones on the apex of the swelling, and apply Xionghuang San (Realgar Powder) topically. Internally administer Chansu Wan (Toad Venom Pill) to induce sweating and resolve toxicity; mild cases gradually subside, suppurative cases rupture with pus discharge, and severe cases result in tissue necrosis. If swelling persists after rupture, insert a Chansu Strip into the wound and cover with an ointment plaster for effective results; for necrotic wounds, apply Yuhong Gao (Jade Red Ointment); for patients with deficiency who fail to heal, concurrently administer tonifying formulas.
Xionghuang San (Realgar Powder)
Xionghuang San contains Chansu (Toad Venom), Bingpian (Borneol), and Qingfen (Calomel).
Grind these four ingredients into a fine powder, mix with water, and apply to the affected area three times daily.
It is indicated for early-stage Tianshe Du characterized by redness, swelling, heat, and pain penetrating to the heart.
Ingredients: Xionghuang (Realgar, bright quality, 2 qian), Chansu (Toad Venom, slightly roasted, 2 fen), Bingpian (Borneol, 1 fen), Qingfen (Calomel, 5 fen).
Grind all into a fine powder, mix with fresh cold water, apply topically, cover with paper, and use three times daily for excellent efficacy.

The following formula is suitable, effective regardless of whether the disease is new or chronic, or whether the etiology is genuine or suspected.
