A man over thirty years old, who was constitutionally weak and unable to tolerate cold food, developed an arm abscess. After it healed, he had little desire for food, experienced occasional abdominal distension or acid regurgitation, and became increasingly emaciated. Medications such as Ginseng and Poria (Shenling) were ineffective. His right chi pulse was weak, indicating Mingmen Fire Decline (deficiency of Kidney Yang), which failed to generate Earth (Spleen/Stomach function). Thus, Bawei Wan (Eight-Ingredient Pill with Rehmannia) was prescribed to tonify the root of Earth. His appetite improved, and he recovered.
Another patient suffered from a Spleen and Stomach disorder; treatment with tonifying agents and acupuncture at Pishu (BL20) was ineffective, and his condition nearly became critical. He recovered after taking three doses of Bawei Wan (Eight-Ingredient Pill with Rehmannia). Another patient with edema due to Spleen Deficiency recovered within half a year after taking this pill.
A third patient appeared robust but had weak Qi; he experienced dizziness when exposed to wind and mouth/tongue ulcers after exertion. He often had phlegm in his chest and red, dry eyes. He found relief after taking this pill. A fourth patient suffering from a Spleen disorder was about to recover after taking this pill, but a quack doctor intervened, prescribing cold-natured medicines instead, leading to his death.
I have verified cases where individuals could not endure fatigue or consume cold foods, experienced abdominal distension after eating, loose stools, or frequent mouth/tongue ulcers. Their symptoms worsened with cold-natured medicines. They also presented with frequent urination, weakness in the lower back and legs, a sensation of swallowing saliva, feeling cold air entering the abdomen upon breathing, dampness and itching in the scrotum, cold hands and feet, pale complexion, dark complexion, aversion to cold, and shortness of breath. All these symptoms belong to the Kidney category, and Aconite (Fuzi) is indispensable.
Several verified treatment cases are mentioned in the previous discussion on "Thirst."
Zhu Danxi stated: Shiliuwei Liuqi Yin (Sixteen-Ingredient Decoction for Circulating Qi) is a formula regulating the exterior, interior, Qi, and Blood. When combined with herbs that disperse wind and assist Yang, it should not be used unless the pulse is surging, moderate, slow, tight, or thin. Practitioners often fail to differentiate meridians, pulses, and symptoms, ignoring seasonal appropriateness. They merely claim to detoxify and resolve toxicity, and when ineffective, add Qi- and Blood-tonifying herbs, causing doubt.
The Classic states: "All pain, itching, sores, and ulcers belong to Heart Fire." This refers to the general rule. However, if sores are severe while the body is emaciated, the pathogen is high but pain is low, and heat precedes cold, this is an atypical presentation requiring flexible treatment based on current conditions to achieve complete success. This formula regulates the exterior, interior, Qi, and Blood. If used for carbuncles and sores, whether newly erupted or established, or for internal support, especially when the patient is fatigued, has mild aversion to cold and fever, or presents with a moderate, choppy, wiry, tight, or thin pulse, it is suitable for dispersing Wind-Cold to assist Yang. This addresses the variation of "heat preceding cold." If applied to accumulated heat toxins without considering meridians and season, one cannot help but feel apprehensive.
Zhu Danxi said: *Jingyao* advocates rapid dispersion for unformed sores and rapid drainage for formed ones. For minor conditions or in winter, internal support methods can be used. In winter, for swelling/ulceration, using this formula can turn heavy into light and deep into shallow. However, in summer, for open ulcers, the warming and dispersing nature of Cinnamon Twig (Guizhi) and Phellodendron Bark (Huangbai/Houpo context implies bark) assisted by Saposhnikovia Root (Fangfeng) and Angelica Dahurica (Baizhi), even with Ginseng and [missing character, likely Atractylodes or Astragalus], are difficult to rely upon. People use this formula indiscriminately, regardless of whether it is a carbuncle or sore, winter or summer, without regard for meridians or sequence, like a blind man riding a blind horse approaching a deep pool at midnight—how dangerous! He further stated: There are too many blood-drying and Qi-draining herbs; those with deficiency should not use it lightly. A scholar developed carbuncles sequentially on his back, buttocks, and legs. Using Wuxiang Liaoqiao Tang (Five-Aromatic Liaoqiao Decoction) and Shixuan San (Ten-Aperture Powder) led to his death. Another sixty-year-old man, fond of alcohol and meat, suffered from a back carbuncle and recovered after consuming fifteen to sixteen catties of Dushen Gao (Single-Ginseng Paste). Had Shixuan San (Ten-Aperture Powder) been used, danger would surely have ensued.
