Carbuncle (Ding Chuang) is caused by the accumulation of heat-toxin and pathogenic wind in the Zang-Fu organs, which struggle against each other within the meridians, leading to stagnation of Qi and Blood. This stagnation flows into the hair follicles, hands, feet, head, and face, manifesting according to the regions corresponding to the Five Zang organs. The lesion resembles a millet grain, causing pain or itching, potentially leading to generalized numbness, dizziness, alternating chills and fever, frequent nausea and vomiting, and in severe cases, heavy limbs, palpitations, and blurred vision. Initially, the swelling protrudes like a nail head, hence the name "Ding" (Nail). Carbuncles contain toxic qi, protruding about one cun, with extreme pain and itching; they can be fatal within one or two days, making them more dangerous than typical abscesses (Yong Ju). According to *Nei Jing*, white Ding appears on the right nostril, red Ding at the root of the tongue, yellow Ding on the lips, black Ding in front of the ears, and green Ding below the eyes, reflecting the correspondence of the five colors to the five Zang organs and their respective locations. However, they may also appear on the shoulders, waist, or feet without fixed positions; those appearing in the joints of the hands, feet, head, and face are most urgent, while others are less so. *Qian Jin Fang* describes thirteen types of Ding:
1. Hemp Seed Ding (Ma Zi Ding): Flesh rises, head like a millet grain, slightly dark color, surrounding area slightly red and itchy. Avoid hemp seed oil, wearing hemp clothes, and walking in hemp fields.
2. Stone Ding (Shi Ding): Skin and flesh connected, color like black beans, very hard, cannot be pierced, slight muscle pain. Avoid gravel and bricks.
3. Male Ding (Xiong Ding): Protrudes, black crust on head, edges everted, yellow pus flows out, size like a coin hole, high shape. Avoid sexual intercourse.
4. Female Ding (Ci Ding): Slightly yellow sore, inward depression, similar to moxibustion scar, pus around edges, center concave and red like a coin hole. Avoid sexual intercourse.
5. Fire Ding (Huo Ding): Like scalding or burning, black crust on head, smoky pus on edges, like red millet grains. Avoid fire and branding irons.
6. Rotting Ding (Lan Ding): Slightly dark with white spots, ulcerated with pus flowing out, size like a spoon surface. Avoid hot food and rotten matter.
7. Thirty-Six Ding (San Shi Liu Ding): Black floating head, shape like black bean, red edges. One appears today, two tomorrow, up to ten. If thirty-six appear, it is incurable; if fewer, it is treatable. Avoid anger and resentment.
8. Snake Eye Ding (She Yan Ding): Black floating skin on head, shape like small bean, resembling snake eye, generally hard. Avoid being looked at by envious people and avoid poison drugs.
9. Salt Skin Ding (Yan Fu Ding): Size like spoon surface, all edges red, black millet-like granules rising. Strictly avoid salt.
10. Water Wash Ding (Shui Xi Ding): Size like a coin, shape like coin hole, white head with black crust, hard center when juice flows out. Avoid drinking cold water, washing with water, and crossing rivers.
11. Blade Ding (Ren Ding): Width like leek leaf, one cun long, left side black like burned. Avoid piercing and cutting; iron blade injuries can be treated with medicine, do not attack randomly.
12. Floating Bubble Ding (Fu Ou Ding): Round and curved body, slightly elongated and narrow like leek leaf, yellow inside and black outside. Puncturing black part causes no pain, puncturing yellow part causes pain.
13. Ox-Dog Ding (Niu Gou Ding): Flesh rises, cannot be broken by pinching.
For these thirteen types, initial symptoms include itching then pain in the sore center, followed by chills then fever. When fever subsides, chills predominate, limbs feel heavy, heart panics, and vision blurs. Severe cases involve vomiting; such cases are difficult to treat. Hemp Seed Ding only itches throughout; violating its specific prohibitions makes treatment difficult. Floating Bubble Ding and Ox-Dog Ding have no prohibitions and are rarely fatal even untreated. Their chills/fever patterns differ from others; careful nursing is required. Spinal stiffness and unbearable pain indicate violation of prohibitions. Other types include Red Thread Ding (Hong Si Ding) and Fish Navel Ding (Yu Qi Ding), with many names. Red Thread Ding appears between hands/feet with a red thread; it must be pricked immediately, otherwise it enters the Heart and becomes incurable. Fish Navel Ding resembles a fish navel. Treatment involves pricking the center and painful areas, then surrounding areas十余 times to bleed out evil blood before applying medicine. Medicine entering the needle holes is ideal. If penetration fails, check for red-black bead-like spots on the cheeks or tongue, indicating toxin invasion. Treatments are similar: prick to bleed, insert Chan Su Pill (Toad Venom Pill) or Hui Ding Dingzi (Return Carbuncle Stick) into the hole, cover with plaster, take Fei Long Duo Ming Dan (Flying Dragon Life-Grabbing Pill) to induce sweating, and drink Wu Xiang Lian Qiao Lou Lu Tang (Five-Spice Forsythia and Rhapontic Decoction) along with Heart-clearing agents. All sores relate to Heart Fire; clearing it dissipates toxins.
Zhui Du Dan (Pursue Toxin Pill): Extracts Huang Lian (Coptis Rhizome), removes Ding head, effectively pursues pus/toxin.
Chan Su (Toad Venom, 1 qian, dried dissolved in wine), Wu Gong (Centipede, soaked in wine, roasted yellow), Sha (Realgar, 1 qian), Bai Ding Xiang (White Civet Cat Feces, 1 qian, substitute with Ba Dou if unavailable), Ba Dou (Croton Seed, 7 grains, shelled, oil retained), Xiong Huang (Realgar, 2 qian), Qing Fen (Calomel, 1 qian), Zhu Sha (Cinnabar, 2 qian, as coating). Grind to fine powder, mix with flour paste to form pills. If unable to form, use wine-flour paste to make wheat-grain-sized, pointed pills. Insert into pricked wound, cover with Shui Chen Gao (Water-Sinking Plaster), then use plasters and flesh-regenerating medicines to expel pus/blood/toxins. Also used for black-depressed fistulas where dead flesh prevents healing; this medicine expels toxins and dead flesh to promote new growth. Small lesions use one pill, large ones increase dose. Mild cases need no pricking; apply with fingernail scratching on top, cover with Shui Chen Gao. Redness/swelling indicates effect; removing dead flesh is optimal.
Shui Chen Gao (Water-Sinking Plaster): Place Bai Ji (Bletilla Rhizome) powder in a cup, let settle with water, apply paper over it. Do not use flesh-regenerating medicine with this plaster. Use this plaster to surround medicated points to protect healthy flesh.
Zhui Ding Duo Ming Tang (Pursue Carbuncle Life-Grabbing Decoction): Secret formula, fast acting, reduces internal swelling.
Qiang Huo (Notopterygium Rhizome), Du Huo (Angelica Pubescens Root), Qing Pi (Citrus Unripe Peel, use more), Fang Feng (Saposhnikovia Root, use more), Huang Lian (Coptis Rhizome), Chi Shao Yao (Red Peony Root), Xi Xin (Asarum Root), Gan Cao (Licorice Root), Chan Tui (Cicada Slough), Jiang Can (Bombyx Batryticatus), Jiao Lian (Chicken-claw Coptis, equal parts). Add He Che Che (Placenta Humanus), Ze Lan (Lygodium Rhizome), Jin Yin Hua (Honeysuckle Flower). For pus add He Shou Wu (Polygonum Multiflorum Root), Bai Zhi (Angelica Dahurica Root). For purgation add Qing Mu Xiang (Aucklandia Root), Da Huang (Rhubarb Root), Zhi Zi (Gardenia Fruit), Qian Niu (Pharbitis Seed). For lower body add Mu Gua (Chaenomeles Fruit, equal parts).
Chop ingredients. Take 5 qian per dose. First dose: add Ze Lan (few leaves), Jin Yin Hua (1 liang each), 10 slices of Sheng Jiang (Fresh Ginger), pound together, mix with wine, heat and drink. Non-drinkers boil in water. Then boil remaining wine/water (1.5 cups each), 10 ginger slices, drink hot until sweating. After symptoms subside, add Da Huang (2 qian), boil, drink hot, purge once or twice to remove residual toxin. Formula seems unrelated but is highly effective. Adjust based on symptoms. For irritability/vomiting, add Gan Cao Jie (Licorice Joint, 1 qian), Lv Dou Fen (Mung Bean Powder, taken with sour brine). For nausea, add Ru Xiang (Frankincense), Lv Dou Fen (taken with Licorice decoction).
Fei Long Duo Ming Dan (Flying Dragon Life-Grabbing Pill): Specifically treats Ding carbuncles, back carbuncles, brain abscesses, breast abscesses, deep bone abscesses, and all headless malignant swellings. Induces suppuration, converts painless to painful, heals established sores. Precious medicine for critical conditions. Family secret recipe.
Chan Su (Toad Venom, 2 qian, dried dissolved in wine), Xue Jie (Dragon's Blood, 1 qian), Ru Xiang (Frankincense, 2 qian), Mo Yao (Myrrh, 2 qian), Xiong Huang (Realgar, 3 qian), Qing Fen (Calomel, 0.5 qian), Dan Fan (Alum, 1 qian), She Xiang (Musk, 0.5 qian), Tong Lv (Malachite, 2 qian), Han Shui Shi (Halloysitum, 1 qian), Zhu Sha (Cinnabar, 2 qian, coating), Hai Yang (Sea Sheep/Snail, 20 pieces), Nao Zi (Camphor, 0.5 qian, optional), Tian Long (Heavenly Dragon/Centipede, 1 piece, roasted in wine, head/feet removed).
Grind Snails with shell into paste, mix with other fine powders to make mung-bean-sized pills. If needed, use wine-flour paste. Take 2 pills. Chew 3-cun Cong Bai (Green Onion White), spit into palm (left for men, right for women), wrap pill in onion, swallow with 3-4 cups hot non-grain wine. Cover patient in wind-free area for distance equivalent to walking 5 li. Drink more hot wine to boost effect. Fever and profuse sweating indicate efficacy. Two pills initially resolve mild cases. Repeat if no sweat. Severe cases (3-5 days) take another 2 pills. If toxin spreads to Heart (Zou Huang), prognosis is poor; cold sweat indicates death. If patient cannot chew onion, crush and wrap pill, swallow with hot wine. Upper sores taken after meals, lower sores before meals. Post-medication diet: avoid cold water, cucumber, eggplant, oil, pork, chicken, fish, meat, wet noodles, and all wind-inducing/toxin-promoting foods. Avoid contact with menstruating women and fox odor. Highly effective.
Ba Huang Yao (Remove Yellow Medicine): Chan Su (Toad Venom) and Fei Luo Mian (Fine Wheat Flour) made into wattle-seed-sized pills. Place under tongue to expel yellow fluid immediately.
Bai Er San (Two-Hundred Powder / Hu Xin San - Protect Heart Powder): Treats irritability and madness from Ding carbuncles. Take urgently.
Gan Cao Jie (Licorice Joint), Lv Dou Fen (Mung Bean Powder), Zhu Sha (Cinnabar, equal parts).
Grind to powder. Take 3 qian mixed with warm water.
Chan Su Gao (Toad Venom Plaster): Treats Ding carbuncles.
Mix Chan Su (Toad Venom) with Bai Mian (White Flour) and Huang Dan (Lead Monoxide) into a paste, roll into wheat-grain-sized pills. Scratch sore, insert pill. For severe cases, prick head, insert one pill, cover with Shui Chen Gao.
Hui Chuang Dingzi (Return Sore Stick): Highly effective for Ding carbuncles.
Cao Wu Tou (Aconite Lateral Root), Chan Su (Toad Venom, 7 grains), Ba Dou (Croton Seed, 7 grains, peeled), She Xiang (Musk, 1 zi).
Grind to powder, mix with flour paste, mold into sticks. For malignant sores/Ding without pain/blood, prick deeply to blood, insert stick, cover with plaster. Apply around edges. Ding naturally expelled in 2-3 days. Similar to Cold Damage (Shang Han); some require bleeding via acupuncture/lancets, others suppurate late. Variations are numerous; caution is essential.
Nei Tuo Lian Qiao San (Internal Support Forsythia Powder): For Ding carbuncles with unchanged skin color, no pain, immobility upon pressure, accompanied by chills/fever. Also for Water Ding, Fish Navel Ding, Purple Swallow Ding, Fire Ding. If yellow pus forms, prick it, take this powder to disperse naturally.
Lian Qiao (Forsythia Fruit), Bai Zhi (Angelica Dahurica Root), Sheng Di Huang (Rehmannia Root), Chi Shao Yao (Red Peony Root, 1 liang each), Da Huang (Rhubarb Root, peeled), Huang Zhi (Gardenia Fruit, stem/pedicel removed), Bo He Ye (Mint Leaf, 7 qian each), Pu Xiao (Mirabilite, 2 liang), Huang Qin (Scutellaria Root, core removed, 0.5 liang), Gan Cao (Licorice Root, 1.5 liang).
Coarse powder. Take 1 liang, boil in 1 bowl water with Deng Xin (Juncus Rush Pith) and Zhu Ye (Bamboo Leaf) to 0.7 portion. For wheezing, add Ren Shen (Ginseng Root). Serious cases heal in 3-4 doses. If irritability/vomiting occurs, stop with Bu Er San.
Bu Er San (Not-Two Powder):
Gan Cao (Licorice Root, 0.5 liang), Lv Dou Fen (Mung Bean Powder, 1 liang).
Grind to powder, divide into two doses, take with sour brine.

Fine powder, honey pills like bullet size. Take one pill before meal, dissolved in child's urine. Avoid alcohol, raw/cold/hard foods.
