Jie (Scabies) and Xuan (Tinea/Psoriasis) are both caused by Damp-Heat in the Spleen Meridian and Wind-Toxin in the Lung Meridian invading the skin and muscles. When Wind-Toxin is superficial, it manifests as Jie; when it is deep-seated, it manifests as Xuan. Xuan is entirely due to Wind-Toxin of the Lung, while Jie is formed by a combination of Damp-Heat from the Spleen. If left untreated for a long time, it spreads over the whole body, causing maceration, ulceration, itching, or pain, with various symptoms. Both conditions involve microscopic parasites and are contagious.
There are five types of Jie:
1. Da Jie (Great Scabies): Redness, itching, pain, and pus formation.
2. Ma Jie (Horse Scabies): Subcutaneous elevation with roots, insensitivity to pain upon scratching.
3. Shui Jie (Water Scabies): Contains serous fluid, clear water flows out when broken.
4. Gan Jie (Dry Scabies): Itching with dry scabs forming upon scratching.
5. Shi Jie (Damp Scabies): Thin-skinned small sores with continuous exudation.
Xuan begins on the skin, presenting as urticaria-like lesions, round or irregular, spreading like moss, containing fluid internally with external boundaries. There are six types of Xuan:
1. Gan Xuan (Dry Tinea): White scales appear upon scratching, looking withered and dry.
2. Shi Xuan (Damp Tinea): Exudes fluid upon scratching, spreading like insect movement.
3. Feng Xuan (Wind Tinea): Numbness and hardness upon scratching, loss of sensation.
4. Niu Xuan (Ox Tinea): Thick and hard skin resembling an ox's neck.
5. Gou Xuan (Dog Tinea): Intermittent mild itching with connected white spots.
6. Dao Xuan (Knife Tinea): Indistinct borders, irregular shape.
Treatment involves applying herbs that kill parasites, drain dampness, and detoxify externally, and taking internal formulas to harmonize the Spleen, clear the Lungs, dispel wind, and dissipate dampness to eradicate the root cause.
Additionally, facial Feng Xuan may start as small sores with intermittent pain and itching, occurring in spring, known as Chui Hua Xuan (Blowing Flower Tinea), common in women. This is due to accumulated Wind-Heat in the Lung Meridian rising to the face or around the eyes and eyebrows. If prolonged, it may develop into Wind Disease. Treatment requires clearing Heart Fire, dispersing Wind-Heat from the Lungs, and applying detoxifying and cooling topicals.
