Pediatric Cold Damage (Xiao'er Shanghan) is caused by exposure to severe cold during winter. The cold pathogen enters the Couli (pores and interstitial spaces of the skin), clashes with the Blood and Qi, and immediately manifests as aversion to cold and fever, headache, and body pain.
In general, infants and young children suffering from Cold Damage should not be treated with drying and acrid herbs to induce sweating. Inducing sweat in this manner causes extreme dryness in the Zang-Fu organs, excessive heat damaging Heart Qi, leading to Jue syndrome (cold extremities) which is often incurable; or it may cause tightness and distension of the skin, flesh, sinews, and vessels, which is also difficult to treat. If hot-natured decoctions are administered, they assist Yang. When Yang reaches its extreme, Yin must contend, resulting in oily sweat on the limbs, hands and feet that are alternately hot or cold, and frequent episodes of mania, epilepsy, fright, and convulsions. These conditions are considered incurable.
