Acute Gan (severe dental caries with tissue necrosis) arises from the accumulation and stagnation of wind-heat pathogens, causing obstruction and stasis in the Spleen and Lungs. The qi of pathogenic toxins rushes upward and floods into the mouth and teeth, thereby forming Gan disease. Its manifestations include sudden discoloration of the lips and oral cavity to cyanotic white, swelling and fullness of the gingiva, simultaneous discharge of pus and blood, rotting and pain of the teeth, red-and-white patches on the sides of the lips and cheeks, or the presence of black meridians/vessels. At this stage, it is necessary to prick the area with a needle to drain the evil blood; otherwise, cauterization should be employed.
For treating acute Gan eroding the teeth and gingiva, with ulceration, swelling, and pain of the lips and mouth, use Huangfan San (Yellow Alum Powder).
Ingredients: Huangfan (Calcanite), Qingfan (Green Vitriol), Baifan (White Alum, calcined until juices run out), Bai Gou Fen Hui (Ash of white dog feces), Mangcao (Radix Daphnes), Xionghuang (Realgar, finely ground, each half tael), Shidan (Chalcanthite, finely ground), Langdang (Datura seed).
Preparation and Administration: Pound and sieve the above ingredients into powder, mix thoroughly, and grind evenly. First, rinse the mouth with salted water three to five times. Then, apply the powder to the affected area. Use three times daily. If saliva is produced, do not swallow it.
