For the treatment of sudden heat toxins that are about to form carbuncles and swellings, the topical medications applied initially should address the fierce and intense heat which is unbearable. If the swollen area appears to have a head (indicating pus formation), warm pastes such as Qucao Gao (Herb-Expelling Paste) should be applied to facilitate resolution.
For the treatment of carbuncles and swellings characterized by pain from heat toxins, erosion of muscle tissue, redness with flaccid swelling, tenderness to touch, impending suppuration, or existing suppuration, with surrounding redness and swelling, the following prescription may be used: Hanshuishi (Halloysitum Rubrum, 2 liang), Yangtaogen (Akebiae Caulis Radix, 1 liang, crushed), Xiaoshi (Natrii Sulfas, 1 liang), Muxiang (Aucklandiae Radix, 0.5 liang), Bailian (Puerariae Radix, 0.5 liang), Dingxiang (Caryophylli Flos, 0.5 liang), Yupi (Ulmus Cortex, 0.5 liang, crushed), Chixiaodou (Vigna Semen, 1 he), Hanfangji (Stephaniae Tetrandrae Radix, 0.5 liang), Chuandahuang (Rhei Radix et Rhizoma, 1 liang, raw). The above herbs are finely ground into powder using a fine sieve. Mix with first vinegar to a suitable consistency, apply onto old soft cloth, and paste over the red areas surrounding the carbuncle head. Replace when dry. For the carbuncle head itself, grind Jisiqingdai (Indigo Naturalis mixed with other ingredients) separately, mix with a small amount of water, and apply frequently using a bird feather, ensuring it does not dry out.
To treat carbuncles and swellings, various wind-toxic heat swellings, back carbuncles (Fabei), and breast abscesses (Ruyong), the Chongtai San formula is suitable. Chongtai (Rehmanniae Radix Preparata, 1 liang), Huangqin (Scutellariae Radix, 1 liang, crushed), Chuandahuang (Rhei Radix et Rhizoma, 1 liang, raw), Yangtaogen (Akebiae Caulis Radix, 3 fen, crushed), Xiaoshi (Natrii Sulfas, 3 fen), Banxia (Pinelliae Tuber, 3 fen), Baizhi (Angelicae Dahuricae Radix, 0.5 liang), Chixiaodou (Vigna Semen, appropriate amount). The above herbs are ground into powder. Apply to the affected area mixed with vinegar to a suitable consistency, spread on old cloth or thin silk, and apply three times daily until the swelling subsides.
For carbuncles and swellings involving back carbuncles with redness and swelling, intolerable heat and pain, the Zige San formula is indicated. Zige (Polygoni Cuspidati Caulis, 3 liang, crushed), Chuandahuang (Rhei Radix et Rhizoma, 3 liang, raw), Bailian (Puerariae Radix, 3 liang), Xuanshen (Scrophulariae Radix, 3 liang), Huangqin (Scutellariae Radix, 2 liang), Chuan Shengma (Bupleuri Radix, 3 liang), Yu Baidpi (Ulmus Alba Cortex, 2 liang, crushed), Muxiang (Aucklandiae Radix, 2 liang), Chixiaodou (Vigna Semen, 3 fen), Huanglian (Coptidis Rhizoma, 3 liang). The above herbs are finely ground into powder. Mix with fresh cold water to a paste-like consistency and apply to the swelling; reapply when dry.
For treating eruptive carbuncles where the skin is abraded and exudate flows like fire, causing unbearable heat, apply this formulation to promote resolution: Dahuang San (Giant Rhubarb Powder). Chuandahuang (Rhei Radix et Rhizoma, 1 liang, raw), Chixiaodou (Vigna Semen, 1 liang), Muli (Concha Ostreae, 1 liang), Huanglian (Coptidis Rhizoma, 1 liang), Bailian (Puerariae Radix, 1 liang), Tugua Gen (Benincasae Caulis, 1 liang), Danggui (Angelicae Sinensis Radix, 1 liang, crushed and lightly stir-fried). The above herbs are finely ground into powder. Each time, mix with egg white, apply to old cloth, and paste on the swollen area. Replace when dry.
For carbuncles and swellings with toxic heat and pain, Xuanshen San (Scrophularia Powder) is used. Xuanshen (Scrophulariae Radix, 0.5 liang), Zige (Polygoni Cuspidati Caulis, 0.5 liang, crushed), Chuandahuang (Rhei Radix et Rhizoma, 0.5 liang, raw), Muxiang (Aucklandiae Radix, 0.5 liang), Juanbai (Selaginellae Herba, 0.5 liang), Chuan Mangxiao (Natrii Sulfas Depuratus, 0.5 liang), Huangyao (Dioscoreae Bulbillum, 0.5 liang), Zitan Xiang (Santalum Album Lignum, 0.5 liang, crushed). The above herbs are finely ground into powder. Mix with egg white to a suitable consistency and thinly apply to the affected area. If the sore has already ruptured, omit Mangxiao.
For carbuncles and swellings with toxic heat, redness, and pain: Chuandahuang (Rhei Radix et Rhizoma, 3 fen, raw), Xingren (Prunus Armeniaca Seed, 3 fen, peeled and raw-ground into paste), Yanhua (Sal Nitratum, 3 fen). The above herbs are finely ground into powder, combined with the almond paste, and ground uniformly. Mix with fresh cold water to a suitable consistency and apply to the swelling; replace when dry.
For back carbuncles associated with general carbuncles, apply Dahuang San (Giant Rhubarb Powder). Chuandahuang (Rhei Radix et Rhizoma, 1 liang, raw), Huangqin (Scutellariae Radix, 1 liang), Baizhi (Angelicae Dahuricae Radix, 3 fen), Hanshuishi (Halloysitum Rubrum, 1 liang), Bailian (Puerariae Radix, 1 liang), Huangbai (Phellodendri Cortex, 3 fen, crushed), Shigao (Gypsum Fibrosum, 1 liang), Chishizhi (Haematitum, 1 liang), Huanglian (Coptidis Rhizoma, 1 liang, de-rhizome). The above herbs are finely ground into powder. Mix with fermented grain juice to form a paste, apply thickly to the sore, and replace when dry.

The above herbs are finely ground into powder... [text missing in original, implied mixing and application].
For back carbuncles and sores where pus and blood have not yet formed, to dissipate swelling and qi, apply this formula: Shed Snake Skin (She Tuipi, 2 chi, burnt to ash), Egg White (Jizi Bai, 3 pieces), Yuntaizi (Brassica Napus Seed, 2 liang, powdered), Chixiaodou (Vigna Semen, 3 he, powdered), Rice Vinegar (Mi Cu, 3 he), Manjingzi (Viticis Fructus, 3 he, powdered), Fresh Machi Xin (Portulacae Herba, one handful), Soil from Hearth Center (Zaozhong Huangtu, 2 liang, powdered).
