Zhong'e (Sudden attack by evil qi) refers to patients suddenly experiencing fullness in the heart, abdomen, and viscera, with inability to vomit or defecate, resembling Gan Huoluan (Dry Cholera). This is commonly known as "Chong'e". It arises from incomplete spirit and excessive fear, leading to attacks by evil ghosts or attachment of pathogenic factors. Symptoms include depression and silence, talking in sleep, delirium, slander, cursing, exposing private matters without regard for suspicion, and frequently predicting future fortune or misfortune which comes true precisely on time. The patient may know thoughts before they arise, climb heights and cross dangers as if walking on flat ground, weep and moan, and avoid people. These manifestations are myriad, resembling intoxication or madness, and cannot be exhaustively listed. These are attributed to possession by spirits, demons, or various monsters, or触犯 (offending) taboos causing actions by land deities. They are not real entities; treatment should follow local customs and verification. Zhong'e occurs when human spirit is weak and suddenly attacked by ghostly or evil qi. When yin and yang are harmonious, and Ying and Wei (Defensive and Nutritive Qi) are balanced, the spirit remains firm, righteousness is strong, and evil cannot invade. If care is improper and spirit weakens, one becomes susceptible to ghostly toxic qi. Symptoms include sudden stabbing pain in the heart and abdomen, severe distress, and feeling like dying. In cases of Zhong'e with abdominal distension and fullness, a pulse that is tight, large, and floating indicates death, while a tight, thin, and faint pulse indicates survival. In cases of Zhong'e with hematemesis of several sheng, a rapid and thin pulse indicates death, while a floating, moderate, and rapid pulse indicates survival. After recovery from Zhong'e, if residual pathogenic factors remain stagnant and recur, it transforms into Zhu disease (chronic infectious disease).
