The classic texts state that Liver Marasmus (Zu Jue Yin Gan Nüe, malaria affecting the Foot Jueyin Liver Meridian) manifests in the meridians as lower back pain, fullness of the lower abdomen, and difficulty in urination, resembling Gong syndrome (Lóng, a condition characterized by difficult and painful urination), yet it is not true Gong; instead, there is frequent urination, a sense of fear, Qi deficiency, and a feeling of depression in the abdomen. When affecting the Zang organs, it causes the complexion to appear bluish-green with frequent sighing, and the patient’s appearance resembles that of a corpse; treatment involves needling the Foot Jueyin meridian. The Foot Jueyin Liver meridian circulates around the genitalia; when pathogenic factors lodge therein, it leads to lower abdominal fullness and urinary difficulty. The Liver is regarded as the "General," responsible for strategic planning; thus, disease results in fear and Qi insufficiency. Bluish-green is the color of the Liver. One should observe the meridians and Zang-Fu organs to guide treatment. After treatment induces sweating, it is referred to as Taiyin Marasmus, also known as Spleen Marasmus.
This formula treats disorders arising from Spleen dysfunction, including dysentery, marasmus with abdominal pain, weakness in the lower body, heavy sensation of the body, flaccidity of the feet, pain in the soles, food stagnation causing abdominal fullness, and inability to move the limbs. Lu Jingyuan’s prescription, previously used to treat marasmus, proved highly effective.

On the fifth day of the fifth lunar month, take an appropriate amount of garlic, crush it into a paste, mix with Fan Hong (Alum Red), and form pills the size of longan seeds. Take one pill per dose, chewing it down with garlic decoction.

Regarding Lung Marasmus, the *Neijing* states it causes coldness in the heart; when the cold is severe, heat follows. During the febrile phase, the patient is prone to fright and hallucinations. For Heart Marasmus, the *Neijing* states it causes mental restlessness, a strong desire for cold water, excessive chills with little heat, and treatment involves needling the Hand Shaoyin meridian. The Heart is the residence of the Shen (Spirit); pathogenic factors cannot easily invade it. If invaded, mental restlessness occurs. The desire for cold water arises because of internal Heat of the Heart Fire. The presentation of excessive chills with minimal heat is due to internal Heat coupled with external Cold. Treatment should focus on unblocking the Heart meridian and draining pathogenic Heat to achieve recovery.
