This is about the principle of Qi ascending and descending. A physician treated it as Stomach Cold, administering Dried Ginger (Ganjiang), Galangal (Liangjiang), Cinnamon Twig (Guigui), Aconite Root (Wutou), Prepared Aconite Root (Fuzi), Clove (Dingxiang), and Agarwood (Chenxiang). The condition worsened day by day, accompanied by thirst and dribbling urination. Yu Tun examined the six pulses, finding them surging (Hong), rapid (Shu), and slippery (Hua), and diagnosed it as Phlegm requiring clearing and transformation. He instructed to urgently decoct Bamboo Sap (Zhuli) for three days. Afterward, the thirst ceased, and urination normalized, but pain in the chest and knees persisted as before. He then administered juice extracted from Radish Seed (Luobozhi), half a bowl, which induced vomiting of half a sheng of phlegm. By night, the pain intensified, leading to cold limbs (Jue), which was precisely due to provoking the rampant pathogenic factor. The next day, one liang of Ginseng Rhizome Stalk (Shenlu) was decocted with reverse-flowing water, but it did not induce vomiting. Another day later, Decoction of Sophora Flavescens (Kushen) was taken, also without vomiting. Subsequently, Aconite Tip (Fuzijian) and Platycodon Rhizome Stalk (Jiegenglu) were used, still without vomiting. One morning, one qian of Veratrum Powder (Lilu) mixed with a small amount of Musk (Shexiang) and adjusted with Acidic Water (Suanjiangshui) was administered, finally inducing massive vomiting, which only stopped at dawn the next day. In total, a small bucket of thick fluid retention (Yin) was vomited out. The pain vanished, and recovery was achieved through conditioning.
Wang Shishan treated a fifteen-year-old girl suffering from palpitations (Xinji), feeling as if someone were chasing her. She had nowhere to hide; although held by her mother and guarded by maids indoors, she remained terrified and unable to sleep peacefully. Physicians attributed this to Heart disease, using An Shen Wan (Calming Spirit Pill) and Zhen Xin Wan (Settling Heart Pill) without effect. Wang diagnosed fine, weak, and slow pulses, stating, "This is a Gallbladder disease." He prescribed Wen Dan Tang (Warm the Gallbladder Decoction), after which she stabilized.
Someone asked, "Why does fear cause the skin to feel cold and trigger goosebumps?" I replied, "Fear causes Qi to sink. When Qi sinks, Yang Qi enters inwardly, causing this state. Once fear subsides and Qi returns, normalcy is restored."
Another question arose: "The previous case was also caused by fear. Since fear causes Qi to sink, why use Wen Dan Tang to descend Qi?" I answered, "This is a Shaoyang Gallbladder disorder, not caused by fear per se, but rather fear resulting from the illness. The Gallbladder requires warmth; deficiency leads to cold, cold leads to Qi stagnation, stagnation generates Phlegm. Phlegm in the Gallbladder organ prevents the Spirit from returning to its residence, causing fear and insomnia. Wang An stated: 'In this formula, Tangerine Peel (Chenpi), Pinellia Rhizome (Banxia), and Fresh Ginger (Shengjiang) are acrid and warm to guide Phlegm, thereby warming the Gallbladder; Zhi Shi breaks stagnation; Fu Ling removes fluid retention; Gan Cao harmonizes the Middle Jiao; Zhu Ru opens Spleen-Stomach depression and clears Lung Dryness. Cooling the Lung Metal soothes Jia Wood (Gallbladder).' Thus, neither cold nor dryness prevails, and the Gallbladder remains warm."
A man over seventy suddenly suffered from mental confusion (Maomei). His optic nerves became tense; closing his eyes induced drowsiness like dozing. He ignored tactile stimuli on his head and face, recovering after a short while. Occurring two or three times daily, treatment for Wind syndrome aggravated it. Wang’s pulse diagnosis revealed intermittent pauses (Jie). Upon waking, pulses were floating, large, rapid, and unresponsive to pressure; sitting up made them slightly smaller and less rapid. Pulse classics state: Rapid pulses indicate Heat, manifesting as Deficiency. Three days later, the left pulse was small and slippery, the right large and slippery. The Lung pulse specifically was floating and soft, sounding like hibernating insects upon pressure, differing from yesterday due to Deficiency. Yang Qi is clear, smooth, and harmonious. Overexertion or dietary excess disturbs Yang, transforming it into Evil Heat. The Spleen and Stomach rely on Yang Qi; when Yang transforms into hot blood, it boils and rises. Fainting results from heat scorching, confusing the Spirit and causing dizziness, resembling Stroke. Wind movement resembles Fire. When lower Jiao heat rises to block the chest and diaphragm, attacks occur. The Spleen meridian travels from foot to abdomen to chest; thus, rising lower heat is Spleen Fire. The chest and diaphragm belong to the Yang region where Clear Yang resides. Evil Heat disrupts this, preventing Yang from flowing freely, confusing the Heart and Spleen Spirits. Since all five Zang organs depend on Stomach Qi for nourishment, Fire damaging the Stomach deprives the Zang organs of support, causing their respective Spirits (Po, Shen, Hun, Yi, Zhi) to collapse into confusion. Attacks often occur around noon, a time of Yang generation; pulses are normal but floating and deficient. I diagnosed this as a Deficiency disease. Intermittent pulses during attacks indicate Blood deficiency and Qi weakness. Upon recovery, Qi and Blood flow smoothly, nourishing the Heart and Mind, restoring normal pulses. I prescribed Shiquan Dabu Tang (Ten Significant Tonifying Decoction) minus Cinnamon Twig (Rougui), adding Ophiopogon Tuber (Maidong) and Tangerine Peel (Chenpi). The patient recovered. Predicting a three-year prognosis, Wang noted that beyond this, outcomes were uncertain, which proved accurate.
A fifteen-year-old boy with sallow complexion suddenly vomited one to two bowls of stagnant Blood in December, stopping immediately. Pediatric treatment failed; he remained weak and dizzy. Three months prior, walking in hot weather caused continuous sweating, compounded by afternoon fatigue, leading to sudden fainting and unconsciousness with limb thrashing, lasting nearly an hour, recurring the next day. Later, attacks occurred daily around noon, progressively earlier (from Chen to Wu hours), increasing in frequency to three or four times, though slightly milder. Heat rose from lower Jiao to the chest, causing blockage and fainting, resolving after a long time. Initially suspected as Malaria evolving into Epilepsy. Physicians attributed it to Fire or Phlegm. Niuhuang Qingxin Wan (Coral and Bezoar Heart-Clearing Pill), ground with Bamboo Sap and Ginger Juice, was given twice (four pills total). Other phlegm-clearing and fire-purging decoctions reduced attacks to once daily. Symptoms included excessive sweating upon cessation, dry mouth, poor appetite, prolonged fever, and brief coolness. Wang diagnosed Yang Qi disturbed by Evil, failing to function at specific times. Excessive sweating depletes Jin Ye (Body Fluids); dry mouth indicates deficiency. Medications like Bezoar (Niuhuang), Cinnabar (Zhusha), Amber (Hupo), Arisaema (Nanxing), and Pinellia (Banxia) further dried fluids, exacerbating dryness and causing temporary relief followed by recurrence, failing to eradicate the root. Prescription: Ginseng (Renshen) [missing herb in source], each 2.5 qian; Polygala Root (Yuanzhi), Hawthorn Fruit (Shanzha), Ligusticum Rhizome (Chuanxiong), Scutellaria Root (Huangqin), each 0.7 qian; Gastrodia Tuber (Tianma), Ophiopogon Tuber (Maidong), Saposhnikovia Root (Fangfeng), Poria (Fushen), each 1 qian; Licorice Root (Gancao), Tangerine Peel (Chenpi), each 0.5 qian; Angelica Sinensis Root (Guishen) 0.8 qian; Atractylodes Macrocephala Rhizome (Baizhu) 1.5 qian. Decocted and taken for over ten doses, the disease did not recur.
Zi Hehe treated a woman over thirty who experienced nocturnal emissions involving ghosts, extreme terror, and visions of spirit halls, underworlds, boats, and bridges for fifteen years, remaining barren. Exorcisms, acupuncture, and moxibustion failed. She became emaciated, febrile, thirsty, and abdominally swollen. Zi Hehe diagnosed: "Yang Fire excess above, Yin Water excess below. Ghosts/spirits represent Yin spirits; spirit halls are Yin locations; boats/bridges relate to Water. Both Cun pulses were deep and hidden, indicating solid Phlegm in the chest." After three rounds of emesis (Yong), purgation (Xie), and diaphoresis (Han), dreams ceased within ten days, and pregnancy occurred within a month.
A woman laughed incessantly for half a year despite various treatments. Zi Hehe said, "Easy to treat." He used three liang of rock salt, heated red-hot, cooled, powdered, and decocted in one large bowl of river water for three to five boils. Taken warm in three doses, it induced vomiting of half a sheng of phlegm. Followed by a large dose of Huanglian Jiedu Tang (Scutellaria and Phellodendron Decoction to Clear Toxicity), laughter stopped in days.
Shen Zongchang treated a man from Linzi who claimed health but exhibited altered mentality, singing and laughing uncontrollably at night. Shen diagnosed: "Yin Fire attacking the Liver. Nighttime Yang activation requires softening agents with slight dispersants for cure."
Master Zhuang treated a patient ill from excessive joy. Zhuang feigned silence upon pulse-taking, claiming he needed to fetch medicine, then disappeared for days. The patient, despairing, bid farewell to relatives. Zhuang knew recovery was imminent, comforted him, and explained citing Suwen: "Fear conquers Joy," demonstrating profound understanding.
Chen Shanggou's *Zanyun Lou Zasuo* records: Elder Li from Luyi, Henan, a farmer, passed the provincial exam in Gui Mao year. His father laughed uncontrollably from joy. Upon passing the metropolitan exam in spring, laughter worsened. Ten years later, promoted to Censor, it became chronic. Initially intermittent, then ceaseless day and night. Worried, the Censor consulted the Imperial Medical Academy, devised a ruse: family told his father the Censor had died. The father grieved intensely for ten days, improving gradually. Then rumors spread the Censor was revived by Dr. Zhao. Li ceased grieving, and laughter never returned. This illustrates medical intent: excessive Joy injures; balancing with Sadness restores harmony, elevating skill to Dao.
Xue Ji treated a child with constant laughter, restlessness, red face, desire for cold drinks, and hot extremities. Two doses of Huanglian Xiexin Tang (Scutellaria and Gardenia Decoction to Drain Heart) calmed him slightly. Sixwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) decocted cured him instantly. Maintenance prevented relapse; discontinuation caused recurrence, resolved by resuming medication.
Scholar Xu treated a woman crying endlessly without cause. Exorcism failed. Xu recalled *Jingui Yaolue*: "Zang Zao" (Visceral Agitation) causing sadness, weeping like possessed states, yawning/stretching. Prescribed Ganmai Dazao Tang (Licorice, Wheat, and Jujube Decoction). Cured in several doses.
[Shen Yuan Note]: Classics state: "In Zang organs corresponds to Lung, in emotions corresponds to Sadness." "Essence and Qi merging in Lung causes Sadness." Joy is Yang, governed by Heart; Anger is Yin, governed by Liver. Women, being Yin-natured, experience less Joy and more Anger/Sadness. "Excess Spirit causes laughter; Deficient Spirit causes sadness." Children cry because their Spirit is immature.
Shi’s *Wenjian Lu* records: A military supervisor suffered sorrowful longing. Hao advised his son to induce palpitations for cure. The supervisor feared Official Li Songqing. Hao requested Li to reprimand the supervisor publicly. Fear and sweating cured the illness.
In Shaoxing Gui Chou year, Xu Shushi treated Dong Sheng in Siming. Dong suffered restless Spirit, flying soul during sleep, feeling body in bed while soul departed, palpitations, nightmares, insomnia. Doctors diagnosed Heart disease. Xu diagnosed Liver Meridian invasion via pulse: "Liver stores Hun (Soul). Normal sleep returns Hun to Liver. Pathogen in Liver prevents return, causing soul flight. Liver governs Anger; minor anger aggravates symptoms." Dong agreed. Xu prescribed Zhenzhumu Wan (Mother-of-Pearl Pill) and Duhuo Tang (Angelica Pubescens Decoction). Cure in one month. Pearl Mother enters Liver meridian first. Dragon Teeth (Longchi) calm Hun; Tiger Eyes (Hujing) settle Po. East Green Dragon (Wood/Liver/Hun) changes; West White Tiger (Metal/Lung/Po) stays still. Understanding these principles cures Soul/Flying issues.
Xu Zhongzi Shiwai reported: His father studied excessively, failed exams repeatedly, vomited blood profusely, became emaciated. Dreams of fighting, fear, loud screaming without voice for ten years, occurring every 5-7 nights or after overwork. Tonifying Heart Blood failed. Reading *Suwen · Zangqi Fashi Pian*: "Liver disease causes anger; deficiency causes blindness, deafness, fear of being chased." Realized it was "Hun Wandering." Night belongs to Liver; Liver stores Blood and Hun. Excessive bleeding starves Hun. Closing eyes triggers dream fights. Liver deficiency causes gallbladder timidity/fear. Severe tonification required. Herbal medicines insufficient. Used Deer Antler Glue (Lujiaojiao) 3 qian dissolved in wine morning. Sleep stabilized in 5 days, muscles grew in 15 days, Spirit restored in 1 month, enabling walking. Later used successfully for Blood deficiency, menorrhagia, vertigo. People know Deer Antler Glue tonifies better than Ginseng/Astragalus, but it severely tonifies Liver Blood; abundant Blood calms Hun.
Huang Shiwen treated a woman bedridden for three years, resembling Consumption (Lao Fei). Deficiency treatments failed. Huang diagnosed: "Ingestion of Yin food combined with fright." Patient recalled eating rice balls when hearing husband fell in water. Shock caused lingering illness. Huang gave Qingmuxiang Wan (Blue Musk香 Pill) plus purgative. Passed a clump of phlegm-wrapped rice ball. Unconscious shock lodged below; recovered thereafter.
One person with Shock Qi entering Heart collaterals lost speech. Medicated with Litharge (Mituo seng) powder mixed in tea. Cured. Another hunted by wolves in mountains cured similarly. A military officer bitten by snake cured likewise.
