[Yin Swelling]
[Dà] When women experience vulvar swelling, it is caused by deficiency and exposure to wind pathogenic factors. The Bao Luo (uterine collaterals) are deficient, allowing wind pathogens to invade. Wind qi attacks the yin area, clashing with blood and qi, causing qi stagnation and obstruction. The cou li (skin pores and interstices) become blocked, preventing the venting and discharge of fluids, thus resulting in swelling.
[Xue] If the aforementioned symptoms are due to deficiency of qi and blood, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) to lift and tonify. If it belongs to damp-heat in the Liver channel, use Longdan Xiegan Tang (Gentian Liver-Draining Decoction) to drain and clear. If accompanied by yin swelling, yin itching, yin coldness, or prolapse of the uterus (Yin Ting), refer to the subsequent discussion. A woman presented with swelling and stifling sensation in the vagina, astringent and difficult urination, swelling in both hypochondria, afternoon tidal fever, irregular menstruation, and occasional chills and fever. This was due to depression and anger affecting the Liver and Spleen, sinking of yuan qi, and stagnation of damp-heat. In the morning, she took Guipi Tang (Restore the Spleen Decoction) added with Chai Hu (Bupleuri Radix) and Sheng Ma (Cimicifugae Rhizoma) to resolve stagnation, tonify Spleen qi, and raise yuan qi. In the evening, she took Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) to clear Liver fire, generate Liver blood, and eliminate damp-heat. After several doses of each, all symptoms resolved. Subsequently, she took Sijunzi Tang (Four Gentlemen Decoction) added with Xiong Gui (Ligusticum Chuanxiong Rhizoma) and Dang Gui (Angelicae Sinensis Radix), along with Dan Pi (Moutan Cortex), to regulate and tonify the Liver and Spleen, allowing menstruation to resume on schedule.

Another formula: For severe vulvar swelling, crush Bian Cao (Patriniae Herba) and apply externally.
[Yin Itching]
[Dà] Vulvar itching in women is caused by parasitic erosion. The "Three Worms" reside between the intestines and stomach. Due to visceral deficiency, these worms become active and erode the interior of the yin. The worms generate heat; mild cases cause itching, while severe cases cause pain.
[Xue] The aforementioned symptoms stem from the Liver channel. Treat internally and externally with Longdan Xiegan Tang (Gentian Liver-Draining Decoction) and Xiaoyao San (Free and Easy Wanderer Powder). Crush Tao Ren (Persicae Semen) into a paste, mix with Xiong Huang (Realgar) powder or chicken liver, and insert into the vagina to control the parasites. A woman presented with chest and diaphragm discomfort, internal heat and thirst, poor appetite, limb fatigue, stifling and itching in the vagina, and red, astringent urine. This was caused by depression and anger injuring the Liver and Spleen. She recovered after taking Guipi Tang (Restore the Spleen Decoction) added with Shan Zhi (Gardeniae Fructus). Later, due to anger, she experienced pain and distension in the affected area and lower abdomen. She took Xiao Chai Hu Tang (Minor Blue Dragon Decoction) added with Shan Zhi, Xiong Gui (Ligusticum Chuanxiong Rhizoma), Dang Gui (Angelicae Sinensis Radix), and Bai Shao (Paeoniae Radix Alba). Pain ceased. She recovered after taking Xiaoyao San (Free and Easy Wanderer Powder) added with Shan Zhi (Gardeniae Fructus). Later, due to overwork, the affected area swelled, and urination remained astringent. She recovered after taking Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Shan Zhi (Gardeniae Fructus), Fu Ling (Poria), and Dan Pi (Moutan Cortex). A woman presented with pain and itching inside the vagina, intermittent discharge, poor appetite, and fatigue. This was due to deficiency of Liver and Spleen qi with downward pouring of damp-heat. She stabilized after taking Guipi Tang (Restore the Spleen Decoction) added with Dan Pi (Moutan Cortex), Shan Zhi (Gardeniae Fructus), Bai Shao (Paeoniae Radix Alba), and Sheng Gan Cao (Raw Glycyrrhizae Radix et Rhizoma). A woman presented with internal vaginal itching and pain, internal heat, fatigue, and poor appetite. This was due to depression and anger affecting the Liver and Spleen, damage to yuan qi, and damp-heat. She recovered after taking Ren Shen (Ginseng Radix), [missing character], Dang Gui (Angelicae Sinensis Radix), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Chen Pi (Citri Reticulatae Pericarpium), Chai Hu (Bupleuri Radix), Chao Shan Zhi (Stir-fried Gardeniae Fructus), Che Qian Zi (Plantaginis Semen), Sheng Ma (Cimicifugae Rhizoma), Bai Shao (Paeoniae Radix Alba), Dan Pi (Moutan Cortex), and Fu Ling (Poria). If there are parasites in the vagina causing itching and pain, it also belongs to Liver Wood pathology. Crush Tao Ren (Persicae Semen) and Xiong Huang (Realgar) and insert into the vagina to kill them, while continuing to use herbs that clear the Liver and resolve stagnation. Some use chicken liver insertion, which is a method to attract worms. One formula involves crushing fresh peach leaves, wrapping in cotton, and inserting into the vagina, changing two to three times daily.
Da Huang San (Rhubarb Powder) treats vulvar itching in women.
Da Huang (Rhei Radix et Rhizoma) (lightly stir-fried); Huang Qin (Scutellariae Radix); Huang Qi (Astragali Radix) (honey-fried, each 1 liang); Chi Shao Yao (Paeoniae Radix Rubra); Xuan Shen (Scrophulariae Radix); Dan Shen (Salviae Miltiorrhizae Radix et Rhizoma); Shan Zhu Yu (Corni Fructus); Chuang Zi (Cnidii Fructus) (each 0.5 liang).
Grind into fine powder. Take 2 qian before meals, mixed with warm wine.
Guangji Formula treats persistent vulvar itching in women.
Ran She Dan (Boa Gallbladder); Xiong Huang (Realgar); Liu Huang (Sulfur); Zhu Sha (Cinnabaris); Xiao Shi (Natrii Nitratis Crystal); Wu Yi (Pruni Semen) (each 0.5 liang); Li Lu (Veratri Radix) (2.5 qian).
Grind into fine powder and mix evenly. Combine with pig fat from the twelfth lunar month to form a paste. Make a bandage like a finger, 1.5 cun long. Apply medicine to it and insert into the vagina. Change once daily. When changing, wash with boiled water of Zhu Jiao Gen (Zanthoxyli Radix) (3-5 liang), slightly warm. Dry and insert. Effective.
Another formula: Use Xiao Ji (Cirsii Herba) in any amount. Boil in water for hot washing, three times daily.
Cui Shi's Formula for unbearable vulvar itching.
Burn Xing Ren (Armeniacae Semen Amarum) to ash. Wrap in cotton while hot and insert into the vagina. Change twice daily.
Another formula: Wash with garlic decoction. Another method uses Gou Qi Gen (Lycii Radix).
Another formula: Ye Lang Ya (Pyrethri Tenuifolii Radix) (2 liang, finely cut); Chuang Zi (Cnidii Fructus) (3 liang). Boil in 3 sheng of water for ten boils. Take hot.
Another formula: Insert hot chicken liver into the vagina. If parasites are present, they will be expelled.
Another formula: Cut Niu Gan (Bovis Cor) into 5-cun pieces. Tie with string and insert into the vagina. After half a day, worms enter the liver tissue; remove it. Pig liver also works.
Shenghui Fang (Holy Helper Formulas) treats vaginal itching and pain due to parasites, swollen eyes, yellow skin, abnormal sexual desire, leukorrhea with bleeding, shortness of breath, and craving for rich food. Use a 1-chi carp. Remove head meat. Grind bones into powder, roast until yellow-black. Mix with pig fat. Place in a silk bag. Insert into the vagina as usual. Stop when pain is felt at the site; worms will exit naturally.
[Yin Pain]
Qianjin Formula for persistent "Little Wife Marriage Pain" (dyspareunia/deep dyspareunia).

Another single-herb formula for "Little Wife Marriage Pain".
Niu Xi (Achyranthis Bidentatae Radix) (5 liang).

Hai Piao Xiao San (Cuttlebone Powder) for "Little Wife Marriage Pain".
Wouyu Gu (Sepiae Endoconcha) (2 pieces).
Roast one ingredient, grind into fine powder. Take with wine using a square-inch measure, three times daily.
Buyi Formula treats non-reducing vulvar swelling and "Little Wife Marriage Pain".
Dong Qing Ye (Ilicis Cornuti Folium); Xiao Mai (Tritici Semen); Gan Cao (Glycyrrhizae Radix et Rhizoma).
Equal parts. Decoct in water for washing.
Wei Shi Formula treats vulvar pain. Stir-fry Qing Yan (Halitum) until hot, wrap in cloth, and apply as a hot compress.
[Yin Sores]
[Dà] Vulvar sores in women are caused by the activity and erosion of the "Three Worms" or "Nine Worms". These worms reside in the intestines and stomach. If viscera are harmonious and qi and blood are abundant, they cannot cause harm. If meridians and collaterals are injured by labor, and intestines/stomach are deficient, worms become active and erode the yin. Mild cases cause itching or pain; severe cases cause sores. Diagnosis: if the Shao Yin pulse is slippery and rapid, it indicates sores in the vagina.
[Xue] The aforementioned symptoms result from emotional stagnation transforming into fire, injuring the Liver and Spleen, with damp-heat pouring downward. External signs include growths in the vagina resembling snakes, mushrooms, or cockscombs; wet, itchy sores; ulceration with discharge; or swelling, stifling, and dragging pain. Internal signs include fatigue, internal heat, irregular menstruation, loss of taste, afternoon tidal fever, chest/hypochondriac discomfort, urinary distension, red/white leukorrhea, and dribbling/difficult urination. Dang Gui Tang (Angelica Decoction) treats vaginal erosion sores. Whenever a woman's Shao Yin pulse is rapid and slippery, sores will inevitably appear in the vagina, named [missing character] sore. Symptoms include pain or itching like worm movement, dripping pus, and near-total erosion. This is caused by mental agitation and weak Stomach qi, leading to stagnant qi and blood flow. Thus, the Classics state: "All pain, itching, and sores belong to the Heart." And: "The Yangming governs the muscles." Since itching and pain belong to the Heart, treatment must tonify the Heart and nourish the Stomach, combined with external fumigation, washing, and suppository treatments.

[Yin Prolapse]
[Dà] Vaginal prolapse in women is caused by injury to the Bao Luo (uterine collaterals), deficiency-cold of the Zi Zang (uterus), or strain during childbirth.
[Xue] The aforementioned symptoms should primarily be treated by lifting and tonifying yuan qi. If Liver/Spleen stagnation causes qi deficiency and sinking, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). If Liver fire and damp-heat cause astringent urination, use Longdan Xiegan Tang (Gentian Liver-Draining Decoction). A woman presented with a mushroom-like protrusion from the vagina, swelling/pain around it, frequent urination, internal heat/afternoon tidal fever, itching/pain sensation, and heavy dragging sensation in urination. This was Liver/Spleen stagnation: Liver fire/damp-heat caused swelling/pain; Spleen deficiency/sinking caused heaviness. First, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Shan Zhi (Gardeniae Fructus), Fu Ling (Poria), Che Qian Zi (Plantaginis Semen), and Qing Pi (Citri Reticulatae Pericarpium Viride) to clear Liver fire and raise Spleen qi. Then use Jiawei Guipi Tang (Modified Restore the Spleen Decoction) to regulate Spleen stagnation. Externally, apply raw pig fat mixed with Li Lu (Veratri Radix) powder to achieve retraction. A woman had a 5-cun protrusion from the vagina, stifling pain/heaviness, dripping fluid, and astringent urination. At night, she took Longdan Xiegan Tang (Gentian Liver-Draining Decoction) to drain damp-heat; in the morning, she took Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) to raise Spleen qi. Symptoms gradually improved. Then she took Guipi Tang (Restore the Spleen Decoction) added with Shan Zhi (Gardeniae Fructus), Fu Ling (Poria), Chuan Xiong (Ligusticum Chuanxiong Rhizoma), and Huang Bo (Phellodendri Cortex), taken intermittently for regulation, leading to recovery.
