[Xue Ji] Chou Che (convulsions, referring to the contraction and shortening of tendons and vessels) refers to the tension and contraction of tendons and vessels; Che (convulsions, referring to the relaxation and extension of tendons and vessels) refers to the looseness and extension of tendons and vessels. One contraction and one extension cause the hands and feet to pull and twitch incessantly. Generally, this condition is similar to convulsions in infants, and is called Chou Che. This syndrome mostly belongs to Wind pathogen, as Wind governs movement and shaking. Luo Longji said: The Heart governs the vessels, the Liver governs the tendons; the Heart belongs to Fire, the Liver belongs to Wood; Fire governs Heat, Wood governs Wind. When Wind and Fire blaze together, Chou Che occurs. Treatment methods: If caused by Wind-Heat, use Gouteng Tang (Uncaria Decoction) with added Chai Hu (Bupleurum Root), Shan Zhi (Gardenia Fruit), Huang Qin (Scutellaria Root), and Bai Zhu (Atractylodes Rhizome) to calm the Liver Wood, descend Heart Fire, and nourish Qi and Blood. If Wind-Phlegm surges upward, add Zhu Li (Bamboo Sap), Nan Xing (Arisaema Rhizoma Cum Melle), and Ban Xia (Pinellia Tuber). If acute twitching is caused by Wind Evil, add Quan Xie (Scorpio) and Jiang Can (Bombyx Batryticatus). If due to deficiency of Qi and Blood, use Bazhen Tang (Eight Treasure Decoction) with added Gouteng (Uncaria Ramulus cum Uncis) and Shan Zhi as the main formula. If there is weak twitching, eyes rolling upward (Dai Yan), body arching backward (Fan Zhe), and sweat beading like pearls, this indicates Liver Qi exhaustion, which are all incurable cases. A pregnant woman could not extend her limbs; after taking formulas that dispel Wind and dry Blood, she suffered from fecal incontinence, excessive phlegm, and limb convulsions. I reasoned that this was due to Liver Fire and Blood Dryness, treated primarily with Bazhen Tang (Eight Treasure Decoction) plus stir-fried black Huang Qin (Scutellaria Root), assisted by Gouteng Tang (Uncaria Decoction), resulting in recovery. Later, due to anger, the previous symptoms recurred with hematuria, alternating chills and fever, insomnia, and poor appetite. She was treated with Gouteng San (Uncaria Powder) plus Shan Zhi (Gardenia Fruit) and Chai Hu (Bupleurum Root), stopping the bleeding. Jia Wei Xiao Yao San (Modified Free Wanderer Powder) was used, resolving the chills and fever and allowing sleep. Liu Jun Zi Tang (Six Gentlemen Decoction) plus Shao Yao (Peony Root) and Gouteng Gou (Uncaria Ramulus cum Uncis) was used, improving appetite and leading to gradual recovery.
