Pregnancy-related obstruction disease, according to Zan Yin's *Chanbao Fang* (Precious Formulas for Childbirth), is referred to as "Zi Bing" (Fetal Disease). *Chao Shi Bing Yuan* (Source of Diseases by Chao Yuanfang) calls it "E Zu" (Nausea and Obstruction of Pregnancy). If a woman has a weak constitution, or has wind-pathogens in her body, or has phlegm-fluid retention, she will suffer from this disease upon becoming pregnant. Her complexion remains unchanged, and her pulse is harmonious and smooth. However, she feels heaviness in her limbs, dizziness in the head and eyes, picky eating habits, aversion to food odors, and a craving for sour and salty tastes; severe cases may present with alternating chills and fever, mental restlessness, vomiting of phlegm-water, fullness and discomfort in the chest and abdomen, and a trance-like state where she cannot support herself. This applies regardless of whether it occurs early in pregnancy or later; the key difference lies only in the severity of the suffering. For mild cases, abstaining from medication is acceptable; for severe cases, medicinal treatment is necessary. *Qianjin Fang* (Essential Prescriptions Worth a Thousand Gold) contains two formulas specifically for treating obstruction disease: Banxia Fuling Tang (Pinellia and Poria Decoction) and Fuling Wan (Poria Pill). However, these two formulas are rarely used nowadays because Banxia (Pinelliae Rhizoma) has properties that can disturb the fetus. Since the fetus is just forming, there is concern about its potential to scatter or abort; thus, caution is imperative. Zhang Zhongjing's *Shanghan Lun* (Treatise on Cold Damage Disorders) states: "When a woman contracts wind-cold and subsequently develops alternating chills and fever at regular intervals, this indicates Heat entering the Blood Chamber (Xue Shi)." Some practitioners use Huanglong Tang (Yellow Dragon Decoction), while others use Xiao Chaihu Tang (Minor Bupleurum Decoction) with Banxia removed. These approaches seem designed specifically for pregnant women. Wang Ziheng proposed Baizhu San (Atractylodes Macrocephala Powder), and *Ju Fang* (Formulas of the Bureau) offers Renshen Dingxiang San (Ginseng and Clove Powder), both of which have proven effective. Although these three formulas are largely similar with minor differences, Yang Zhen proposed Renshen Jupi Tang (Ginseng and Tangerine Peel Decoction), and Qi Shiming proposed Xingpi Yin (Spleen-Awakening Decoction); I have tested them and found them effective. None of these formulas use Banxia or other herbs that might disturb the fetus. Practitioners should be aware of this.
For treating E Zu (Nausea and Obstruction) with vomiting of clear fluid, where in severe cases no porridge or liquid can be retained for over ten days:
Baizhu (Atractylodis Macrocephalae Rhizoma) (1 liang)
Renshen (Ginseng Radix et Rhizoma) (0.5 liang)
Dingxiang (Caryophylli Flos) (2.5 qian)
Gancao (Glycyrrhizae Radix et Rhizoma) (1 qian)
Grind the above ingredients into a fine powder. Take 2 qian per dose. Add one zhan (approx. 200ml) of water and five slices of Shengjiang (Zingiberis Rhizoma Recens), decoct until reduced to 0.7 zhan, and take warm.
For treating obstruction disease with vomiting of phlegm-water:
Renshen (Ginseng Radix et Rhizoma) (remove Lu/roots)
Chen Juhong (Citri Reticulatae Pericarpium Rubrum)
Baizhu (Atractylodis Macrocephalae Rhizoma)
Mendong (Ophiopogonis Radix) (remove heart, each 1 liang)
Gancao (Glycyrrhizae Radix et Rhizoma) (3 qian)
Houpo (Magnoliae Officinalis Cortex) (processed)
Fuling (Poria) (remove skin, each 0.5 liang)
Grind the above ingredients into a coarse powder. Take 4 qian per dose. Add one and a half zhan of water, a piece of Danshuzhu Ru (Bambusae Caulis in Taenia) the size of a bullet, and three slices of Shengjiang (Zingiberis Rhizoma Recens). Decoct until reduced to 0.7 zhan, strain, clarify, and take warm. Take on an empty stomach or before meals. *Ji Yan Fang* (Collected Proven Prescriptions) omits Fuling, Mendong, and Gancao.
