Discussion: What is the condition of fetal death in the uterus caused by heat disease? The answer is: Because the mother suffers from a heat disease, after six or seven days, the zang-fu organs become extremely hot, steaming and cooking the fetus like boiling water, leading to its death. Since the fetus has died, its body becomes cold and cannot expel itself; one should only administer Heishen San (Black Divine Powder) to warm the uterus, and within a short time, the fetus will be expelled naturally. How does one know that the fetus has already died? By observing if the parturient's tongue is cyanotic; this is the sign.
(Also known as Wujin San [Black Gold Powder]. Named Rougui San [Cinnamon Twig Powder] in *Lingyuan Fang*)
Guixin (Cortex Cinnamomi), Danggui (Radix Angelicae Sinensis), Shaoyao (Radix Paeoniae Alba), Gancao (Radix Glycyrrhizae Preparated with Honey), Jiangjiang (Rhizoma Zingiberis Preparated), Shengdihuang (Radix Rehmanniae Preparata) (each 1 liang), Heidou (Semen Sojae Nigrum, stir-fried and peeled, 2 liang), Fuzi (Radix Aconiti Lateralis Preparata, peeled and de-navel, 0.5 liang).
The above are ground into fine powder. Take 2 qian per dose, on an empty stomach, mixed with warm wine. (Another prescription excludes Fuzi and includes Puhuang [Pollinis Typhae]).
Chen Wuyue commented: Pregnancy is called "chongshen" (double life), as two lives depend on it! Improper management can damage the fetus, not only due to heat disease steaming. It may also result from sudden falls or fright, exposure to external pathogens during travel, or pre-existing zhengjia (masses) and jijie (accumulations), which most often damage the fetus. The sign is a cyanotic tongue, indicating fetal death. *Yangtai Lun* states: If the face is cyanotic and the tongue red, the mother dies but the fetus lives; if the lips are cyanotic and froth is vomited, both mother and fetus perish. In cases of twin pregnancy, where one dies and the other lives, the signs are difficult to discern. Unless changes are observed at the time of delivery, it is hard to predict beforehand; thus, thorough study is essential. However, warming the uterus with Heishen San is inferior to supporting the parturient, ensuring her qi is upright, preventing deficiency and exhaustion, so that the fetus descends naturally. Cuansheng Tang is superior to Heishen San.
Treats fetal death in the uterus, or parturient qi deficiency and prostration, dryness and涩ness of the birth canal.
Cangzhu (Radix Atractylodis Macrocephalae, soaked in rice water, washed, stir-fried, 2 liang), Kujie (Radix Platycodonis, note: text says 苦梗 which is usually Jiegeng, here likely typo for 桔梗 Radix Platycodonis or refers to specific preparation, standard is Jiegeng), Chenpi (Pericarpium Citri Reticulatae, 6 qian), Baizhi (Radix Angelicae Dahuricae), Guixin (Cortex Cinnamomi), Gancao (Radix Glycyrrhizae Preparated with Honey) (each 3 qian), Chuanxiong (Rhizoma Chuanxiong, 1.5 qian), Chuanwu (Radix Aconiti Kusnezoffii), Danggui (Radix Angelicae Sinensis), Jiangjiang (Rhizoma Zingiberis Preparated), Houpo (Cortex Magnoliae Officinalis Prepared), Shaoyao (Radix Paeoniae Alba), Fuling (Poria), Banxia (Rhizoma Pinelliae Preparated with Ginger and Alum), Fuzi (Radix Aconiti Lateralis Preparata, peeled), Nanxing (Rhizoma Arisaematis Preparated) (each 2 qian), Zhike (Fructus Aurantii Preparated, 4 qian), Muxiang (Radix Aucklandiae, 1 qian), Xingren (Semen Armeniacae Amarum, peeled and de-husked, stir-fried), Ejiao (Colla Corii Asini, stir-fried) (each 1 fen).
The above are ground into fine powder. Take 1 daqian per dose, mixed with warm wine. If feeling heat and oppression, add Baimi (White Honey) and mix with fresh well water. This formula is essentially Wuji San (Five Accumulations Powder) without Mahuang (Ephedrae Herba), but adding Chuanwu, Fuzi, Nanxing, Ejiao, Muxiang, and Xingren, totaling six additional herbs.
